Basaloid squamous carcinoma of the head and neck. Report of a case occurring in the anterior floor of the mouth.

Basaloid squamous carcinoma was initially characterized in 1986, and it has been an uncommon entity, with 86 reported cases occurring in the head and neck. Although histologically associated with squamous cell carcinoma and squamous atypia, basaloid squamous carcinoma has distinctive clinical features. It is frequently diagnosed at an advanced stage, often with distant metastases, and it may be associated with second primary tumors. This tumor most commonly occurs in the base of the tongue, hypopharynx, and larynx. We report a case of basaloid squamous carcinoma that occurred in the anterior floor of the mouth--a distinctly unusual location.

Craniofacial resection for intracranial inverting papilloma and frontal sinus mucocele.

Inverting papilloma is a locally aggressive neoplasm; however, intracranial extension is very rare. The authors herein report a case of massive inverting papilloma with intracranial extension and an associated frontal sinus mucocele, causing marked frontal lobe compression. Treatment consisted of combined anterior craniofacial resection. Magnetic resonance imaging provided accurate preoperative differentiation between neoplasm and inflammation. Skull base reconstruction may prove more difficult when there is significant frontal dead space resulting from chronic brain displacement by an associated mucocele. The management of this unusual neoplasm and a review of the literature is presented.

A devitalized tooth as a complication of septorhinoplasty.

Complications of septorhinoplasty often involve adjacent structures, resulting in cosmetic as well as functional abnormalities. Dental complications of nasal surgery are rare. We describe a patient who underwent septorhinoplasty with a postoperative complication of a devitalized tooth. To our knowledge, this is the first report of such a dental complication. Bony manipulation in the region of the nasal spine may have resulted in the interruption of the neurovascular supply to the right central incisor, yielding a devitalized tooth. An aberrant vascular supply, or root apex situated high in the premaxilla, can predispose to dental injury while performing bony manipulation near the nasal spine. Preoperative evaluation of patients who are at risk for dental complications may allow the surgeon to safely achieve the functional and cosmetic goals of septorhinoplasty.

Paragangliomas of the head and neck.

Nonchromaffin paragangliomas of the head and neck are usually benign, slow-growing lesions arising from neural crest tissue. Although uncommon, these neoplasms should always be considered in the differential diagnosis of a neck mass. The diagnostic and therapeutic challenges posed by these lesions require continued clinical attention.

Characteristics of a coupled cell-free transcription and translation system directed by vaccinia cores.

1. A coupled transcription and translation system is described in which protein synthesis is directed by mRNA synthesised in situ by vaccinia virus cores. The cell-free system is based on a micrococcal-nuclease-treated reticulocyte lysate. 2. The polypeptides made in vitro include many authentic early vaccinia proteins, but also other proteins which were not detected in infected cells. 3. Concentrations of cores which inhibit host cell protein synthesis in vivo caused a delayed inhibition of translation in vitro; this was partly, but not entirely, due to dsRNA associated with the cores. 4. The mRNA made was methylated by core enzymes. Inhibition of methylation reduced the rate of translation tenfold; unmethylated RNA bound ribosomes poorly, but was nevertheless translated faithfully.
