Further observations on dental parameters of trigeminal and atypical facial neuralgias.

One hundred thirty-one patients with primary trigeminal neuralgia and 77 patients with atypical facial neuralgia or pain were treated by oral surgical procedures, with complete or almost complete pain remission in 88% of the cases and without persistent residual anesthesias, dysesthesias, or dysalgesias. The following conditions were related to patients' pain perceptions: cavities in alveolar bone at tooth extraction sites, bone fistulas, periodontal infections, and maxillary sinus infections draining into alveolar bone. The bone cavities and fistulas mentioned above were usually not visualized by standard x-ray diagnostic procedures, and their detection required a new diagnostic approach which is described. Microbiologic findings indicated involvement of a mixed, variable flora in the above conditions. Histopathologic observations of scrapings from involved bone showed a variable incidence of bone necrosis, predominantly chronic inflammatory cell populations and fibrous tissue.

Comparison of histologic subtypes of multiple biopsies of malignant lymphoma.

This is a retrospective study of 163 patients with Hodgkin disease and 364 patients with other lymphomas who were admitted to one Cancer Hospital (1940--1971). All the histological slides were reviewed and reclassified by one pathologist. In patients who had multiple and sequential biopsies (16 with Hodgkin disease and 97 with non-Hodgkin lymphomas), the histological subtypes remained the same in over 60% of the cases. When changes did occur, the neoplasms progressed toward histologically more malignant forms.

A short course - low dose preoperative irradiation of endometrial carcinoma. Preliminary report.

This is a preliminary and comparative report of a randomized prospective study in which 14 unselected women with a proven histological diagnosis of endometrial adenocarcinoma were subjected to a course of external irradiation treatment with total 1,500 rad mid-pelvic dose delivered on five consecutive days followed by total abdominal hysterectomy and bilateral salpingo-oophorectomy. Surgery was performed within 1 week after the completion of radiation therapy. At the same time, 15 additional patients with identical histological diagnosis were treated with the conventional external irradiation technique of 5,000 rad delivered in 5 weeks followed by surgery 6 weeks later. The stage indicates no significant differences between these two radiotherapeutic modalities in regard to survival rate, recurrences or complications. The short course-low dose approach markedly reduces the cost and overall treatment time.

Metastatic carcinomatosis of the liver mimicking cirrhosis: case report and review of the literature.

Carcinomatous involvement of the liver mimicking cirrhosis is a rare complication of metastatic carcinoma, most frequently seen with scirrhous carcinoma of the breast. The case of a 46-year-old woman with breast carcinoma presenting with ascites, jaundice, spider angiomata, and portal hypertension is reported. On hepatic scan the liver appeared to be almost the same size as the spleen, a finding frequently seen in cirrhosis. A chemotherapeutic program was introduced, but was of no benefit, and the patient died with uncontrolled esophageal bleeding. The prominent histologic feature on autopsy was intense stromal fibrosis and intravascular tumor infiltration with compression of vessels of the portal system.
