[Comparative study of MRI and x-ray computed tomography in the preoperative evaluation of rectal carcinoma].

Twenty-one patients with middle and low rectal carcinomas have been operated by abdominoperineal resection (APR) in 11 patients, and low anterior resection (LAR) with coloanal anastomosis in 7 and colorectal anastomosis in 3. The distance of the lower margin of the tumor to insertion of the levator ani on the rectal wall was correctly evaluated by CT in 15 of 21 cases (71%) and by MR in 14 of 21 (67%) while digital examination correctly assessed the distance in 17 of 21 (81%). CT and MR were unable to assess extension through the rectal wall. No significant discordance was observed between CT and MR in assessing extension to the perivesical fat, adjacent organs, pelvic side wall or lymphnodes. According to the TNM classification, MR correctly staged 76% (16 of 21) of patients while CT correctly staged 71% (15 of 21).

[Cardiac imaging using magnetic resonance].

Today, two types of cardiac imaging may be obtained in MRI. The spin-echo technique with ECG dependency permits to obtain multiple sections in all spatial planes; the morphology of the heart cavities may then be evaluated as well as the condition of the myocardium (thickness, signal, movements). The gradient-echo technique permits a quick imaging or cine-MRI. 16 to 32 images per cardiac cycle are obtained: therefore, analysis of the flows, turbulence and regurgitations. Early developments involve computer sciences: automatic analysis of the heart cavities (volume, stroke volume, etc.) and regurgitations in order to quantify them. Comparative trials show a good correlation with standard explorations. Late developments at 4-5 years: these are newly acquired techniques such as echo planar (spirals of Fourier's plane) permitting to obtain in one second, 8 stages of the heart cycle. This would then be a true and fast cardiac recording. The same techniques of computer analysis will permit to obtain precise measurements.

[Desmoid tumors of the mesentery in Gardner's syndrome. Value of x-ray computed tomography].

3 cases of desmoid tumours of the mesentery occurring as part of Gardner's syndrome are reported. In 2 cases, CAT scanning was carried out. We underline the value of this examination compared to other imaging methods. It allows the establishment of a diagnosis, investigation for complications, the operability to be assessed and is useful for monitoring patients. Its major value is in avoiding an unnecessary laparotomy, a factor certain to produce recurrence, in certain patients. Other alternative therapy may then be proposed.

[X-ray computed tomographic aspects of dermoid cysts of the ovary].

155 patients with 210 ovarian tumours were examined pre-operatively by CT from December 1984 to February 1988. 27 patients had dermoid cysts, unilateral in 25 cases and bilateral in 2 cases. These cystic teratomas were benign in 26 patients. In one case, malignant transformation occurred. CT detected 29/29 tumours (100%). Presence of fat (26/29, 90%), calcification (20/29, 69%), Rokitansky protuberance (25/29, 86%), tufts of hair (20/29, 69%), or fat fluid level (4/29, 14%), allowed to make a definite diagnosis in 28/29 cases (96%). These findings and especially demonstration of fat, appeared to be specific for dermoid cysts in our series. The diagnosis of torsion of the cyst associated with torsion of the adnexa was made in 2 patients. No abdomino-pelvic extension occurred in 26/26 patients presenting with a benign dermoid cyst and CT correctly diagnosed the lymph node and hepatic spread in the single case of cancer.

[Contribution of MRI in the staging of cancer of the bladder. Apropos of 40 cases].

56 patients with bladder carcinoma were examined by magnetic resonance in the pre-operative staging. In 40 patients where total cystectomy with enterocystoplasty and pelvic node dissection were performed, a good correlation with surgical and pathologic findings was obtained. MR examination using T1 (TR 400 ms, TE 28 ms) and T2 (TR 1,200-1,600 ms, TE 40 80 120 ms) weighted images in different planes where performed after biopsy of the bladder tumor through endoscopy and within 1 or 2 weeks before surgery. Neoplasms were characterized by site, size and growth pattern. Extension through the deep muscle of the bladder wall was correctly identified in 95% with a sensitivity of 95% and a specificity of 95%. Extension through perivesical fat was assessed accurately in 85% with a sensitivity of 66% and a specificity of 100%. Accuracy in evaluating invasion of adjacent organs was 85%, sensitivity 44%, specificity 96%. Lymphadenopathy has been accurately assessed in 97.5% with a sensitivity of 83% and a specificity of 100%. MR correctly staged the tumor according to TNM classification in 24/40 (60%) patients, overestimated the extension in 3/40 (7.5%) and underestimated the extension in 13/40 (32.5%).

[Transrenal ureteral occlusion using detachable balloons. An alternative to cutaneous ureterostomy in the treatment of vesicovaginal fistula].

The treatment of urinary fistulae is particularly delicate when they occur in the course of a neoplastic disease. The development of less invasive, but variably successful, percutaneous techniques seems essential in these advanced patients. The authors report a case of ureteric occlusion by releasable balloon allowing the simple and rapid cure of a very disabling vesico-rectal-vaginal fistula associated with recurrent pelvic tumour.

[Idiopathic spontaneous adrenal hematoma in adults: echography and x-ray computed tomography. Apropos of 5 cases].

The five patients presented with sudden lumbar pain, transient collapse, and a right hypochondrium palpable mass. The location and the type of the mass could be determined by ultrasonography which showed a septated heterogenous mass displacing the retroperitoneal fat anteriorly. Computed tomography showed in two cases tiny parietal calcifications and a spontaneous hyperdensity in one case. All the patients underwent surgery and no tumoral tissue could be found.

Primary lung cancer staging: prospective comparative study of MR imaging with CT.

Forty-four patients with bronchogenic carcinoma were studied prospectively by both computed tomography (CT) and magnetic resonance (MR) imaging of the thorax during the week preceding thoracotomy. Transaxial MR imaging sequences included T1- and T2-weighted sequences. Coronal and sagittal T1-weighted sequences were added according to tumor location. CT and MR studies were reviewed separately, and the results were compared with surgical and pathologic findings on the basis of TNM classification. No statistically significant differences were found between the two imaging methods for the evaluation of tumor extent or node involvement. T2-weighted sequences did not yield further information on tumor extent or node involvement. Additional imaging planes (coronal or sagittal) appeared useful to study chest wall invasion. Analysis of concordances and discordances did not indicate whether one modality could be substituted for the other, or which diagnostic strategy might be more useful.

[Pulmonary digital subtraction angiography. A comparative study of 2 technics. Electrocardiographic servo-assistance versus 3 images per second].

A prospective study in 60 consecutive patients evaluated gain in quality of image using ECG servo-assistance during pulmonary digital subtraction angiography (PDSA). Two groups of 30 comparable patients were randomly allocated to examination with ECG servo-assistance or three images per second technique. Criteria for assessment of quality of image were defined and used to compare results. No significant difference were noted and ECG servo-assistance failed to improve images during PDSA.

[Treatment of jaundice caused by neoplastic obstruction of the bile ducts with percutaneous placement of endoprostheses: 53 cases].

From October 1983 to October 1985, 53 patients with malignant biliary obstruction were referred to our institution for a transhepatic biliary stent. One or two endoprostheses were inserted in 46 patients (87 p. 100). Stent insertion was usually performed in two sessions, after two or three days of external drainage. There were 23 men and 23 women. Their mean age was 70.6 years +/- 12 years (m +/- DS). Fourty-two patients (91 p. 100) were referred after failure of an endoscopic or surgical drainage procedure; fourty-four p. 100 of the patients had stage II or III high periportal obstruction. Five cases of severe early complications (11 p. 100) and 8 delayed complications requiring in-hospital treatment (17 p. 100) were observed. Among the latter, 7 were due to plugging of the endoprosthesis. All patients were unfit for surgery either because at a high operative risk or because of the extent of the cancer. Our results showed that percutaneous biliary drainage can be achieved in a high percentage of cases following failure of a surgical or endoscopic drainage procedure. The endoscopic transpapillary approach, which allows the insertion of 12 French endoprostheses in one session should be tried first. Percutaneous biliary drainage should be performed as a complementary procedure when endoscopic drainage has failed or in stage II or III high periportal obstruction associated with persistent jaundice or cholangitis.

[Radiologic aspects of acute invasive pulmonary aspergillosis in 18 immunodepressed patients].

A retrospective study of 18 immunodepressed patients with acute invasive pulmonary aspergillosis enabled the radiologic picture of this increasingly frequent opportunist affection to be described, and different manifestations of the disease as a function of underlying conditions to be determined. Two groups could be distinguished: the first of patients with malignant blood diseases (n = 10) when the "aplasia" factor was dominant; the second of patients without blood diseases (n = 8) when the immunodepression was usually induced by a recent increase in corticoid therapy. Neutropenia was not a finding in any of these patients. The most frequent radiologic finding was single or multiple round excavating foci, these corresponding to zones of bronchopneumonia containing aspergillus filaments with central tissue necrosis forming the "target" lesion. The aplasia was distinguished by the multiplicity of lesions and their excavation following aplasia in the shape of a crescent due to sequestrum formation. Prognosis was related to severity of subjacent lesions. In the 2nd group, lesions could be single and nodular; prognosis was improved by the possibility of reducing immunodepression factors.

[Percutaneous drainage of intra-abdominal abscesses guided by real-time ultrasonography].

From October 1982 to October 1984, a percutaneous drainage under realtime ultrasound guidance was performed in 53 patients with abdominal abscesses. The location of the abscesses was subphrenic (23), retroperitoneal (16), and intrahepatic (14). A safe access route was found by using ultrasound and fluoroscopy in 53 out of 55 patients (96 p. 100). Percutaneous drainage failed in 8 patients and 3 of these patients died. The causes of death were: cerebral abscess (1), renal failure after surgery for correction of a duodenal fistula (1), and pancreatic abscess (1). The other five patients were cured by surgical drainage. Two complications were observed: one case each of pneumothorax and purulent peritonitis. Forty-five patients were healed by percutaneous drainage without operation. The duration of the catheter drainage was 14 days +/- 13 (m +/- 1 SD). Our results suggest that percutaneous drainage under realtime ultrasound guidance is an efficient and safe way to treat abdominal abscesses.
