Severe combined immunodeficiency due to defective binding of the nuclear factor of activated T cells in T lymphocytes of two male siblings.

Peripheral blood lymphocytes (PBL) and alloreactive T cell lines of two male infants born to consanguinous parents and presenting with severe combined immunodeficiency (SCID) showed a pronounced deficiency in T cell activation. Although phenotypically normal, the proliferative response of the childrens' T cells was strongly reduced but could be improved by the addition of interleukin-2 (IL-2). Furthermore both childrens' T cells were unable to produce the cytokines IL-2, interferon-gamma (IFN-gamma), IL-4 and tumor necrosis factor-alpha (TNF-alpha). This multiple cytokine production deficiency could not be restored by IL-2 or co-stimulatory signals provided by antigen-presenting cells (APC). Moreover, mRNA for IL-2 and IFN-gamma could not be detected. In contrast, expression of the activation-dependent cell surface markers CD25 and CD69 was within normal limits. To determine whether the functional defect of the patients' T cells was due to the absence or abnormal binding of transcription factors involved in cytokine gene expression, electrophoretic mobility shift assays were used to examine the DNA binding of AP-1, Oct, CREB, SP1, NF-kappa B and the nuclear factor of activated T cells (NF-AT) to their respective response elements in the promoter of the IL-2 gene. Whereas AP-1, NF-kappa B, Oct, CREB and SP1 displayed normal binding activities in nuclear extracts, the binding of NF-AT to its IL-2 promoter response element was barely detectable both before and after T cell stimulation. Our results strongly suggest that this NF-AT/DNA binding defect is responsible for the multiple cytokine deficiency and the SCID phenotype observed in the two infant brothers.

[Laparoscopic or conventional repair of inguinal hernia with synthetic mesh?].

Laparoscopic inguinal hernia repair by means of the transabdominal preperitoneal technique (TAPP) was compared with open plug-and-patch-repair (PP) in a prospective study. From September 1992 until July 1994, 86 patients were operated using the TAPP technique; after introduction of PP in August 1994, 105 patients were operated using this technique. From August 1994 until April 1995, 24 more patients wanted the laparoscopic procedure. The two groups were comparable with regard to age, sex and the distribution of hernia types. Eighty-eight (84%) of the PP patients were operated under local anesthesia, while the others chose general anesthesia. The only intraoperative complication was a bleeding from the epigastric vessel in the laparoscopic group. Postoperative complications were three wound hematomas after TAPP (2.2%) and two wound hematomas and one superficial wound infection after PP (2.4%). All complications healed spontaneously. No detectable statistical difference was noted between the two groups with respect to postoperative pain, the consumption of analgetics, duration of hospitalization and reconvalescence. After a median follow-up of 34 months, two recurrences after TAPP (1.4%) and no recurrences after PP has been detected. Based on our experience we prefer PP for inguinal hernia repair in adults because it affords the same patient comfort as TAPP but is less expensive and can be performed under local anesthesia.

[Effect of orthograde intestinal irrigation with Prepacol and polyethyleneglycol solution on duration of postoperative ileus after colorectal resections].

In a prospective randomized study in 100 patients undergoing conventional (n = 66) or laparoscopic (n = 34) colorectal resection, mechanical lavage with 2 l of a polyethylene glycol (PEG) solution (group A, n = 50) combined with a laxative was compared with lavage with 4 l of PEG alone (group B, n = 50). The influence of bowel preparation on postoperative ileus was investigated. The efficacy of bowel preparation was determined intraoperatively by a surgeon blind to the type of preparation. The time interval between the end of the operation an first flatus or bowel movement was recorded. The efficacy of preparation was no different between the groups (group A: 94%, group B: 84%; P = 0.5). The time lapse before first flatus (A: 3.1 +/- 1.2 days, B: 3.2 +/- 1.3 days; P = 0.6) or bowel movement (A: 3.9 +/- 1.3 days, B: 4.1 +/- 1.3 days; P = 0.5) also did not differ between the groups. The only factor found in multivariate analysis (age, sex, operation, duration and type of surgery, bowel preparation) to prolong the duration of postoperative ileus was the type of surgery. The first bowel movement occurred 3.2 +/- 1.1 days after conventional surgery and 4.3 +/- 1.2 days (P < 0.001) after laparoscopic surgery. The type of preoperative bowel preparation had no influence on the duration of postoperative ileus. The postoperative interval before the first bowel movement was shorter after laparoscopic surgery.

Monocyte deactivation--rationale for a new therapeutic strategy in sepsis.

Inflammatory cells, in particular monocytes/macrophages, release pro-inflammatory mediators in response to several infectious and non-infectious stimuli. The excessive release of these mediators, resulting in the development of whole body inflammation, may play an important role in the pathogenesis of sepsis and septic shock. TNF-alpha, acting synergistically with cytokines such as IL-1, GM-CSF and IFN-gamma, is the key mediator in the induction process of septic shock, as shown in several experimental models. Based on this concept and on the encouraging results obtained in several experimental models, a number of clinical sepsis trials targeting the production or action of TNF-alpha or IL-1 have been performed in recent years. Unfortunately, these trials have failed to demonstrate a therapeutic benefit. One reason for this may be the lack of exact immunologic analyses during the course of septic disease. Recently, we demonstrated that there is a biphasic immunologic response in sepsis: an initial hyperinflammatory phase is followed by a hypo-inflammatory one. The latter is associated with immunodeficiency which is characterized by monocytic deactivation, which we have called "immunoparalysis". While anti-inflammatory therapy (e.g. anti-TNF antibodies, IL-1 receptor antagonist, IL-10) makes sense during the initial hyperinflammatory phase, immune stimulation by removing inhibitory factors (plasmapheresis) or the administration of monocyte activating cytokines (IFN-gamma, GM-CSF) may be more useful during "immunoparalysis".

Videoendoscopic vascular surgery on aortoiliac vessels. An experimental study.

BACKGROUND: In order to avoid the postoperative complications following conventional surgical treatment of aortoiliac atherosclerotic occlusive disease, a new concept in videoendoscopic vascular surgery on the infrarenal aortoiliacal artery was created. METHODS: New instruments for endoscopic vascular surgery were therefore developed. Having elaborated the practicability of "videoendoscopic surgery" in four short-term pilot studies in piglets, we performed a standardized acute study in healthy male piglets (32 +/- 2 kg). Blood flow was monitored ultrasonically in the femoral arteries and systemic and hemodynamic parameters were evaluated in all animals. RESULTS: The investigation regarding ultrasonically monitored blood flow in the femoral arteries (four aortofemoral bypasses, four iliac patchplasties) and other systemic, hemodynamic parameters after videoendoscopic vascular reconstruction revealed positive results. The following series of standardized long-term animal experiments revealed that 2-7 weeks after the aortofemoral bypass grafting (n = 5), the animals were all doing well. The angiographic examination showed that the prostheses were patent. CONCLUSIONS: Even though it was possible to carry out the endoscopic vascular method according to the rules of conventional vascular surgery, further experimental research and development of endoscopic instruments have yet to be done in order to optimize the new surgical method. This is the subject of our ongoing long-term animal studies.

Quality of life after surgical therapy of bronchogenic carcinoma.

Quality of life (QL) after the "curative" resection of non-small cell bronchogenic carcinoma was assessed by patients using the EORTC QL questionnaire (QLQ) and by a psychologist using the Spitzer Index. Quality of life was assessed in 52 patients on one occasion 12 months postoperatively and in 20 patients regularly starting with a preoperative assessment. Self- and external evaluation showed a significant correlation (r = 0.41), but QL was assessed as being higher by the external observer. After surgery it was mainly affected by restrictions related to physical activities, job and household tasks, and disease symptoms, whereas limitations in emotional, social, and financial domains were found less frequently and less severely. Of the different medical (surgical procedures, tumor recurrence) and social factors (sex, marital and employment status), only tumor recurrence was determined to have a significant and negative influence on postoperative QL (P < 0.02). When compared to the preoperative assessment, QL had deteriorated on discharge from hospital but was restored within 3-6 months postoperatively in disease-free patients.

Open window thoracostomy in the treatment of esophageal or bronchopleural fistula with advanced mediastinitis and septic shock.

Mediastinitis and septic shock following esophageal or bronchopleural fistula are rare but serious conditions with a high mortality rate. Six patients were treated with open window thoracostomy (OWT) after primary suture repair and closed tube drainage had failed to cure the patient's condition. In all cases the clinical condition improved immediately. Two patients died later because of unrelated diseases. OWT should be considered in critically ill patients with broncho- or esophagopleural fistula when primary therapy fails to control the septic focus.

Prenatal diagnosis of a choledochal cyst: a case report and review of the literature.

There are 16 reported cases of prenatally diagnosed choledochal cyst in the literature. We present a new case diagnosed at 29 weeks' gestation by routine ultrasound scanning. At 40 weeks' gestation, a male infant was born by spontaneous delivery. At 16 weeks of age, the patient underwent a laparotomy, which confirmed the diagnosis of a choledochal cyst. The cyst and the gallbladder were removed en bloc and reconstruction of the biliary tree was performed by formation of a retrocolic Roux-en-Y-hapatojejunostomy with an antireflux valve. Histological examination of the cyst showed a thickened fibrous tissue wall with necrotising areas and without epithelial lining. The patient's postoperative course was uneventful. Review of the literature shows that it is possible to make a presumptive prenatal diagnosis of this anomaly as early as 15 weeks' gestation. The differential diagnosis of a sonolucent lesion in the fetus should also include the rare choledochal cyst. Treatment of choice is the radical excision of the cyst with construction of a Roux-en-Y hepaticoenterostomy. Long-term results with this method are very good. Surgery may be instituted at the earliest possible opportunity before severe complications such as cholangitis, liver abscesses, cirrhosis, and malignant degeneration can occur.

Laparoscopic Billroth-II gastrectomy.

Although the use of laparoscopic techniques in gastric surgery had become a focus in upper gastrointestinal surgery, standardized procedures have not yet been developed. The purpose of the study was to develop a standardized technique for laparoscopic Billroth-II resection in a cadaver model. End points were intraoperative complications and patency of the gastro jejunostomy. Laparoscopic partial stomach resection was performed in seven cadavers. The specimen was removed through an enlarged trocar incision. The first part of the jejunum was temporarily taken out through the same incision and a side-to-side jejuno-jejunostomy created. The gastrojejunostomy was stapled intracorporally. After surgery, all cadavers underwent autopsy. No lesions of intraabdominal organs were found, and the gastrojejunostomy was patent with correct stapling. No major intraoperative complications were recorded. Using the described technique, a laparoscopic Billroth-II operation can safely be accomplished. The technique has been successfully performed in three patients in the last month.

Anastomotic tissue oxygen tension during esophagectomy in patients with esophageal carcinoma.

Submucosal tissue oxygen tension (PtO2) was measured in 20 patients with cervical esophagogastrostomy after resection of esophageal carcinoma, using a Clark-type oxygen electrode. The mean gastric baseline PtO2 was 54.6 +/- 10.7 mm Hg. Following ligature of the vasa brevia and the left gastroepiploic artery PtO2 decreased to 45.8 +/- 9.9 mm Hg, ligature of the left gastric artery caused a decrease to 34.2 +/- 9.7 mm Hg and resection of the lesser curvature and pulling up of gastric tube led to 25.5 +/- 9.0 mm Hg. Clinical value and practicability of intraoperative PtO2 measurements could be proven.

[Current therapy of bile duct cysts. II. Intrahepatic cysts (Caroli syndrome)].

17 patients with a Caroli's syndrome are reviewed with emphasis on clinical features and late results (follow-up: 2-10 years). The group consists of 9 women and 8 men ranging in age from 17 to 80 years (mean of 42.6). 14 patients had a diffuse form, 5 of whom had periportal fibrosis with secondary biliary cirrhosis, which in 3 cases resulted in a portal hypertension with hepatic failure. In only 3 cases the dilatation was limited to the left lobe of the liver. 9 of the patients had previously undergone surgery in the form of cholecystectomy and choledocholithotomy elsewhere. The disease was complicated by lithiasis (14 intrahepatic, 9 extrahepatic). Curative treatment was only possible in the 3 patients with unilobar Caroli's syndrome (partial liver resection). In 5 patients we performed a cholecystectomy and choledocholithotomy combined in 2 cases with a transduodenal sphincterotomy. There was no operative death. Endoscopic treatment consisted in removal of stones and decompression of the biliary tree by sphincterotomy in 9 cases and endoscopic retrograde biliary drainage (endoprosthesis) in 5 cases. Excepting the 3 curative operated patients who are asymptomatic respectively 2, 4 and 5 years after surgery, the remaining cases had repeated bouts of acute pains accompanied by recurrent episodes of cholangitis. A late mortality of 23.5% (4 patients) is proof of the poor prognosis of this disease.

[Laparoscopic gastrectomy. A case report].

Although laparoscopic techniques in gastric surgery have become an important part of upper gastrointestinal surgery, they are mainly utilized for organ preserving procedures. A standardized technique for laparoscopic gastrectomy has not yet been described in detail. The purpose of this study was to develop a standardized technique for laparoscopic gastrectomy with reconstruction by a stapled termino-lateral esophago-je-junostomy and Roux-en-Y-anastomosis in a cadaver model. The resected stomach was removed through an enlarged trocar incision. The described technique has been successfully performed in one patient with early gastric cancer.

[Cystic tumors of the pancreas].

Cystic neoplasms of the pancreas are rare but in the last years more frequently detected. Within a 10-year-period we treated 30 patients, including 8 serous cystadenomas, 6 mucinous cystadenomas, 12 mucinous cystadenocarcinomas, 2 cystic neuro-endocrine tumors and 1 papillary cystic tumor respectively acinar cell cyst-adenocarcinoma. 80% of the patients had symptoms, experienced abdominal pain, weight loss, weakness or abdominal mass. In eight patients the tumors had been misdiagnosed as a pancreatic pseudocyst. The correct type of cystic tumor was diagnosed by preoperative investigations only a few cases. All patients with serous or mucinous cystadenomas are well and without evident recurrence after resection of the tumor. However the survival time of malignant cystic tumors varied strongly. The curative resection of these tumors give patients the chance of long-term survival.

[After-care of gastrointestinal malignancies--an evaluation of costs and benefits].

AIM OF THE STUDY: To determine the costs and benefit of standardized follow-up programs after surgical therapy of gastrointestinal malignancies. METHODS: Retrospective analysis of the results and the costs of regular outpatient follow-up examinations after radical resection of colorectal (n = 552), gastric (n = 180), and esophageal carcinoma (n = 160). RESULTS: The diagnostic efficiency of regular follow-up concerning the diagnosis of asymptomatic tumor recurrences was 9% in esophageal, 10% in gastric, and 15% in colorectal carcinoma. The average follow-up costs for each asymptomatic recurrence were about 10.000 DM. R0-resection of asymptomatic recurrences or metachronous tumors was performed in none of the patients with esophageal and gastric carcinomas and in 22 patients with colorectal carcinomas (4% of all patients with colorectal carcinomas). In contrast to the low therapeutic efficiency in tumor recurrences, the therapeutic efficiency with regard to the treatment of postoperative nonmalignant functional disorders was significantly higher (esophageal carcinoma: 63%, gastric carcinoma: 40%, colorectal carcinoma: 24% of the follow-up examinations). CONCLUSIONS: In order to examine, whether the cost-benefit-relation of regular follow-up programs can be increased by some quantitative and qualitative modifications of the programs, controlled economic studies are required.

[Ambulatory artificial nutrition: 15 years experiences at the Cologne University Surgical Clinic].

Beetween 1979 and 1994 629 respectively 223 patients were treated by home enteral and parenteral nutrition. The duration of the treatment on average was 361 days in case of enteral and 346 days for home parenteral nutrition, in special cases however up to 15 years. In enteral nutrition there were no metabolic complications and only minor technical problems. Catheter related problems represented the most challenging complications in parenteral feeding. 12 out of 14 patients treated more than 1 year with home parenteral nutrition because of non-cancerous disease died from recurrent pulmonary embolism or catheter-related septicemia.

[Systematic lymphadenectomy in esophageal carcinoma--preliminary results of a prospective randomized study].

The influence of transhiatal blunt esophagectomy with dissection of all reachable lymph nodes (ESD) and en-bloc-esophagectomy (EB) on perioperative cardio-pulmonary stress and late results was analysed in a prospective randomized trial. Blunt dissection of the esophagus was performed without systematic lymph node dissection (SD) in patients with increased cardio-pulmonary dysfunctions. Until now 26 patients (ESD: 13, EB: 13) entered the study as well as 14 patients treated by blunt dissection. The number of dissected lymph nodes in each patient was 7.5 (SD), 19.9 (ESD) and 37.6 (EB). Ratio of invaded to removed lymph nodes was 0.26 (SD), 0.25 (ESD) and 0.1 (EB). The 30-day mortality rate was 8.0% in each group. Under the consideration of this mortality, one-year survival rate was 79% (SD), 70% (ESD) and 77% (EB). Patients with negative lymph node involvement showed a one-year survival of 85% and positive involvement lead to a survival rate of 70% after one year, independently of the surgical approach.

[Current therapy of bile duct cysts. I. Extrahepatic cysts].

A total of 23 patients with extrahepatic biliary cysts (11 type I, 4 type II, 4 type III and 4 type IVa according to the classification by Todani) seen at our institution in a 15-year period were reviewed with emphasis on management and long-term results at follow-up of 3 weeks-12 years. Associated hepatobiliary disease occurred in 4 patients, including one case with malignant degeneration in the cyst 23 years after cystenterostomy, and an association of a gallbladder carcinoma with a choledochocele in the other patient. Radical excision of the dilated bile duct and reconstruction by Roux-en-Y hepatico-jejunostomy was performed in 13 cases (9 type I, 3 type II and 1 type III), combined in 3 cases with an antireflux valve. All 13 operated on patients remained in good health for 6 months to 12 years. These results confirm the need for complete early excision of type I, (II) and IVa cysts at all ages before severe complications can occur. Endoscopic treatment of type III choledochocele should be limited to the management of smaller lesions.
