[Evaluation of surgical renal revascularization in renovascular arterial hypertension. Measurement by 99m Tc DTPA scintigraphy].

The effects of surgical renal revascularization on both hypertension and kidney function was assessed in 16 patients with renovascular hypertension (14 unilateral). We have taken a special interest to the split renal function studied with DTPA renogram. The function of the operated kidney was improved in 9 patients and not improved (worsened or unchanged) in 7. Hypertension was cured in 25% and improved in 56% of the patients, without correlation with split renal function outcome.

[Isolated dissection of the renal artery treated by conservative surgery in 3 patients].

We reported 3 cases of male patients with isolated dissecting aneurysm of the renal artery. The clinical presentation included lumbar pain followed by hypertension (malignant in 2 cases) in the next 3 months. These three patients have underwent surgical renal artery repair. At maximal follow-up (4, 5, 11 years after surgery), the blood pressure was normalized in these 3 patients (with one antihypertensive drug in 2) and the renal function remained stable.

Simplified urinary oxalate determination using an enzyme electrode.

An enzyme electrode for urine oxalate measurement has been produced using acrylamide gel-entrapped oxalate decarboxylase retained over a CO2 sensor. Urine required pre-treatment with EDTA, but oxalate extraction was not necessary and inhibition by phosphate and sulphate was not apparent until after 10-14 days. The linear range was 0-0.2 mmol/l. Analysis of 50 urine specimens diluted 1 in 10 in pH 3.0 glycine buffer showed good correlation with a widely-used colorimetric method (y = 1.101x-0.018, r = 0.955).

[Contribution of 99mTc MAG3 in hypertension in an elderly subject with renal artery thrombosis].

To ascertain the responsibility of renal artery thrombosis for the genesis of arterial hypertension, the methods available are not sufficiently sensitive and reproducible, and the residual revascularization maintained by a vicarious arterial network is not easy to demonstrate. Renal scintigraphy with technetium 99m-labelled mercaptoacetyltriglycine (MAG3) was performed in 4 old and severely hypertensive male patients with chronic thrombosis of one renal artery. With this tracer the thrombotic kidney was detected lately and weakly at the vascular and nephrographic stages. When the functions of both kidneys were compared, that of the affected kidney was always estimated at less than 5%. At the excretory stage a residual activity could be detected in the kidney with renal artery thrombosis. Owing to the specific biophysical behaviour of the tracer, MAG3 scintigraphy coupled with arteriography and assays of plasma renin activity in the veins contribute to the decision concerning nephrectomy.

[Pheochromocytoma disclosed by isolated inflammatory syndrome].

Among the atypical forms of phaeochromocytoma the isolated inflammatory form is rare and difficult to diagnose clinically. The authors report such a case, where computerized tomography and magnetic resonance imaging contributed enormously to the diagnosis.
