Accuracy of subcutaneous fat measurement: comparison of skinfold calipers, ultrasound, and computed tomography.

OBJECTIVE: The purpose of this study was to compare skinfold caliper and ultrasound measurement of subcutaneous body fat at three abdominal sites with computed tomography, which is considered to be the gold standard. DESIGN: This was a cross-sectional study in which computed-tomography, ultrasound, and skinfold caliper measurements were made at three distinct abdominal sites. All body composition and anthropometric measurements were performed on each subject on one occasion. SUBJECTS: Twenty-two subjects were recruited (13 men and 9 women). Mean ages (+/- standard deviation) were 43 +/- 4 years for the women and 51 +/- 18 years for the men. All subjects had been previously scheduled for an abdominal or pelvic computed-tomography scan at the Department of Radiology, St Paul's Hospital, Vancouver, British Columbia, Canada, and participated in the study on a volunteer basis. MAIN OUTCOME MEASURES: A better agreement was found between the skinfold calipers and computed-tomography methods than between the ultrasound and computed-tomography method for the measurement of subcutaneous body fat. This was observed when the data were analyzed for both correlational agreement and for graphical interpretation. STATISTICAL ANALYSES PERFORMED: The relationships among skinfold, ultrasound, and computed-tomography measurements were analyzed by determining Pearson correlation coefficients. A graphical method described by Bland and Altman was also used to assess agreement among the three methods. RESULTS: Significant correlation coefficients were observed between skinfold calipers and computed tomography at all three abdominal sites (site 1, r = .60, P = .003; site 2, r = .70, P = .0001; site 3, r = .73, P = .0001). Ultrasound and computed-tomography methods only showed a significant correlation at site 3 (r = .54; P = .009). The graphical method revealed that the variation in the ultrasound measurements was much greater than that of the skinfold measurements when compared to computed-tomography values. APPLICATIONS/CONCLUSIONS: The results of this study indicated that relative agreement in the measurement of subcutaneous body fat between skinfold and computed-tomography measurements was superior to that exhibited between ultrasound and computed-tomography measurements. This finding enhances the potential use of skinfold calipers in the clinical setting, particularly in view of the fact that measurement of subcutaneous body fat at different body sites is becoming increasingly important for the characterization of risk of certain disease states.

Transcervical tubal cannulation and falloposcopy for the management of tubal pregnancy.

This is the first report of transcervical salpingoscopic visualization of tubal pregnancy in two patients. The falloposcope was introduced through a catheter used routinely for transcervical tubal cannulation, guided by tactile impression. We have previously demonstrated that it is possible to diagnose and treat tubal pregnancies via a transcervical intra-Fallopian cannula. Falloposcopy could help select appropriate patients for transcervical intra-Fallopian therapy by verifying the site of implantation and the characteristics of the ectopic pregnancy.

The incidence of late-onset congenital adrenal hyperplasia due to 3 beta-hydroxysteroid dehydrogenase deficiency among hirsute women.

OBJECTIVE: The present study was designed to determine the incidence of 3 beta-hydroxysteroid dehydrogenase deficiency (3 beta-HSD) in adult women with hyperandrogenism. DESIGN AND PATIENTS: In 78 hirsute patients and 30 normal women in the same age range, an ACTH stimulation test was performed on day 5 of the cycle by administration of a single bolus of 0.25 mg ACTH-(1-24) at 0800 h. MEASUREMENTS: The following steroids were measured before, 30 and 60 minutes after ACTH injection: delta 5-pregnenolone (delta 5-P), 17-hydroxy-delta 5-pregnenolone (17-OH delta 5-P), dehydroepiandrosterone (DHEA), delta 5-androstenediol, progesterone (P), 17-hydroxyprogesterone (17-OHP), androstenedione (A), testosterone (T) and cortisol. RESULTS: Maximum ACTH-stimulated values of delta 5-steroids were in excess of the 90% confidence limits of the control group in 19 hirsute women. Ten patients had an isolated increase in delta 5-P, 17-OH delta 5-P, DHEA or delta 5-androstenediol. Nine patients had an increase in two delta 5 steroids and none had increased values of three or four delta 5 steroids. The ratios of 17-OH delta 5-P to 17-OHP, DHEA to A, delta 5-P to P and delta 5-androstenediol to T were increased in 5, 1, 1 and 1 patients respectively. No patient had elevated values of more than one ratio. CONCLUSIONS: Using stringent diagnostic criteria, partial 3 beta-HSD deficiency was excluded in all 78 patients and therefore appears to be a rare disorder.

3-Hydroxysteroid Dehydrogenases

Treatment of endometriosis with a delayed release preparation of the agonist D-Trp6-luteinizing hormone-releasing hormone: long-term follow-up in a series of 50 patients.

Fifty patients with proven endometriosis were treated for 6 to 9 months with a delayed release preparation of microcapsules of the luteinizing hormone-releasing hormone (LH-RH) agonist D-Trp6-LH-RH, injected intramuscularly at monthly intervals. After a transitory ovarian stimulation at the onset of treatment, serum estradiol was suppressed to menopausal levels (50 pg/mL). This state of hypogonadism was reversible after the discontinuation of treatment, and menses resumed within 4 months after the last injection. Pelvic pain was relieved during treatment in 87.5% of patients. After a follow-up period of up to 37 months, 24 patients are in clinical remission and 9 experienced recurrence of endometriosis 7 to 14 months after completing treatment. One patient failed to respond to therapy with the agonist and 7 patients were lost to follow-up. Among 16 previously infertile patients with no other factors contributing to infertility, 7 became pregnant; 2 of these pregnancies were the result of gamete intrafallopian transfers. An eighth patient without documented infertility also conceived spontaneously. Side effects due to hypoestrogenism were reported by nearly all patients. In conclusion, D-Trp6-LH-RH microcapsules are effective and easily-administered agents for the treatment of endometriosis.

Diagnosis and treatment of ectopic pregnancy by retrograde selective salpingography and intraluminal methotrexate injection: work in progress.

Selective retrograde trans-cervical salpingography was carried out in four patients in whom ectopic pregnancy was suspected. The Fallopian tube was catheterized using a catheter set designed in our centre. The 6F catheter was placed in the cornua and an inner coaxial catheter was easily introduced into the proximal portion of the Fallopian tube, guided by tactile impression. Contrast medium was then injected and a round haloed mass was seen. Thereafter, the Fallopian tube was catheterized using a metallic guidewire until its tip was adjacent to the ectopic pregnancy. The coaxial catheter was then advanced along the guidewire and after removal of the latter, 5-35 mg of methotrexate was injected into the Fallopian tube. This new procedure was simple, well tolerated and had no side effects. A tubal pregnancy was demonstrated and surgery was avoided in all cases.

Metabolic effects of combined cyproterone acetate and percutaneous 17 beta oestradiol after six and twelve months therapy in 61 patients.

The metabolic effects of combined cyproterone acetate (50 mg) and percutaneous 17 beta oestradiol were studied during one year in 61 patients admitted for hyperandrogenia. Before treatment and at 6 and 12 months the following tests were performed: oral glucose tolerance test (OGTT) with insulinemia dosage, determination of total cholesterol, LDL, VLDL and HDL fractions, triglycerids, A1 and B apoproteins, liver function tests: bilirubinemia, alkaline phosphatases, transaminases and gamma glutamyl transferases. The patients' mean age was 27.0 +/- 6.8 years, the body mass index was 22.4 +/- 3.5 kg/m2. After one year of treatment the body mass index was not modified. Blood glucose slightly increased during OGTT; at 6 months this was significant at +30 minutes, at 12 months at +30, +60 and +90 minutes (P less than 0.05). There was no variation in insulinemia during OGTT. Total cholesterol decreased significantly at 6 and 12 months (P less than 0.001), this was associated with a decrease in HDL cholesterol, but without modification of the LDL + VLDL/HDL ratio. Decrease in HDL cholesterol was associated with a significant decrease in A1 apoproteins. No change in triglycerids and in liver function tests was observed at either date. In conclusion the metabolic effects of this association are described. These effects are minimal compared to those observed with cyproterone acetate and ethinyl oestradiol association in the literature. However, attention should be drawn to the possibility of glucose intolerance and decrease in HDL cholesterol and A1 apoproteins.

[Immunocytological distribution of HP1 group protamines in human testes and ejaculated spermatozoa].

Specific antisera have been obtained against the purified human sperm protamine HP1b. The specificity of antibodies was assessed in dot-blot and western-blot assays. Antibodies showed high binding to protamine HP1 group. Nevertheless cross-reactivity between HP1 and the other protamines (HP2 - HP3 - HP4) could not be excluded. Immunohistochemical studies revealed that protamines HP1 were detected in nuclei of ejaculated sperm after chromatin decondensation. In testis, they appeared in nuclei of elongating spermatids.

[Traumatic aorto-right ventricular fistula. Apropos of a case].

The authors report the case of a 19 year old young man with an aorto-right ventricular fistula caused by a stab wound. The diagnosis was not made initially during surgery to control haemorrhage (left hemothorax due to a damaged internal mammary artery). However, three weeks later, a continuous murmur was detected and the diagnosis established by echocardiography and cardiac catheterisation. The lesions were repaired by open heart surgery under cardiopulmonary bypass. The authors describe the main features of this condition based on their own experience and a review of the literature.
