Instrumentation for the continuous measurement of gas exchange and ventilation of infants during assisted ventilation.

A system of instrumentation for continuous measurement of oxygen consumption (VO2), carbon dioxide production (VCO2) and minute ventilation (V1) in human newborns on assisted ventilation is described. VO2 and VCO2 are measured by open-circuit indirect calorimetry utilizing a Servomex OA 184 differential paramagnetic oxygen analyzer and a Beckman LB 2 infrared CO2 analyzer. Minute volume is measured with a body plethysmograph. Bench performance is described, limitations of the system are defined, and sample clinical data are presented. The instrumentation can measure safely, accurately, and continuously physiologic variables in sick infants on assisted ventilation.

Preservation of platelets during extracorporeal circulation in sheep. A comparison between aspirin and sulfinpyrazone.

We studied the effect of aspirin and sulfinpyrazone on changes in platelet count, thromboxane B2 production, and pulmonary vascular resistance following the onset of veno-venous membrane oxygenator perfusion in sheep. Perfusion under identical circumstances was performed in three groups of animals, with one group serving as controls, one group receiving pretreatment with 1.5 grams of sulfinpyrazone iv, and one group receiving pretreatment with aspirin 50 mg/kg, iv. Both aspirin and sulfinpyrazone eliminated the rise in thromboxane B2 and the increase in pulmonary vascular resistance seen in control animals. Platelet preservation was improved significantly with sulfinpyrazone pretreatment, but not affected by aspirin. We conclude that sulfinpyrazone and aspirin have different mechanisms of action as far as platelet preservation during extracorporeal perfusion is concerned. The major difference may be sulfinpyrazone's ability to reduce platelet adhesion to the membrane surface. Since both aspirin and sulfinpyrazone eliminated the pulmonary vascular-response, but only sulfinpyrazone preserved platelets, it is apparent that the pulmonary vascular response is more closely related to thromboxane synthesis than to platelet disappearance.

Changing trends in hypertension detection and control: the Chicago experience.

Of 177,692 persons screened in 1977 as part of an ongoing City-Wide Hypertension Screening Program in Chicago, 14,988 (8.4%) had diastolic blood pressure (BP) greater than or equal to 95 mm Hg as compared to 13.2% of a similar population in 1976. Only 7% (3,910) of the hypertensive population (diastolic BP greater than or equal to 95 mm Hg or presently on antipressor drugs) had previously undetected hypertension in contrast to 11.9% (4,184) the year before and 48.7% in the same community in 1972. Conversely, 73.2% (40,738) had adequately controlled blood pressure as contrasted to 59.3% (20,897) the previous year and 20.6% in 1972. Of the remaining hypertensives, 7.5% (4,201) were known but not treated and 12.3% (6,824) were under treatment but not controlled in contrast to 12.1% (4,251) and 16.8% (5,905) respectively the year before. This upward trend in controlled hypertension was present in all strata of the population.

[Reproducibility of the jaw relation record].

The jaw relations in edentulous patients were determined with the use of the gotic arch and wax bite blocks. To compare the different methods interchangeable tracing devices and wax registrations were used. This apparatus was alternatingly used by various investigators, but with the same base plate. A quick and reliable survey of the results was possible with this experimental procedure. Our study confirmed the superior reproducibility of the graphic method with centrally supported needle.
