[Role of osteocutaneous external brachial flap in the treatment of composite loss of substance of the mandible].

The authors report on their research begun in 1987 on this flap and its uses in reconstruction of composite defects of the horizontal and symphysical parts of the mandible, regardless of the cause of the loss. For treatment of average sized defects (less than 7 centimeters), it is reliable flap, easy to remove and to model by osteotomies, giving a thin skin mobile with bone and the removal of which results in only moderate development of scar tissue at the donor site.

[The normal mouth in newborn infants].

The mouth is the place where the newborn begins his experience of life. An anatomical study of the mouth and of the airways-digestive tract junction can explain the particular physiological features of suction and deglutition. These functions bear witness to the neonatal brainstem maturation, improve the newborn's waking state and are the sources of novel acquisitions in the field of interrelations between mother and child.

Mucormycosis in an apparently normal host. Case study and literature review.

Mucormycosis refers to an opportunistic fungal infection most often related to predisposing conditions, especially uncontrolled diabetes. It can occur as a rapidly extending rhinocerebral infection presenting a high mortality rate. The same micro-organism has, however, been identified in benign paranasal infections in the absence of a general debilitating condition, suggesting that host factors are of the utmost importance in the outcome of paranasal mucormycosis.

[The submental island skin flap. A surgical protocol. Prospects of use].

The authors report their use of a previously undescribed skin flap situated in the submental region and vascularised by the submental artery derived from the facial artery. It is possible to raise flaps as large as 11 cm X 6 cm with an identical colour to that of the face. The pedicle flap can reach any part of the homolateral oral cavity or the lower 2/3 of the homolateral side of the face (with the exception of the nasal region). The free flap, using a good calibre pedicle (including facial arteries and veins, may be as long as 7 cm and appears to be extremely reliable. It is also possible to use a distal or composite pedicle taking a segment of the internal basilar margin. The donor site scar dissimulated under the mandible is perfectly acceptable.

Adolescent

Sequential combination of 5-fluorouracil, cis-platinum and irradiation. 1. Advanced head and neck cancers.

Based on the synergistic action of 5-fluorouracil (5-FUra), cis-dichlorodiamminoplatinum(II) (cis-DDP) and gamma-rays, which was suggested in experiments on murine tumours, a sequential treatment combining irradiation and chemotherapy for human solid tumours known to be resistant to conventional treatments has been developed. A pilot study was carried out on 30 patients with recurring head and neck cancers previously treated by radiotherapy and surgery. The good tolerance and the initial results justified applying this protocol to previously untreated cases. The second study involved 40 patients with stage III and IV tumours. After 3 cycles of combined radio- and chemotherapy followed by a conventional radiotherapy, 78% were good responders (51% in complete remission). Oropharynx and oral cavity, without base of tongue, have a 51% actuarial survival at 3 years when they achieved an early complete remission.

[The osteocutaneous outer arm flap. A new concept in microsurgical mandibular reconstructions].

The authors present five cases where the OCLAF technique was adapted to reconstruct large mandibular defects. Some significant advantages made us think about the possibility of the OCLAF as an acceptable alternative in that specific case. These are the ease and safety of the dissection, the respect of a bony vascularisation, a very valuable pedicle size, and a remarkable mobility of the skin paddle to the underlying bone. Of course we had to face some minor disadvantages: the microsurgical procedure, the donor site scar, the absence of cancellous bone in the transfer and the weakening of the bone shaft at the donor site. Nevertheless we think the OCLAF is to be considered whenever large mandibular defects have to be reconstructed with vascularized bone.

[Vagal paraganglioma. Apropos of 2 cases. Review of the literature].

The authors relate the case of two patients of 61 and 64 years old, each one having a high latero-cervical tumor, arching the soft palate, developed from the right vagus nerve and which was a chromaffin paraganglioma. After a recall about the APUD system tumors and the "paraganglioma" concept, they relate their observations at state of the vagal paraganglioma histology, pathology and treatment.

[Cholesteatoma of the facial sinuses. Apropos of an ethmoid-orbital cholesteatoma].

A case of cholesteatoma of the ethmoid extending to the orbit is reported. These cholesteatomas of facial sinuses are rarely reported, those documented involving localization in maxillary or frontal, or ethmoido-frontal or ethmoido-maxillary sinuses. Primary perisinusal cholesteatomas of embryonic origin must be distinguished from the rarer secondary iatrogenic or post-traumatic lesions. Histopathology allows distinction between epidermoid cysts and cholesterol type granulomas. Treatment is by surgery using a wide approach.

[Posttraumatic pseudoaneurysm of the zygomatic area].

A case of post-traumatic false aneurysm of a segment of the zygomato-malar artery is used as a basis for a discussion of this fairly rare complication of facial injuries and the complementarity of angiographic examinations and surgical exploration findings.

[Neurologic manifestations of basal cell nevomatosis. A case].

The naevoid basal cell syndrome, also called Gorlin's syndrome, is a dysembryoplasia akin to the phakomatoses and which may affect the skin, the bones, the nervous system, the eyes, the endocrine glands and the genitalia. A case with numerous tumours and malformations is presented, together with a review of the literature.

[Contribution of radiological technics to the study of the mandibular canal. Application to human fossils and comparison with modern man and Pongidae].

Fossil mandibles from the "La Chapelle-aux-Saints" Man and Cro-Magnon "old man" are considered to be representative of Neanderthal and Cro-Magnon era. Radiographic studies of such mandibles - including C. T. Scan and panoramic views in common use in maxillofacial surgery - allowed the authors to make useful comments: the mandibular canals are nearly similar to those in Modern Man and quite different from those in Pongidae; the imaging was of adequate quality for a good evaluation of fossil internal structures.

[Anatomic and physiologic comments concerning the innervation and function of the omohyoid muscle in man].

Electrophysiological studies of the rather strange, enigmatic, likely vestigial omohyoid muscle led the authors to point out the dual nature of its two bellies. Electromyographically, the inferior belly with a nearly constant activity may be opposed to the superior belly with intermittent activity. Motor nerve stimulation combined with electromyographic study demonstrated the superior belly to be supplied by the ansa cervicalis while the inferior belly is supplied by the accessory nerve. Based on these findings, anatomical studies conducted with histological examination were consistent with this double innervation.

[Intraorbital abscess of dental origin in a child].

A two years old girl developed orbital infection secondary to dental trauma. Computed tomographic scanning of the orbit shows subperiosteal abcess in the medial orbital wall and proptosis of left globe. Teeth removal, appropriate antibiotics and surgical drainage are essential for preservation of vision.
