Sleep apnea.

OBJECTIVE: To study clinical presentation of sleep disordered breathing (SDB) in children, their causative factors and response to treatment. METHODS: A retrospective study of clinical data nd results of overnight polysomnography done at baseline and after therapy were reviewed in 56 patients under 18 years of age. RESULTS: Of the 56 patients included in the study 23(41%) cases were positive for SDB. 12 (52.1 %) patients had craniofacial abnormalities, 4 (17.3%) had neuromuscular and skeletal disorders, 2 (8.6%) had adenotonsillar hypertrophy, 1(4.3%) had bilateral vocal cord palsy and 3 (13%) had sleep apnoea associated with multisystemic disorders. Post-operative data showed improvement in all 6 cases of craniofacial abnormalities and both cases of adenotonsillar hypertrophy. Positive airway pressure treatment was useful in cases with obstructive sleep apnea (OSA) due to vocal cord palsy, thoracic scoliosis, systemic disorders and central hypoventilation. CONCLUSION: 41% of suspected cases were detected to have SDB. Craniofacial abnormality was the leading cause of OSA in the present study. Surgical correction improved symptoms apnea-hypopnea index (AHI) and desaturation in cases of craniofacial disorders and adenotonsillar hypertrophy. Vocal cord palsy, thoracic scoliosis, hypoventilation and systemic disorders associated OSA responded to positive airway pressure ventilation.

Clinical and laboratory observations of tuberculosis at a Mumbai (India) clinic.

OBJECTIVES: To study the positivity of sputum acid fast bacilli (AFB) smears in patients with pulmonary tuberculosis using 24 hour sputum collection. To detect HIV seropositivity in patients suffering from tuberculosis, and to analyse the pattern of tuberculosis disease in this subgroup. To determine the outcome of patients treated with directly observed therapy. SETTING: The tuberculosis referral unit of a tertiary care hospital. DESIGN: A total of 893 consecutive patients with tuberculosis, diagnosed between 1 November 2000 and 30 September 2002, were included in the study. An HIV test was performed in all patients, with adequate counselling and informed consent. Treatment was prescribed as per World Health Organisation treatment categories. RESULTS: Out of 893 patients with tuberculosis, 695 had pulmonary tuberculosis and 198 had extrapulmonary tuberculosis. Out of the 695 pulmonary tuberculosis patients, 673 (96.8%) were sputum smear AFB positive. Overall, 71 patients (8.0%) were HIV positive. The pattern of tuberculosis was the same in HIV seropositive and seronegative patients. Treatment outcome could be analysed in 112 out of 150 patients: 78 patients (70%) were declared cured or completed treatment. CONCLUSIONS: Sputum smear AFB could be a very sensitive test when a large quantity of sputum is used. The presence of HIV coinfection does not alter the clinical presentation. Only 70% of patients treated were cured/completed treatment, in spite of a strict directly observed therapy.

Effects of soybean genotype and growing location on oil and fatty acids in tofu.

Soyfood products like tofu are becoming popular among American consumers due to health benefits. In order to increase production to meet consumer demands, it is imperative that factors that effect quantity and quality of tofu be characterized. The present study was conducted to determine the effects of soybean genotypes and growing locations on contents of oil and fatty acids in tofu which was prepared from twelve soybean genotypes (BARC-8, BARC-9, Enrei, Hutcheson, MD86-5788, Nakasennari, S90-1056, Suzuyutaka, V71-370, V81-1603, Ware, and York) grown at three southern U.S. locations (Huntsville, Alabama; Princess Anne, Maryland; and Petersburg, Virginia) during 1995. The results showed that tofu quality was determined by the soybean genotype. The tofu made from seeds of high seed-protein and low seed-oil genotypes (BARC-8 and BARC-9) resulted in tofu with low contents of oil (15.8 and 11.3 g/100g, respectively) and total saturated fatty acids (2.59 and 160 g/100g, respectively). Tofu made from seeds of conventional grain type genotypes, such as Hutcheson, resulted in higher oil (24.0 g/100g) and total saturated fatty acids (3.80 g/100g) contents in tofu. Effects of growing environment on contents of oil were not significant but tofu made from seeds grown in Alabama had significantly higher content of total saturated fatty acids (3.50 g/100g) as compared to that made from seeds grown in Maryland (2.88 g/100g) or Virginia (2.96 g/100g). Tofu made from seeds of large and medium-seeded genotypes had higher contents of total monounsaturated fatty acids in tofu as compared to that made from small-seeded genotypes. Highly positive correlation existed between contents of oil, 18:1, 18:2, total saturated, and total unsaturated fatty acids in the seeds and their contents in the tofu (+0.80, +0.75, +0.79, +0.62, +0.68, respectively). These results indicated that tofu quality is governed by soybean genotype, seed size and growing location.

Pulmonary arteriovenous malformation treated with embolotherapy.

Pulmonary arteriovenous malformations (PAVM) represent pulmonary vascular anomalies and in majority of cases are congenital in origin. We report a case that presented with predominant complaints of dyspnoea on exertion and cyanosis. Clinical examination revealed a bruit in the left lower interscapular and infrascapular areas which led us to suspect PAVM. The diagnosis was established on the basis of computed tomography (CT) thorax with three-dimensional virtual reconstruction technology (3D VRT) images and pulmonary angiography. Patient was successfully treated by embolotherapy using steel coils, which resulted in 90% exclusion of the aneurysm from the rest of the systemic circulation.

Upper lobe fibrosis in ulcerative colitis.

We report a case of ulcerative colitis where chest radiograph and high-resolution computed tomography (HRCT) of the lungs revealed bilateral upper lobe fibrosis, which was misdiagnosed and treated as pulmonary tuberculosis. The diagnosis of ulcerative colitis was confirmed by sigmoidoscopy and rectal biopsy.

Chronic thromboembolic pulmonary hypertension--diagnosed on spiral CT angiography.

Chronic thromboembolic pulmonary hypertension (CTEPH) is a part of the spectrum of venous thromboembolism in which pulmonary thrombus fails to resolve, resulting in occlusion of the major pulmonary artery. Diagnosis of this disease is important as it is potentially curable by pulmonary thromboendarterectomy. A case of CTEPH diagnosed non-invasively on spiral computerized (CT) pulmonary angiography is reported.

Hemorrhagic pleural effusion due to pleural hemangioma.

We report a rare case of isolated pleural hemangioma presenting as recurrent, haemorrhagic pleural effusion. Chest radiograph and computed tomography of chest was suggestive of left apical tumour. Diagnosis of hemangioma was revealed only after thoracotomy and excision of the tumour mass.

Post cardiac injury syndrome--one more cause of false positive IgG, IgM antibodies in pleural fluid against antigen-60 of Mycobacterium tuberculosis.

Post cardiac injury syndrome (PCIS) is known to occur following myocardial infarction, cardiac surgery, blunt chest trauma, percutaneous left ventricular puncture and pace-maker implantation. The diagnosis is one of exclusion. We report a case of PCIS following cardiac surgery who showed false positive IgG, IgM antibodies to antigen A60 of Mycobacterium tuberculosis in pleural fluid.
