High duplication of the internal jugular vein: clinical incidence in the adult and surgical consequences, a report of three clinical cases.

Duplication of the internal jugular vein (IJV) is a rare malformation. Three intraoperative cases are reported. In our personal experience, the clinical incidence of the anomaly is approximately 4 per 1,000 unilateral neck dissections. The venous duplication is at a variable height, affecting the superior part of the IJV. The lateral branch of the accessory nerve (XI) always passes medially to the anterior vein and laterally to the posterior vein, between the venous duplication. This is most often unilateral but sometimes bilateral. The IJV may be normal, dilated or ectatic. The discovery of this anatomical variation has practical implications during cervical lymph node clearance, either functional or radical, during oncological surgery necessitating viewing the IJV and its affluents and the lateral branch of the accessory nerve. The embryological explanation suggests a topographical "conflict" between the development of the IJV and the lateral branch of the accessory nerve. Value of two mortality assessment techniques for organ cultured corneal endothelium: trypan blue versus TUNEL technique.

BACKGROUND/AIM: It is known that trypan blue staining is not a good predictor of loss of corneal endothelial cells (ECs) during organ culture. As it is primarily an indicator of membrane integrity, it would also not be expected to identify ECs undergoing apoptosis. The aim of this study was to determine the ability of the in situ TdT dUTP mediated nick end labelling (TUNEL) technique to detect cell death in the corneal endothelium caused by apoptosis during organ culture, compared with conventional vital staining with trypan blue. METHODS: 31 human corneas were organ cultured at 31C for 3-35 days. Staurosporine was used to induce apoptosis in five control corneas. The endothelium was assessed by trypan blue and by the in situ TUNEL technique. The percentages of trypan and TUNEL positive ECs were compared. Their links with sex, donor age, time from donor death and organ culture, initial and final EC density and cell loss were studied. RESULTS: TUNEL stained ECs were observed in all corneas. TUNEL positive ECs were mostly located either in corneal folds or at the periphery of corneal folds showing central shedding. The mean percentage of cell death at the end of storage, assessed by the trypan blue technique, was 1.47% (SD 2.63, range 0.03-12); assessed by the TUNEL technique it was 12.7% (SD 16.4 range 0.6-65.5). There was a significant correlation between the two techniques (r = 0.7, p<0.001). The percentage of TUNEL stained ECs was correlated negatively with EC density at the end of storage (r = -0.47, p <0.005) and positively with percentage EC loss during storage (r = 0.46, p < 0.05). CONCLUSION: This study demonstrates that organ cultured corneas systematically carry non-viable ECs that are implicated in cell death by apoptosis and go undetected when trypan blue staining is used. Because the in situ TUNEL assay detects earlier events in the cell death process than does trypan blue, it should be used to quantify endothelial viability, especially for experiments with new storage media.

In situ immunohistochemical study of Bcl-2 and heat shock proteins in human corneal endothelial cells during corneal storage.

AIM: To investigate the expression of Bcl-2 and heat shock proteins (HSPs), which are known to increase cell survival, in human corneal endothelial cells (HCECs) of corneas stored in organ culture. METHODS: 32 paired corneas were randomly assigned to either a short or a long storage time. The flat mounts of endothelium were examined after immunostaining with monoclonal antibodies to Bcl-2 and HSP 27, 60, 70, and 90. RESULTS: HCECs expressed generally all the proteins studied. Bcl-2 expression was weaker in the long stored corneas (p=0.035). There was no relation between immunostaining, age, sex, or death to culture time. Frequently some Descemet membranes carried negative cells preferentially located in folds and exhibiting morphological changes consistent with swelling cells corresponding to early stages of apoptosis. CONCLUSION: Expression of these cytoprotective proteins reflects the high level of HCEC resistance to stresses induced by organ culture. The decreased immunostaining of Bcl-2 in the long storage group could act in cellular loss currently observed with storage time. The negativity of Bcl-2 and HSP labelling in corneal folding may be related to apoptosis.

[Epithelial invasion of the corneal button. Good techniques for human corneal harvesting].

Corneoscleral excision is the most common harvesting technique used in France before keratoplasty. We report two cases of corneal button epithelial invasion during organ culture caused by an excision that was too small. Two corneas stored for 23 days in organ culture at 31 degrees C had a cellular proliferation on the surface of Descemet's membrane. Cornea button diameters were respectively 11 and 13mm. The epithelial origin of the invasion was confirmed by examining the flat mount of Descemet's membrane by inverted microscopy and standard histopathology. The authors discuss the origin and the potential consequences of the epithelial invasion and emphasize the importance of good corneal harvesting recommendations before keratoplasty.

[Spindle cell lipoma and 13q deletion: diagnostic utility of cytogenetic analysis].

AIM: Spindle cell lipomas are rare adipose tissues tumors. Histologically, these lesions are composed of mature adipocytes and spindle cells associated with collagen bundles. Spindle cell lipomas are benign tumors that can be difficult to distinguish from malignant tumors such as spindle cell liposarcomas, myxoid liposarcomas or well-differentiated liposarcomas. RESULTS: We report herein the description of two new cases. The first case was a deeply situated and infiltrating tumor located in the retromastoidian area. The karyotype showed the presence of two chromosomal abnormalities, a partial deletion of the long arm of chromosome 13, del(13)(q12) and a balanced reciprocal translocation t(2;6)(p16~21;p21). The second case was a subcutaneous tumor of posterior cervical localization. The karyotype showed a 13q deletion associated with a complex rearrangement of chromosomes 5, 6 and 10. The presence of the 13q deletion allowed us to confirm the diagnosis of spindle cell lipoma in both cases. This deletion has been previously described in six out the eleven published karyotype reports. The 13q deletion is usually associated with partial monosomy 16. The present case confirms that it can occur independently. The 6p21 rearrangement may also play a role in the pathobiology of this tumor, as suggested by the positive HMGIY expression detected by immunohistochemistry. CONCLUSION: Our study further illustrates that spindle cell lipomas can infiltrate the surrounding muscle and emphasizes the usefulness of cytogenetic analysis in the differential diagnosis of soft tissue tumors.

Descriptive anatomy of the cricoarytenoid articulation: application to articular dynamics in carcinology.

The descriptive anatomy of the cricoarytenoid articulation provides an essential foundation for understanding disorders of mobility of the larynx, especially in carcinology. Thirteen formaline-preserved anatomic specimens of the adult larynx were studied and 4 pathologic larynges with loss of mobility due to a malignant tumor. The cricoid and arytenoid articular surfaces showed major intra- and inter-individual variations, causing dynamic asymmetry at the glottic level. They were joined by a connective-elastic articular capsule bounding a cavity, characterized by a pseudo-meniscal synovial ridge and deep peripheral blind recesses, indicative of great articular mobility. The cricoarytenoid ligament shares in stabilizing the articulation. The posterior cricoarytenoid m. (abductor) and the lateral cricoarytenoid m. (adductor) have a motor innervation derived from the inferior laryngeal nerve, which forms an endolaryngeal arch with a ventral concavity, in contact with the lateral articular recess. The cricoarytenoid articulation thus appears as a diarthrosis possessing three degrees of liberty during movements of glottic abduction and adduction: an antero-posterior rocking movement, an antero-medial shift of the arytenoid on the cricoid, and a less marked axial rotation. Histological study of the cricoarytenoid articulation where mobility was reduced by carcinomatous infiltration showed that each articular component may be affected (muscles, cartilage, capsule, nerve), and that several components may be involved simultaneously to a minimal degree. The therapeutic implications are important, particularly in conservative laryngeal surgery.

Evaluation of global DNA hypomethylation in human colon cancer tissues by immunohistochemistry and image analysis.

BACKGROUND: Global hypomethylation of DNA is frequently observed in human tumours. This alteration is detected in early adenomas in colorectal tumorigenesis. Information is currently acquired after extraction of DNA from tissues, digestion with nucleases, and analysis by reverse phase chromatography, or treatment with restriction enzymes followed by gel electrophoresis analysis and Southern hybridisation with radiolabelled probes. AIMS: The purpose of our work was to evaluate the global methylation status of DNA in malignant lesions without loosing the histopathological features of the samples. PATIENTS: The investigation was performed on paired normal-tumour tissues from 13 patients undergoing surgical resection of colorectal adenocarcinomas. METHODS: Antibodies raised against 5-methylcytidine can be used to label methyl rich regions in interphase nuclei. This technique was adapted to the study of paraffin embedded tissues and an immunohistochemical method was developed to assess the global methylation status of individual nuclei while preserving cell morphology and tissue architecture. Computer assisted quantification of the staining intensity was performed on malignant and normal zones of human colon tissues to test the correlation between the immunolabelling signal and the respective histological patterns observed. RESULTS: Qualitative and quantitative differences were observed and measured between the normal and malignant part of each sample. Morphologically altered nuclei displayed densely labelled spots within faintly labelled areas whereas normal nuclei were darker and uniformly stained. Image analysis allowed calculation of the average integrated optical density of the nuclei in both types of tissues, demonstrating a constant and significantly lower intensity for the former type of cells.

Descriptive anatomy of the human auditory tube.

The aim of this study was to correlate current morphologic data relating to the lumen of the auditory tube. Four methods were used: dissection under the operating microscope; microendoscopy of the tubal lumen; optical and electron microscope histology; and MR or CT imaging. The auditory tube consists of two unequal cones, a small posterior third, fixed and osseous (protympanum), and a mobile fibrocartilaginous anterior two-thirds, both joined by the tubal isthmus, a short constriction which is pseudosphincteric at endoscopy. The tensor veli palatini muscle (TVPM) and the levator veli palatini muscle (LVPM) are the chief muscles that vary the tubal lumen of the fibrocartilaginous portion, which is collapsed at rest. CT and especially MR imaging allows their observation in static conditions. Serial histologic sections reveal the continuity between the TVPM and the tensor tympani muscle. The main cartilage framing the lumen varies in shape according to the level surveyed. The tubal mucosa is lined with an epithelium combining ciliated and mucus cells, involved in mucociliary drainage and gas exchanges in the auditory tube. These morphologic elements represent a basis for study of tubal physiology and for planning treatment in dysfunctions of the auditory tube.

[Paranasal mucormycosis with involvement of the sphenoid sinus].

Mucormycosis with hemipansinusitis and left orbital involvement was observed in a 66-year-old diabetic woman. Diagnosis was confirmed by mycology and pathology examinations. The patient was treated surgically with wide exeresis of necrotic tissue and medically with amphotericin B and insulin. Cure was obtained after three months of treatment at the cost of retraction of the left nostril due to infectious lysis and resection of the nasal infrastructure. The sinus involvement was exceptional compared with reports in the literature. Mucormycosis is an uncommon condition caused by opportunistic infection in immunodepressed subjects. The causal agent is a fungus belonging to the Rhizopus, Absidia or Mucor genera. Prognosis is poor with overall mortality reaching 50%.

Lack of evidence for natural cytotoxicity deficiency against human ex vivo tumour cells in allograft recipients.

BACKGROUND: Allograft recipients, who have a high risk of developing malignancies, are deficient in natural killer cells which mediate natural cytotoxicity against tumour cells. We evaluated the relevance of NK deficiency in transplant patients with regard to the natural lysis of human tumour cells. METHODS: Target cells were ex vivo tumour cells, obtained from solid tumours from kidney transplant patients and non-immunocompromised patients. Peripheral blood was extracted from kidney recipients and from normal controls. Mononuclear cells were separated after anti-CD3 and anti-TCR immunostaining to obtain NK and T cell subsets. LAK cells were obtained by in vitro IL2-activation. For each tumour, natural cytotoxicity assays were performed to compare effector cells from a kidney recipient with those from a normal control. Twenty-seven solid tumours, either allogenic or syngenic, were analysed, mainly consisting of renal, colon, and skin carcinomas. RESULTS: Natural cytotoxicity assays on K562 targets confirmed the expected NK deficiency in the kidney recipients. However, the NK and LAK cells from the kidney recipients did not kill a smaller number of tumour target cells than the controls, whatever the tumour type, under both the syngenic and allogenic conditions. CONCLUSIONS: We conclude that natural cytotoxicity against human tumour cells cannot be extrapolated from the cytotoxicity assays with established cell lines, and that natural lysis of ex vivo human tumour cells is not impaired in transplant patients.

An immunohistochemical study of the simultaneous expression of bcl-2 and p53 oncoproteins in epithelial tumors of the colon and rectum.

OBJECTIVE: Bcl-2 and p53 genes may participate in a common pathway for regulation of apoptosis. The aims of this study were (1) to study the immunohistochemical expression of the bcl-2 oncoprotein in colorectal tumors, (2) to correlate it with that of p53 protein overexpression, and (3) to compare it with histopathologic prognostic factors, such as TNM classification and grade. DESIGN: Prospective study of expression of bcl-2 and p53 oncogenes in colorectal tumors. We examined 6 colorectal hyperplastic polyps, 33 adenomas, and 61 carcinomas. SETTING: Regional academic medical center. METHODS: An immunohistochemical study with bcl-2 and p53 antibodies was performed on frozen sections of colorectal tumors. The levels of bcl-2 and p53 expression were evaluated using a semiquantitative grading system. Two-color immunohistochemistry was performed to examine the intracellular colocalization of bcl-2 and p53 in all tumors with a strong positivity for both antigens. RESULTS: Bcl-2 was expressed in 28 (85%) of the 33 adenomas, whereas p53 was expressed in only one adenoma, which had areas of in situ carcinoma. Bcl-2 and p53 were each expressed in 43 (70.4%) of the 61 carcinomas. Thirty-one (50%) of the colorectal carcinomas coexpressed the two oncoproteins. There was no correlation between the number of cells expressing bcl-2 and the number expressing p53 in a given carcinoma. No correlation was observed between the expression of bcl-2 or p53 and the established prognostic factor. CONCLUSION: Abnormal bcl-2 oncoprotein expression appears earlier than p53 accumulation in colorectal carcinogenesis. This study suggests that there is more than one sequence and mechanism of bcl-2 and p53 gene deregulation in colorectal carcinomas.

[Two benign vascular tumors of the kidney capsule].

Capsular tumors of the kidney are uncommon or poorly documented. The authors report two cases of benign vascular tumors: a so-called "vascular leiomyoblastoma" and a glomus tumor. An ultrastructural study of the first tumor demonstrated smooth muscular characteristics. Obvious histological features of glomus were seen in the second. Immunohistochemical study revealed positive immunoreactivity for epithelial and mesenchymal antibodies in the two tumors. Glomus tumor originates from the particular system of vessels in the capsule. Histogenesis of the vascular leiomyoblastoma is more controversial. This tumor could be akin to leiomyomas of the renal capsule and intra parenchymatous leiomyomas; lymphatic origin and even a glomangiomatous nature are also possible explanations.

A quantitative study of epithelial alterations during the early stages of experimental colonic tumorigenesis in mice.

The weekly administration of 1,2-dimethyl-hydrazine (DMH) by subcutaneous injection for a period of 16-20 weeks is a well known procedure for producing colonic tumors in mice and rats. Quantitative histomorphological, histochemical and kinetic studies, as well as investigation of the significance of epithelial cell density were carried out in mice between the 7th and the 91st day after the first DMH injection. These studies showed that between the 28th and the 35th day, several simultaneous alterations in the colonic epithelium involving modification of glandular form, decreased mucus secretion, an increase in epithelial cell density and an increase in the number of S phase cells (BrdU labeling index: LI). Around the 35th day, the glands tended to expand and from the 35th to the 63rd day, they were stretched and displayed compartments of dedifferentiated and non-mucinous crypts (DNMC). In these crypts the cell density became very high, reaching twice the control value on the 91st day. This feature was accompanied by alteration in cell morphology and by an increase in the available basement membrane area. A decrease in mucus secretion was apparent from the 14th day and by the 63rd day, mucus secretion was only about 60% of the control value in all crypts. The LI was increased until the 35th day following which a paradoxical and progressive decrease occurred in all glandular compartments.
