Oxygen regulation of development of the photosynthetic membrane system in Chloroflexus aurantiacus.

Oxygen levels which control induction of the assembly of the pigment-protein photosynthetic polypeptides in dark-grown Chloroflexus aurantiacus were determined. The induction signal by low-oxygen tension is not directly related to the respiratory competence of these photosynthetic cells. Cytochrome c554, the primary electron donor to P865+ of the reaction center, is not present in dark-grown respiratory cells but is induced in parallel with bacteriochlorophylls a and c and at similar oxygen partial pressure. The development of these components of the photosynthetic apparatus and its electron transport chain is completely independent of the presence of any detectable light or bacteriochlorophyll c or a pigments in C. aurantiacus.

Comparison of infusions of morphine and lysine acetyl salicylate for the relief of pain after surgery.

The effect of a constant i.v. infusion of lysine acetyl salicylate (LAS) on pain after operation was compared with that of a constant infusion of morphine in 30 patients undergoing unilateral inguinal herniorrhaphy. LAS provided analgesia equivalent to that provided by morphine and was associated with significantly less drowsiness, nausea and vomiting. No patient in either group was noted to suffer from respiratory depression. No untoward side effects were noted during or following the administration of LAS.

Comparison of infusions of morphine and lysine acetyl salicylate for the relief of pain following thoracic surgery.

A double-blind study comparing the effectiveness of a continuous infusion i.v. of lysine acetyl salicylate (LAS) with an infusion of morphine for the treatment of pain following pulmonary surgery is described. Mean pain scores in the two groups were not significantly different at any stage during the 24-h period of study. LAS was not associated with any significantly greater blood loss in the period after operation. The incidence of drowsiness, nausea and vomiting, and the need for antiemetic medication were similar in both groups.

Parenteral aspirin for pain relief in day-case dental anaesthesia. A randomised double blind placebo controlled trial.

Forty patients undergoing day-case anaesthesia for surgical extraction of third molar teeth received either lysine acetylsalicylate 1.8 g in 10 ml of saline intravenously or a placebo injection of 10 ml of saline intravenously just prior to induction of anaesthesia. Lysine acetylsalicylate was associated with a significantly more satisfactory postoperative recovery overall. However, there was no significant difference in mean pain scores between the two treatment groups at 1, 6, 12 or 24 hours following administration. Both treatment groups self administered additional oral analgesics to the same extent during the first 24 hours following surgery.

Ingestion of egg raises plasma low density lipoproteins in free-living subjects.

In addition to their usual diet, 17 lactovegetarian college students consumed 400 kcal of test foods per day containing one extra-large egg for three weeks and similar isocaloric eggless foods for an additional three weeks in a randomised double-blind crossover trial. Ingestion of the egg increased dietary cholesterol from 97 to 418 mg per day. Mean plasma low density lipoprotein (LDL) cholesterol was 12% higher (p = 0.005) and mean plasma apolipoprotein B was 9% higher (p = 0.007) when eggs were being consumed than during the eggless period. Mean plasma high density lipoprotein cholesterol, apolipoproteins A-I and A-II, very low density lipoprotein cholesterol, and total triglycerides did not change significantly. Thus, ingestion of egg seems selectively to raise cholesterol and protein in LDL particles in the plasma of free-living normal people. Plasma LDL may be more sensitive to cholesterol at low intakes than at moderate to high intakes.

The feasibility of adjuvant surgery in limited-stage small cell carcinoma: a prospective evaluation.

Forty patients with limited-stage small cell carcinoma of the lung were prospectively evaluated for adjuvant surgery after intensive chemotherapy to determine resectability. All patients giving informed consent and having a Karnofsky performance status greater than or equal to 50% were included in the study, which ran from May, 1980, to September, 1982. Ages ranged from 40 to 70 years (median, 59 years). One patient was lost to follow-up. Thirty-nine patients were evaluated for operation 9 to 15 weeks after diagnosis and after having received two to four cycles of chemotherapy intravenously every 3 weeks (cyclophosphamide, 1,000 mg/m2; doxorubicin, 50 mg/m2; vincristine, 1 mg/m2; VP-16, 300 mg/m2). Two patients had clinical Stage I tumors; 12 patients, Stage II; and 25 patients, Stage III. At the time of reevaluation there were 13 (33%) complete responders, 21 (54%) partial responders, and 5 (13%) with stable disease. Eleven (28%) of the 39 patients underwent thoracotomy using standard resection criteria for non-small cell carcinoma. Eight of these 11 had resectable lesions (2, Stage I; 3, Stage II; 3, Stage III); five pneumonectomies and three lobectomies were performed. Tumor was present in six of eight specimens. Twenty-eight patients were not candidates for operation for various reasons: poor pulmonary function, 5; unresectable tumors, 10; refusal, 6; very poor medical condition, 6; and primary site not identified, 1. Median survival for complete responders was 17 months and for partial responders, 11 months. We have prospectively identified suitable candidates for adjuvant surgery among the total group (denominator population) of patients with limited-stage small cell carcinoma.(ABSTRACT TRUNCATED AT 250 WORDS)

The anaesthetic management of the Eisenmenger syndrome.

The Eisenmenger syndrome is a form of cyanotic congenital heart disease not usually amenable to corrective surgery. It is, however, compatible with leading an active life in early adulthood and due to advances in medical therapy it is not uncommon for patients with this syndrome to live to 30 years or more. Occasionally, therefore, anaesthetists and surgeons will be required to care for these patients when they present for incidental surgery. This article reviews the appropriate measures for the safe operative and perioperative care of patients, based on an understanding of the pathophysiological changes which occur in the Eisenmenger syndrome.

The relation of the height of primary school children to population density.

The relation of the height of primary school children to population density, as measured at the 1971 census, was investigated for children in 21 study areas in England, using data from the 1972 survey of the National Study of Health and Growth. Shorter stature was found with increasing population density, and this relation remained after allowing for child's birthweight, mother's height, father's height, social class, number of siblings, and additionally a measure of home overcrowding. The results are in contrast to those found elsewhere, and are unlikely to indicate a cause-and-effect relationship between population density and height. The findings cannot be explained either by known differences between urban and rural areas in social characteristics or by plausible hypotheses of genetic heterogeneity.
