Cobalt content of household cleaning products.

In our Department, the incidence of sensitization to cobalt has more than doubled in the last 5 years. 28% of cobalt-sensitive patients showed no other sensitivity. Of isolated cobalt sensitive patients, 68% were housewives. We determined by atomic absorption spectrophotometry with a graphite camera, the cobalt and nickel content of the 30 most popular Spanish domestic detergents and cleaners sold in 1985. 78.6% of them contain considerable amounts of cobalt.

[Etiological variety of balanopreputial pigmentations].

The authors report 17 cases with a similar clinical pattern but corresponding to different processus: lentigo simplex, dermal and junction nevus, lichen planus, lentigo premalignant, secondary Incontinentia pigmenti. The diagnosis of the disorders was made by microscopical study.

[Contact dermatitis caused by nickel].

This paper attempts to explain the initial onset in which the population are sensitized to nickel at an early age, above all in females. A study has been made on 200 controls who tolerated metals and on 964 patents who did not tolerate them. The study comprised of patch tests with 8 distinct washers, made of a variety of metals and alloys all containing nickel, and with nickel sulphate to 1% in petrolatum and cobalt salts to 1% in petrolatum. The results, professional and atopic relationship and other considerations are commented.

[Localized cutaneous infantile myofibromatosis. Apropos of a congenital self-limiting case].

A case of a 8 year girl presenting a congenital tumoral lesion of the lumbar region is reported. Histologically a hamartomatous fibromatosis was found. A review of the literature was made. The final diagnosis was solitary infantile myofibromatosis, cutaneous type (peculiar form of congenital generalized fibromatosis, also named infantile myofibromatosis). Probably this variety is less rare than thought, because biopsies of apparently benign superficial lesions on children are not usually performed.

Painful subungual keratotic tumors in incontinentia pigmenti.

We report a case of dyskeratotic subungual tumor in a 28-year-old woman. The patient presented achromic lineal lesions, typical of residual incontinentia pigmenti (IP), on her legs. Furthermore, the physical examination showed anodontia of the right incisor tooth. The familial background demonstrated miscarriages of male fetuses and characteristic dental abnormalities of IP in female siblings. The patient's daughter presented a typical background of IP and dyschromic splashed lesions. Light and electron microscopic studies of the subungual tumors were made. The analogy between subungual tumors of IP and keratoacanthoma is emphasized.

Microscopic abnormalities in the liver of two patients with Porphyria variegata.

In contrast to Porphyria cutanea tarda (PCT), which is characterized by constant hepatic involvement, the liver seems to be unaltered in Porphyria variegata (PV). However, the authors have found microscopic alterations in the liver of 2 patients with PV (mild inflammation and fibrosis of the portal tracts and the presence of iron deposits in one of the cases). Although these changes could be attributed to other causes, the hypothesis of possible liver involvement in PV is not excluded.

[Dermatitis caused by Milwaukee braces].

The results of the study of 51 patients bearing a Milwaukee corset indicate that they present a mechanical and irritative dermatitis caused by the personal predisposition, the action of the plastic material and the occlusion and humidity produced between skin surface and the corset. The authors suggest the name Occlusive Dermatitis Syndrom for this cases as a subtype of Mechanical Irritative Dermatitis.

Why do melanomas ulcerate?

Ulceration is an indicator of unfavourable prognosis in malignant melanoma (MM). But why do melanomas ulcerate?. Possible causes of ulceration were investigated in a group of 69 ulcerated and 69 non-ulcerated malignant melanomas. A significant correlation (P less than 0.001) between ulceration and mitotic index was found. In a group of 69 ulcerated malignant melanomas, 49 had over 7, 15 had 4-6, and only 5 had 0-3 mitotic figures per 10 high-power fields. Among 69 non-ulcerated melanomas, 28 had more than 7, 24 had 4-6, and 17 had 0-3 mitotic figures per 10 high-power fields. This study supports the hypothesis that ulceration of melanomas is usually the result of the destruction of the epidermis by the proliferating neoplastic cells or modification of blood supply due to the expansile activity of the tumor. The relationship between ulceration and mitotic activity helps clarify the prognostic significance of ulceration.

[Early forms of porphyria cutanea tarda. Apropos of 2 cases with a study of familial enzymatic deficiency and definition of the mode of genetic transmission].

The authors report two cases of cutaneous porphyria in children aged 6 and 9 respectively. The clinical and biochemical patterns were similar to those of porphyria cutanea tarda. The manifestations were drug-induced in both cases (by sulfonamides and griseofulvin in the first patient, and by hydantoin and benzodiazepine in the second). The clinical examination and the determination of porphyrins in the relatives showed no abnormalities. However, the study of the erythrocyte uroporphyrinogen decarboxylase showed 50 p. 100 reduced activity in the father and grandfather of the first patient, and in the second case's mother. Treatment with chloroquine (3 mg/kg/week) produced the total remission of both cases with no recurrences in 4 and 2 years respectively.
