[Alveolar echinococcosis of the liver--computerized tomography findings].

In order to ascertain the typical computed tomographic findings of hepatic alveolar echinococcosis, 24 computed tomograms of 19 patients were evaluated. The liver was involved in all cases whereas the diaphragma was infiltrated in 32%, and the retroperitoneal area in 42%. The right liver lobe was affected in 65%. Both before and after intravenous bolus contrast medium administration, the lesions were mainly inhomogeneous and of low density; a masking of the lesions due to the contrast medium administration was not observed; the enhancement pattern was irregular. Calcifications were detected in 96% of the cases, cystic structures in 50%, and cholestasis in 54%. On the basis of the crucial finding of calcifications in combination with the other typical observations, CT seems to be very suitable for the evaluation of hepatic alveolar echinococcosis.

[The early recognition of infected fluid collections in acute pancreatitis in computed tomography].

UNLABELLED: Infected accumulations of abdominal fluid are a severe complication and the major cause of death (mortality rate between 20 and 60%) in patients suffering from acute pancreatitis. In 93 patients with confirmed acute pancreatitis including 28 with surgically confirmed pancreatic abscess CT findings (volume of pancreatic and extrapancreatic necrosis, capsular enhancement, gas bubble formation and contact between fluid collection and intestinum) were retrospectively correlated with clinical and operative data to assess abscess formation. This revealed a sensitivity of 0.57 in respect of pancreatic necrosis; for extrapancreatic necrosis the sensitivity was 0.39, for capsular enhancement 0.5, for gas bubble formation 0.46 and for intestinal contact 0.93. The specificity was high for gas bubbles (1.0) and intestinal contact (0.91) and low for the other criteria. IN CONCLUSION: There is a strong relationship between fluid accumulations in direct contact with intestinal structures and the occurrence of abscess. This parameter enhances the reliability of CT in the early detection of extrapancreatic infection.

[Dynamic arthro-CT in the diagnosis of recurrent shoulder dislocations].

"Dynamic" double contrast arthro-CT in internal and external rotation was performed on 42 patients with recurrent subluxation of the shoulder. The differential distribution of contrast and air caused by the rotation resulted in optimal demonstration of the capsulo-labrale components responsible for joint stability. In 22 patients there appeared to be an indication for arthroscopy; this confirmed the previous findings. In all cases the anterior part of the capsule and the middle and lower labrum were seen best during internal rotation. In this way all the 15 anterior capsule lesions could be seen, as well as 16 out of 17 Bankart lesions; it also showed one lesion of the middle labrum which had been masked by a haemarthrosis during external rotation. The study also defined lesions of the upper labrum caused by a different pathological mechanism which was present in 7 patients (6 Andrews and 1 S.L.A.P. lesion). These and 2 posterior labrum tears could be diagnosed during external rotation. Sensitivity for lesions of the anterior labrum was 95%.

Bone densitometry and histomorphometry in patients with hairy cell leukemia.

Tumor necrosis factor alpha, the cytokine that participates in the autocrine growth control of hairy cell leukemia has strong bone resorptive properties. This prompted us to look for bone involvement in HCL. Bone mineral density (BMD) was not decreased in 14 HCL patients who did not have radiographic evidence of bone destruction. Osteopenia was found in only two HCL patients with skeletal complications of the disease and bone pain preceded diagnosis in both cases. Lymphomatous infiltration of the vertebral bodies T 12/L1 was confirmed histologically in the female patient while bilateral necrosis of the femoral head preceded the diagnosis of HCL in the male patient. Histomorphometry of bone biopsy samples was performed in another 12, previously untreated male HCL patients. Trabecular bone volume was found to be reduced in 11, greatly reduced in 5 of them, while osteosclerosis was found in only one patient. The increase of trabecular bone pattern factor, a new parameter for simple quantification of trabecular interconnection indicated a poorly connected trabecular lattice in eight of these 12 patients.

[Sonographic aspects of clinically suspected acute appendicitis].

Revealing a sensitivity of 97.5% and a specificity of 95.12% this study emphasizes the excellent diagnostic value of sonography in the elucidation of patients with clinical signs of "acute appendicitis". Evaluation of morphology on the one hand and maximal diameter of the organ and the organ wall on the other, in conjunction with the clinical presentation, allows a restrictive management of the surgical treatment and thus decreases the rate of negative laparotomy.

[Percutaneous aspiration thromboembolectomy in the treatment of acute occlusion of the lower leg arteries].

Percutaneous aspiration thrombembolectomy (PAT) is a very suitable method for the recanalization of the popliteal and lower limb arteries after embolic occlusion. In thrombotic occlusion in patients with arteriosclerotic disease, PAT can easily be combined with other interventional procedures, yielding good results. With the use of PAT the dose of regionally effective fibrinolytic drugs, which may be additionally administered, can be significantly reduced.

[Abdominal aortitis in polymyalgia rheumatica. Disease course as documented with computerized tomography and proton spin tomography].

In a 59-year-old woman, after four years of ambiguous and unidentified course of her disease (feeling of weakness, inappetence, weight loss, pain in the shoulder girdle, sub-febrile temperatures, increased BSR), polymyalgia rheumatica was eventually diagnosed. The findings at the abdominal aorta were particularly marked: sonography revealed a 10 mm wide echo-poor fringe around the aorta which in the computed tomogram became slightly more echogenic after injection of contrast medium. In the magnetic resonance tomogram (MR) the aortic wall was homogeneously thickened and well-defined against the retroperitoneal tissue. Subsequent to the diagnosis a therapy with glucocorticoids was initiated and strictly continued for by now 18 months. This resulted in long-term clinical remission. CT and MR proved highly suitable for long-term monitoring (> 5 years).

[The postoperative MR tomographic findings following the removal of a hypophyseal adenoma].

36 patients with pituitary adenomas were examined via MRI to describe physiological changes and to visualise a residual tumour. Pre- and postoperative examinations included T1- and T2-weighted SE sequences. T1-weighted images were obtained in sagittal and coronal orientation pre- and post-Gd-DTPA application and T2-weighted images in coronal orientation. In 12 cases a residual tumour was found. Its signal intensity and contrast enhancement were similar to those of the primary tumour. Implanted material could be distinguished by localisation, decrease in volume and different signal intensity. The behaviour of contrast enhancement was helpful, since implanted material showed a rim enhancement. In our experience a sensitive imaging protocol in the follow-up of operated pituitary adenomas would be an early examination three months postoperatively followed by a control examination after one year. Information on the size and localisation of the primary tumour and the performed operative procedure is essential.

[The CT classification of intra-articular calcaneus fractures].

93 patients with 102 intraarticular calcaneus fractures (ICF) were examined by CT from 1986 to 1992. The images were evaluated with the use of a modified classification based on the number of fractured heel bone facets (2 facets in 4.8%, 3 facets in 53.9%, 4 facets in 32.3%, comminution in 8.8% of the fractures), the involvement of the calcaneus-cuboid joint (60.8%) and the fracture mechanism (tongue-type in 28.4%, joint depression in 62.7%) with the weight-bearing calcaneal compartments taken into special consideration. In that way, each intraarticular calcaneus fracture could be scored, enabling a fast diagnosis comprising factors relevant for the therapy and prognosis.

[Carotid angiography by magnetic resonance].

Using magnetic resonance imaging, arteriosclerotic lesions of the supra-aortic vessels may be well demonstrated. The comparison between 3 magnetic resonance angiography (MRA) techniques (Spin Echo, Gradient Echo 2D FLASH, Gradient Echo 3D FISP) and Digital Subtraction Angiography (DSA) as reference in 50 patients suspicious of stenotic lesions proved the high reliability of these methods. Specially in determining the degree of stenosis, a good correlation between DSA and SE in 87%, between DSA and 2D FLASH in 78% and between DSA and 3D FISP in 83% could be assessed. Since 3D technique may exaggerate the degree of stenosis, we didn't observe any false negative result in severe stenosis. Therefore we think that the 3D technique is a safe technique which enables a good overview of the supra-aortic vessels and could therefore be suitable for screening.
