Dissolution rate measurements of sparingly soluble compounds with the Coulter Counter model TAII.

A Coulter Counter, Model TAII was used to determine both solubility and dissolution rate from suspensions of griseofulvin and hydrocortisone acetate, where the initial particle size and shape had been characterized. The method assesses the weights and surface areas of particles remaining undissolved as a function of time. A comparison with a conventional method, assessing dissolved quantities of materials, showed that solubility data of the same order of magnitude can be obtained. The TAII data fitted a previously established equation that linearly relates log solubility and log of the intrinsic dissolution rate and indicated that the dissolution process was not diffusion controlled. The TAII method described is a rapid and convenient means of estimating the solubility and dissolution rate where no adequate technique for analysing the dissolved fraction exists.

Prognostic importance of digitalis after acute myocardial infarction.

Because previous reports have suggested that digitalis administration may lead to increased mortality after hospital discharge for acute myocardial infarction, the independent importance of digitalis therapy in long-term prognosis after acute myocardial infarction was investigated by analyzing 1,599 patients after definite myocardial infarction. After hospital discharge, mortality rate for the entire group at 4 months was 7.7% and after 1 year 14.2%. At discharge, 36.6% of the patients were taking digitalis. Compared with those not taking digitalis, those taking digitalis had more historical risk factors and a higher incidence of important clinical prognostic variables during the hospitalization. Their cardiac mortality rate after 4 months and 1 year (12.5 and 22.4%, respectively) was significantly higher than that of patients not taking digitalis (5.0 and 9.6%, respectively). Mortality was higher for patients taking digitalis whether or not they had congestive heart failure during hospitalization. However, in a multivariate Cox analysis for 1 year outcome, neither digitalis nor any other medication variable displaced the important clinical variables of age, congestive heart failure during the hospitalization, previous myocardial infarction, maximal heart rate during the hospitalization and previous angina. Quinidine and digitalis at discharge were selected sixth and seventh (not significant) by the analysis. It is concluded that digitalis therapy at discharge after myocardial infarction was not an independent predictor of late mortality in these patients.

A study of the information-seeking skills of medical students and physician faculty.

In the research reported here, the authors compared (a) the abilities of third- and fourth-year students and physicians to solve problems requiring review of current literature with (b) the information-seeking procedures of the three groups in researching those problems. The subjects were given a patient care question to answer, and logs were maintained to document their information-seeking processes. The findings indicated no differences among the three groups in accurately solving patient care problems; a difference in information-seeking processes between third-year students and physicians; and no differences between third-year and fourth-year students or between physicians and fourth-year students in their approaches to seeking information. Mean scores indicated that all three groups need additional training on accessing the literature in making patient care decisions.

[A few remarks on the subject of correct planning of epidemiologic studies].

The paper presents some problems on the planning of epidemiological studies. The definition of epidemiology is related to medicine, statistics, sociology, demography and other branches of science. Different types of epidemiological studies are enumerated (descriptive, analytical and experimental epidemiology, retrospective and prospective analysis). In the plan of epidemiological studies fifteen stages are distinguished and described. Special attention is given to the aim and object of research, sources of data, methods and techniques of evaluation, presentation and the analysis of results. The consequences of working without plan or with incomplete and wrong plan are presented. The authors suggest how to develop modern epidemiology in Poland.

Assessing patient evaluation skills: a structured versus traditional approach.

The purpose of the study on which this article is based was to determine differences in interviewing skills between third-year medical students who were provided structured, evaluative feedback and those assessed via traditional feedback methods. Experimental group students received a written assessment for every orthopedic hospital patient worked up, while the control group received traditional assessment feedback. Comparison measures were made based on student performance on a simulated patient examination. Results indicated that students who were provided structured feedback scored significantly higher (p less than .05) than those assessed by traditional assessment means.

Antiinflammatory activity of the new aminomethyl derivatives of 1-cyclohexyl-5-alkyl and 1-cyclohexyl-5,5-dialkylbarbituric acids.

Several new aminomethyl derivatives of barbituric acid were evaluated pharmacologically. 1-Cyclohexyl-5-piperidylmethyl-5-allylbarbituric acid and 1-cyclohexyl-3-morpholinomethyl-5,5-diallylbarbituric acid displayed antinflammatory activity in the carrageenin-edema and xylene-tests but did not produce analgesia. It has been also found that introduction of aminomethyl groups into the antiinflammatory barbituric derivatives brings about elevation of the resorption of the tested compounds.

Antioxidants as agents reducing the toxicity of indomethacin.

Simulataneous oral administration of butylhydroxytoluene (BHT) (50 or 100 mg/kg) with indomethacin (IND) (3, 6 or 20 mg/kg) prevented the damaging effect of IND on the rat gastric mucosa. Butylhydroxyanisole (BHA) and nordihydroguiaretic acid (NDGA) rather potentiated than prevented the noxious action of IND. A reduction of IND toxicity without affecting its anti-inflammatory action seems to be possible to achieve by concomitant administration of IND with BHT.

Pharmacologic properties of some derivatives of N-(2-carboxyphenyl)-4-phenoxyacetamide.

Three new inhibitors of prostaglandin (PG) synthetase: N-(2-carboxy-4-chlorophenyl)-phenoxyacetamide (ZR 29), N-(2-carobxy-4-chlorophenyl/-bromophenoxyacetamide (ZR-32), N-(2-carboxy-4-chlorophenyl)-4' - nitrophenoxyacetamide (ZR-35), in doses of 30-100-200 mg/kg p.o. inhibited edema in an acute inflammatory model, ZR-32 inhibited formation of granuloma. In the chronic inflammatory model, ZR-32 exerted strongest activity in the second and third weeks. ZR-32 demonstrated strong analgesic activity in the test of stimulation with electric current and in the "hot plate" test, besides antipyretic activity. Both compounds weekly irritated the gastric mucosa. In the tests applied, these compounds had no effect on the CNS. Their acute to toxicity was substantially lower than that of acetylsalicyclic aid and indomethacin.
