Abnormal high natural radium concentration in surface waters.

Unexpected high 228Ra concentrations, up to 2 Bq 1(-1), were found in waters of a coastal lagoon close to a monazite sand separation plant. Due to their use as process waters in this plant, the initial supposition was a contamination related to its operation. However, it was concluded that these abnormal radium concentrations had a natural origin, springs at the lagoon head area with high 228Ra and 226Ra concentrations. The strong relationship among radium and light rare-earth elements (LREEs), the observed 228Ra/226Ra activity ratio and the rare-earth element pattern in the spring waters suggested that monazite is the main source of nuclides for water, indicating the disturbance of monazite chemical stability by the combined effects of low pH and high salinity. Both factors combined allow relatively low mobility of thorium, but, on the other hand, a relatively high mobility of radium and LREEs.

Environmental impact studies of barium and radium discharges by produced waters from the "Bacia de Campos" oil-field offshore platforms, Brazil.

Produced water samples from different E&P offshore petroleum platforms, belonging to the Bacia de Campos oil field, Brazil, were analyzed for barium, 226Ra and 'Ra. The concentrations measured are in the range of 0.36-25.7 mg l(-1) for barium, 0.012-6.0 Bq l(-1) for 226Ra and <0.05-12.0 Bq l(-1) for 1Ra. A strong correlation between the concentration of barium and radium isotopes was observed (226Ra: r2=0.926: 228Ra: r2=0.785). Additionally, seawater and sediment samples were taken at different distances (from 250 to 1,000 m) from the two selected platforms. Water samples were analyzed for dissolved and particulate barium, 226Ra and 225Ra and the sediment samples for total and leachable barium, 226Ra and 228Ra. The results showed that even for the shortest sampling distance (250 m) from the discharge point, barium, 226Ra and 228Ra concentrations are similar to the local background, indicating that dispersion by local currents is enough to minimize environmental impacts.

226Ra, 228Ra and 228Th in scale and sludge samples from the Campos Basin oilfield E&P activities.

More than 40 scale and sludge samples from the Campos Basin oilfield were analysed in terms of 226Ra, 228Ra and 228Th. Although the exact origin of the samples was not known, scale samples derived from production pipes and sludge samples from water-oil separation units. Reported values ranged from 0.13 to 331 kBq.kg(-1) for 226Ra, from 0.10 to 245 kBq.kg(-1) for 228Ra and <0.10 to 272 kBq.kg(-1) for 228Th in sludge samples. The concentrations in scales were much more regular than in sludges and ranged from 16.2 to 93.2 kBq.kg(-1) for 226Ra, from 4.0 to 36.9 kBq.kg(-1) for 228Ra and from 4.5 to 18.5 kBq.kg(-1) for 228Th. Based on the Basic Safety Standards recommendations and on the derived results, these activities cannot be regarded as exempted and specific procedures should be designed for their radiological control.

Natural radionuclides in Brazilian mineral water and consequent doses to the population.

Based on production data published by the Brazilian Mineral Water Producers Association, bottled mineral waters were bought at the market, covering, at least, 50% of the annual production of each Brazilian geographical region. 228Ra, 226Ra and 210Pb were analyzed and the weighted mean values obtained for Brazilian mineral waters were 0.097, 0.027 and 0.066 Bq l-1 respectively. Individual effective dose rate estimates due to ingestion of mineral water in Brazil were of the order of 10-100 microSv yr-1, depending on the geographical region. Based on the available data, it was also possible to evaluate the per caput effective dose rate and the highest value obtained was 4.3 microSv yr-1 for the northern region.

[Paraneoplastic subacute cerebellar degeneration: case report].

We report a 40-year-old woman with subacute cerebellar degeneration associated with ovarian cancer. We briefly review the clinical and laboratory features of this syndrome, and emphasize the importance of its prompt recognition, which many times makes possible the early detection and treatment of the primary disorder.

[Spontaneous intracranial hypotension: case report].

Spontaneous intracranial hypotension is a rare syndrome associated with postural cephalalgia and low pressure of the cerebrospinal fluid. We report the case of a 45 years-old man with spontaneous intracranial hypotension with MRI diffuse meningeal enhancement.

[Evaluation of a new MRI cholangiography sequence with a half-Fourier acquisition technic].

PURPOSE: The aim of this study was to evaluate a new MR cholangiography sequence in which thick slices are obtained with half Fourier acquisition (SSFSE) in biliary obstructive diseases. PATIENTS AND METHODS: Fifty patients with suspected bile duct obstruction were prospectively investigated with MR cholangiography using SSFSE sequence with thick slices. The gold standard was ERCP (n = 40), percutaneous cholangiography (n = 1) or surgical findings (n = 9). According to this gold standard, 17 patients had bile duct lithiasis and 21 had neoplastic obstruction. RESULTS: The sensitivity and specificity of MR cholangiography were 100% and 94% for the diagnosis of lithiasis obstruction and 95% and 97% for neoplastic obstruction. Agreement between the MR cholangiogram and the gold standard was good whatever the site of obstruction (range of kappa values: 0.79-1). CONCLUSION: MR cholangiography with SSFSE sequence is a useful, precise and easy to use technique. Acquisition time for thick slices is short (< 2 sec) limiting cardiorespiratory artefacts and eliminating the need for post-treatment.

[Sporadic amyotrophic lateral sclerosis. Diagnostic criteria].

The authors report two cases of amyotrophic lateral sclerosis (ALS) misdiagnosis (a craniocervical junction disorder, and a cervical spinal cord ependymoma). They review some causes of ALS-like syndrome and propose a protocol to be adopted for the study of all patients who present clinical abnormalities suggesting ALS.

[Motor neuron disease: metabolic evaluation].

The authors studied serum and urinary calcium and phosphorus levels, as well as abnormalities on the spine of 30 patients with motor neuron disease. The authors believe in multifactorial aspects in the pathogenesis of motor neuron disease, calling special attention to toxic and metabolic factors.

[Isolated vasculitis of the central nervous system and involvement of the 8th cranial nerve: rare manifestations of acquired immunodeficiency syndrome].

The authors report the case of an AIDS patient with rare neurologic manifestations: primary vasculitis of the central nervous system and VIII cranial nerve dysfunction. The authors make a review on the subject, and call special attention for the differential diagnosis. In fact, the patient, a 36 year old woman, with promiscuous life, presented with dizziness, gait ataxia, nausea, headache and hypoacusia. Seven days after the admission, she noted blurred vision in both eyes and soon she became blind. The physical examination showed bilateral optic neuritis and vestibulocochlear dysfunction, stiff neck and fever. No abnormalities were detected on CT scan. CSF showed 40 mononuclear cells/mm3, 79 mg/dl of proteins and normal glucose content. Microbiological research was negative. Serum anti-HIV test was positive. The hypothesis of primary CNS vasculitis was made, and pulse methylprednisolone therapy was introduced with good recovery of neurological syndrome except for persistent amaurosis.

[Iatrogeny. The importance of clinical diagnosis. Myopathies induced by clofibrate].

The authors describe the neurological manifestations of a female patient with hypercholesterolemia who developed myopathy in the course of her treatment with clofibrate. After the drug was tapered off, the neurological signs and symptoms disappeared. Therefore, attention is called for the importance of the differential diagnosis of iatrogenic myopathies with polymyositis.

Propylthiouracil and peripheral neuropathy.

Peripheral neuropathy is a rare manifestation in hyperthyroidism. We describe the neurological manifestations of a 38 year old female with Graves' disease who developed peripheral neuropathy in the course of her treatment with propylthiouracil. After the drug was tapered off, the neurological signs disappeared. Therefore, we call attention for a possible toxic effect on peripheral nervous system caused by this drug.

[Intracranial saccular aneurysm: report of 3 cases in a same family].

The authors report the cases of three patients from the same family, all with intracranial saccular aneurysm (left carotid artery, anterior communicating artery, and middle cerebral artery). All three patients were operated on with good recovery and no complications. The authors call attention for some etiopathogenic aspects of familial saccular aneurysms.

Distribution of 137Cs in soils due to the Goiânia accident and decisions for remedial action during the recovery phase.

In September 1987, a powder radioactive source was removed from a teletherapy machine in Goiânia, Brazil. Subsequently, it was ruptured in a residential garden causing the dissemination of 137Cs throughout the city. Soil resuspension processes and burial of contaminated house waste in unused gardens were the major contributors to the Cs dissemination in soils at the secondary contaminated sites. Only locations within a radius of 50 m from the primary contaminated sites presented the need for remedial action. The radiation dose-rate measurements and the soil profiles were good indicators of the extent of the secondary contamination and were fundamental for the decisions taken regarding decontamination procedures. In cases of surface contamination, 60% on average of the total activity remained in the upper 1.5-cm layer over the first 5 mo after the accident, and topsoil removal proved to be an effective procedure for decontamination.

Cesium-137 in the Goiânia waterways during and after the radiological accident.

The main contamination areas in Goiânia were exposed soil surfaces, and with the first rainfalls, 137Cs quickly reached the Meia-Ponte River, the main local waterway. Regular measurements of bottom sediment, surface water, and fish were begun 3 wk after the accident. In the first survey, 137Cs was detectable in sediments up to 12 km downstream from the accident area. Maximum 137Cs concentrations found in sediments of the Meia-Ponte River were 1.3 kBq kg-1 dry weight and 200 Bq kg-1 fresh weight for fish. Cesium-137 was always less than 10 Bq L-1 in surface waters plus suspended particulates, even in the most contaminated creeks. Because it was the beginning of the rainy season, downstream sediment transport was enhanced. This resulted in a decrease of 137Cs concentrations in sediments of the urban area by a factor of approximately 20, 5 wk after the accident. The main sedimentation area for suspended sediments in this portion of the river is the Rochedo Reservoir, which is 80 km downstream from the accident area. In a survey performed 10 mo after the accident, 137Cs concentrations in the reservoir were up to 14 Bq kg-1 for fish, 1 Bq kg-1 for aquatic plants, and 100 Bq kg-1 dry weight for bottom sediments.
