[Association of Behçet's disease and pelvispondylitis in a subject without HLA W 27 antigen].

The authors report a new case of Behcet's disease in association with ankylosing spondylitis. A study of the HLA system in this patient, showed the presence of an HLA 5 (Behcet's disease) and the absence of HLA W 27 (ankylosing spondylitis). The authors recall the frequency of HLA W 27 during ankylosing spondylitis, whether primary or associated with psoriasis, or enteric disease. Desensitisation to autologous saliva proved a failure. A trial of D-Penicillamine produced no beneficial effect on the course of the mucosal ulcers and the inflammatory joints.

[Lupus syndrome during treatment with isoniazid].

In a young girl, aged 17 years, there occurred, 9 months after starting treatment with isoniazid and rifampicin, clinical signs of rheumatoid arthritis with facial erythema of vespertilio type. The presence of L.E. cells and antinuclear antibodies with a high titer, the rapid disappearance of all the clinical signs on stopping isoniazid, and the transient exacerbation of the syndrome on giving a single dose of isoniazid, gave the diagnosis of systemic lupus erythematosus induced by isoniazid. 18 months after stopping the drug, the patient is apparently cured, but there persists in the serum antinuclear antibodies in low concentration (1/200). A study of acetylation of isoniazid in this patient and her 3 sisters, showed that they were all of the slow acetylating phenotype. The late prognosis of this disease and the physiopathological value of the slow acetyl phenotype are worth discussion.

[Interference of noramidopyrin and its derivatives on the dosage of uric acid].

Administration of noramidopyrine or of noramidopyrine-methane-sulfonate interferes with the determination of blood uric acid by both the colorimetric method of Folin-Denis and an enzymatic uricase peroxidase method. This interference which is observed at least during the 24 hours following intravenous administration of noramidopyrine, results in either an increase (Folin-Denis) or a decrease (uricase) of the intensity of the final colour. The same effects are also observed in vitro when one adds noramidopyrine to an aqueous solution of uric acid. Existence of this interference should be taken into consideration especially in the diagnosis, the prognosis and the analgesic treatment of renal lithiasis.

[Retrospective study of 594 HIV-1 seropositive patients seen in 8 hospitals of western Paris].

This multicenter retrospective study concerns 594 cases of HIV-1 positive patients. The majority were asymptomatic. Some had Aids-related complex and some had AIDS. They were all seen in seven hospitals of the western outskirts of Paris and in one Paris hospital between 1983 and 1988. There was a predominance of homosexuals and of cases of AIDS in the center of Paris. Intravenous (IV) drug users and asymptomatic cases were more numerous in the city outskirts, a reflection of the populations at risk around each hospital. The proportion of cases of AIDS was lower among IV drug users than among homosexuals or bisexuals probably due to the fact that the viral infection was discovered at an earlier stage in a population contaminated at a later date or to the tendency to ask for the test "systematically" in drug addicts. 22% of the cases were women with a sex ratio of 3.5. HIV infection was attributed to use of intravenous drugs in 51% and to heterosexual transmission in 18% of the women. But the incidence of heterosexual transmission increased significantly, essentially in women. Overall, the incidence of infection by the HIV 1 has decreased since 1986. A number of recent publications seem to indicate the same tendency. But this fundamental observation remains to be validated by further studies. The yearly incidence of AIDS increases. The 5 year calculated actuarial rate of probability of remaining asymptomatic decreased to 40% in 385 initially asymptomatic patients.

[Hiccup caused by epigastric compression in a hemiplegic patient].

The authors report the case of a patient with permanent, refractory hiccup, resistant at all classical treatment methods. Hiccup was due to compression of the epigastric region by the left fist of the patient immobilised by a spastic hemiplegia. Passive mobilisation of the diseased upper limb resulted in the disappearance of hiccup.

[Computed tomography in thoracic pathology].

The fundamental principles of computed tomography (CT), its clinical applications and costs are presented followed by the indications and results in diseases of the mediastinum, the oesophagus, heart and great vessels, and the lung and pleura. The CT scan can give the precise localization and density of mediastinal tumours and be used to distinguish thymomas, goiters, lymph nodes, lymphomas, neurinomas and different bronchogenic and pleuropericardial cystic formations. The operability of oesophageal cancer can also be determined. In cardiovascular diseases, the CT scan is particularly useful to identify inborn anomalies, aneurysms, aortic dissection or caval compression or thrombus formation. In lung diseases, the indications for a CT scan are particularly important in bronchopulmonary cancer, tuberculosis, bronchopathies and chronic lung diseases. In bronchogenic cancer, for example, the CT scan is not only a major diagnostic tool but is also particularly useful in determining the prognosis and for following the effectiveness of treatment. The CT scan can be used to identify both effusions of liquid and gas as well as pleural reactions producing thick membranes of importance for both aetiology and later follow-up. Bronchectasis can be identified on serial sections where the degree of extension can be measured. CT scan is also indicated in patients with emphysema, both for evaluating extension and follow-up. Finally the indications and contraindications for interventional computed tomography, particularly in guiding needle biopsies, is presented.
