Strategies to optimize the cardiorespiratory status of the critically ill.

The goal of cardiorespiratory monitoring is to evaluate the components of oxygen delivery and consumption. Parameters obtained from the physiologic profile are used to assess and optimize oxygen transport to meet the tissue needs of the critically ill patient. Threats to the process of tissue oxygen balance can lead to inadequate utilization at the cellular level. Therapeutic interventions can be implemented to optimize the cardiorespiratory status of the critically ill patient. Intervention strategies are not directed at simply achieving normal values or even supranormal values but at identifying the relation of oxygen delivery to oxygen consumption to potentially eliminate the development of oxygen debt.

Adult

Right ventricular volumetric monitoring.

The goal of invasive hemodynamic monitoring is to evaluate the components of oxygen delivery and consumption. Parameters obtained from the physiologic profile are used to assess and optimize oxygen delivery to meet the tissue needs of the critically ill patient. Oxygen delivery is defined as cardiac output multiplied by the arterial oxygen content. Research has demonstrated that adequate increases in oxygen delivery improve survival. The primary intervention for optimizing oxygen delivery is appropriate fluid resuscitation. Classic endpoints of adequate fluid resuscitation have been pressure-based parameters, which possess many inherent assumptions. Current clinical issues are directed toward assessment of the patient's status in relation to the oxygen supply-and-demand balance. Identification of therapeutic interventions to achieve the goal of increasing oxygen delivery are paramount. Evaluation of the role of the right ventricle (RV) in biventricular performance and incorporation of volumetric measurements to assess the critically ill patient are presented.

The right ventricle: significant anatomy, physiology, and interventricular considerations.

The crucial role of the right ventricle for optimal global cardiac function has only recently become appreciated. Once thought of as dispensable, the right ventricle now is viewed as an integral component of biventricular function. Key anatomical and physiological differences between the ventricles affect the right heart's response to alterations in the determinants of cardiac output. Disruption of the strong interrelationship between the two ventricles can lead to the pathophysiologic sequelae of biventricular failure.
