[Iron overload in a beta thalassemia heterozygote of the intermediate type in a subject of Alsation origin. Results of iron chelation treatment].

Heterozygous beta thalassaemia with microcytic anaemia (hemoglobin concentration 77 g/l) has been recognized in a 49 year-old woman of Alsatian extraction. A long history of microcytic anaemia had led to inadequate oral iron treatment before the patient was referred to us because of the persisting microcytic anaemia and iron loading. Indeed the patient also had haemosiderosis with a high transferrin saturation (73%) and markedly elevated ferritinaemia (1,114 micrograms/ml). Ferrokinetic data showed increased plasma iron turnover, early transfer of iron to the liver and evidence of ineffective erythropoiesis. She was treated with desferrioxamine (3 g every three days subcutaneously) and serum ferritin levels gradually decreased together with transferrin saturation. After 15 months serum ferritin and transferrin saturation were within the normal range. Several hypotheses are discussed to explain why this patient had haemosiderosis associated with heterozygous beta thalassaemia. The propositus was found to be HLA-A3, which is strongly associated with idiopathic haemochromatosis. Her sister also carries HLA-A3 with heterozygous beta thalassaemia but she has neither anaemia nor iron overload. Thus double heterozygotism is unlikely in our patient.

Immunomodulating effects of a short-term oral treatment with C 1821 in untreated cancer patients: a controlled study.

C 1821 is a purified glycoprotein extract from Klebsiella pneumoniae serotype 2 with a molecular weight of about 350,000. It enhances immune responses in animals when given orally and the oral route of administration is devoided of any toxicity even in humans. The present controlled trial showed that C 1821 given per os at the single daily dose of 4 mg for 14 days in untreated cancer patients (mostly lymphomas) significantly enhanced delayed cutaneous hypersensitivity to recall antigens using the Multitest system (7 antigens). It also increased basal levels of lymphocyte cAMP and particularly of cGMP which were decreased in these patients. When incubated in vitro with lymphocytes from either normal controls or patients, C 1821 showed a dose-dependent stimulation of cAMP synthesis which was more pronounced in patients than in controls.

Sodium diethyldithiocarbamate (imuthiol) and cancer.

Sodium diethyldithiocarbamate (imuthiol), a nonantigenic, nontoxic and noncarcinogenic compound, evinces distinctive properties in recruitment and activation of T cells, and no direct influences on B cells or macrophages. The biological attributes of imuthiol (destruction of neoplastic cells, bacteria, fungi and parasites; a detoxifying influence against carcinogens or agents toxic for the liver) could strengthen its activity as a biological response modifier. Preliminary clinic testings show that imuthiol restores T cell activities and regulates the ratio among T cell subsets in cancer patients, without untoward side effects.

Alterations of peripheral blood lymphocyte cyclic AMP and cyclic GMP in untreated patients with hodgkin's disease.

Cyclic AMP and cyclic GMP are important regulatory agents of lymphocyte functions. Depressed T-lymphocyte functions are frequently associated with Hodgkin's disease and suppressor monocytes have been implicated in the pathogenesis of this defect. In the present study cAMP and cGMP resting levels were measured in lymphocytes from 18 untreated patients with Hodgkin's disease using a sensitive radioimmunoassay. A significant decrease of cAMP (P less than 0.001) and, to a lesser degree, of cGMP (P less than 0.01) was found in monocyte-depleted lymphocyte suspensions from the patients compared to controls. Studies of patient and control lymphocyte subpopulations showed in patients a clear deficit of cAMP in T-depleted lymphocytes, rather than in T cells, with a low cAMP/cGMP molar ratio in both subpopulations. From this data it is clear that factors other than prostaglandin-mediated suppression of monocyte origin are involved in the pathogenesis of the T-lymphocyte depression associated with Hodgkin's disease.

[Idiopathic thrombocytopenic purpura. Treatment by intravenous transfusion of polyvalent immunoglobulins].

Thirteen patients (2 children and 11 adults) with idiopathic thrombocytopenic purpura were treated with high doses of human immunoglobulins intravenously (0.4 g/kg bodyweight/day for 5 days). The platelet count rose sharply in 11 patients, with considerable individual variations in terms of maximum count achieved and delay as well as duration of response. Only two adult patients refractory to previous treatments failed to show any significant response. Complete correction of platelet counts was observed in the two children, whereas most adult patients showed incomplete and transient response. No difference was observed between splenectomized and non splenectomized patients. Survival of 111 Indium-labelled autologous platelets was studied in 2 patients after correction of platelet count and was still found shortened. Circulating immune complexes, when present, were not modified by immunoglobulin infusions. In all patients a significant reduction of the fourth component of serum complement was noted after treatment.

Enhancement of delayed cutaneous hypersensitivity in untreated cancer patients given a short-term oral treatment with C 1821.

C 1821 is a purified glycoprotein extract of Klebsiella pneumoniae serotype 2 with immunomodulating properties in animals (in vivo and in vitro) and in humans (in vitro). The compound is devoid of any apparent toxicity when given orally. The aim of the present work was to evaluate the effects of a short term oral administration of C 1821 on delayed cutaneous hypersensitivity to recall antigens in untreated cancer patients (mostly lymphomas). Consecutive patients were alternately allocated to receive C 1821 or placebo for 14 days. C 1821 restored and significantly (P less than 0.02) enhanced skin reactions, as shown using the Multitest system.

Regional immunotherapy in resectable squamous cell lung carcinoma. Analysis of a randomized study.

Patients with resectable squamous cell carcinoma were randomly allocated after surgery to receive either no further treatment (57 patients) or a single intrapleural injection of BCG (61 patients). No significant improvement in survival was observed in patients treated with BCG, even when their disease was staged as N0. There was a slight trend for the recurrence rate to be lower in patients classed as N0, but this was not significant.

Evaluation of peripheral blood T lymphocytes in renal adenocarcinoma before and after surgical removal of the tumor.

Peripheral blood T lymphocytes were studied in 24 untreated patients with renal adenocarcinoma, 80 per cent of whom had local extension of the tumor. T lymphocytes were evaluated with the total and the active rosette assays, and were further submitted to stimulation by a range of phytohemagglutinin concentrations in vitro. Controls were healthy volunteers tested on the same days as the patients. Total E-rosette forming cells were slightly decreased but the difference compared to controls was not significant. Active rosettes were normal. When present lymphocytopenia affected total E-rosette forming cells. The mean of the log-converted stimulation indexes was reduced significantly in the patient group only when a suboptimal concentration of phytohemagglutinin was used. Total E-rosette forming cells returned to normal 3 weeks postoperatively but lymphocyte reactivity did not improve and active rosette forming cells remained within normal range. Thus, only minor abnormalities of T lymphocytes were disclosed in this study, suggesting functional alterations rather than depletion. Our study confirms that a cancer operation may act as immunotherapy.

Hemolytic anemia and sulfhemoglobinemia due to phenacetin abuse: a case with multivisceral adverse effects.

The authors report a case of recurrent sulfhemoglobinemia resulting from a chronic abuse of phenacetin. Cyanosis was accompanied by hemolytic anemia and numerous features of tissue aging. While phenacetin was found to be the oxidizing drug, no source of sulfur was identified. The origin of sulfhemoglobinemia in man is discussed and the possible role of intraerythrocytic glutathione is emphasized.

Decreased autologous rosette-forming T lymphocytes in alcoholic cirrhosis: absence of correlation with other T cell markers and with delayed cutaneous hypersensitivity.

Autologous rosette-forming T lymphocytes were enumerated in periphal blood of patients with established alcoholic cirrhosis and compared with T lymphocytes forming rosettes with sheep red blood cells (total and 'active' rosette-forming cells; RFC). Decreased autologous RFC was the most frequent T lymphocyte abnormality observed in 43 patients and was shown to occur even in patients with normal total and 'active' RFC. No correlation was found between autologous RFC and total or 'active' RFC. Skin anergy to 2,4-dinitrochlorobenzene was noted in 44% of the patients tested. Autologous RFC did not correlate with delayed cutaneous hypersensitivity to 2,4-dinitrochlorobenzene.

["Active" rosettes and skin tests to recall antigens and phytohemagglutinin in 48 patients with non metastatic squamous cell lung carcinoma and before treatment (author's transl)].

Looking for an early immune defect assay in non metastatic (MO) squamous cell lung carcinoma, 48 untreated patients were tested at the time of diagnosis for peripheral blood "active" rosette-forming T lymphocytes, delayed cutaneous hypersensitivity to recall antigens and skin reactivity to phytohemagglutinin (PHA). 32 patients had a resectable tumour (group 1) and 16 patients had a loco-regional extension which made the tumour inoperable (group 2). No significant difference (p greater than 0.05) was observed between any of the two groups of patients and normal controls nor between group 1 and 2 for "active" rosettes and skin tests to recall antigens. However, skin reaction to PHA was significantly impaired in patients of both groups compared to normal age-matched controls whatever the dose of PHA (4 microgram and 10 microgram). The number of unreactive patients in group 2 was higher than in group 1, but the difference was not significant (p greater than 0.05). It is concluded that skin reactivity to PHA is an useful and easy assay for cell-mediated immunity deficiency in early squamous cell lung carcinoma, whereas "active" rosettes are not altered.

Delayed cutaneous hypersensitivity testing in untreated Hodgkin's disease using a standardized new device.

Delayed cutaneous hypersensitivity to 7 recall antigens has been tested in 32 patients with untreated Hodgkin's disease using a standardized and reproducible system. Anergy to all 7 antigens was observed in 15/32 patients and hypoergy in 4/32 leading to a cumulative deficiency rate of 59%. Although the defect was distributed throughout all stages, substages and histologic types, it seemed to predominate in less favorable forms of the disease as well as in female patients.
