Automated static perimetry to evaluate diabetic retinopathy.

The Octopus automated static perimeter was used to evaluate patients with early diabetic retinopathy. It showed islands of threshold sensitivity depression that were equal to areas of nonperfusion seen on fluorescein angiography. The geographic area of the fundus at risk of developing these field defects was found to be between 20 and 45 degrees, representing the central area of the midperiphery. This procedure has potential as an excellent screening test for early diabetic retinopathy.

Occult hepatic sinusoid tumor of infancy simulating neuroblastoma.

Two infants with hepatosplenomegaly and an occult tumor of hepatic sinusoids are reported. Although secretion of biogenic amines of neuroblastoma was not elevated, infrequent neurosecretory granules were observed by electron microscopy in the cytoplasmic processes of the tumor cells. The infants responded to vincristine and prednisone therapy and are tumor free 8 and 2 years later, respectively. The clinical, radiographic, biochemical, and microscopic findings of these cases are presented. The distinction from other infantile hepatic sinusoid small round cell tumors is based on the light and electron microscopic findings. This neuroepithelial tumor is either an unusual form of neuroblastoma or a neoplasm of APUD cell origin. If chemotherapy is utilized, it should be selective and limited.

Leptospirosis--an occupational disease of soldiers.

Two cases of leptospirosis are described in soldiers who fell into a river together. One developed Weil's disease due to icterohaemorrhagiae serogroup and the other "Mud-Fever" due to grippotyphosa serogroup. The organisms were initially cultured in standard blood culture media. There were six cases of leptospirosis in BAOR in the 10 years to 1980 giving an incidence 10 times greater than in the civil population of UK and West Germany. Leptospirosis is an occupational disease of soldiers in BAOR and the health risks of fresh water immersion should be publicised.

Response to termination of pregnancy for genetic reasons.

The study was undertaken to demonstrate the reactions to termination of pregnancy for genetic reasons and to examine follow up services that are available. Women resident in Mid Glamorgan who had experienced a termination of pregnancy between 1977 and 1981 either as a result of positive findings during the alpha-fetoprotein screening programme for neural tube defect or chromosome abnormality following amniocentesis because of their high risk, were interviewed using a structured interview. Three internal comparison groups were formed from those women who had also experienced a spontaneous abortion, previous stillbirth or neonatal death and a previous therapeutic abortion. Of 48 women interviewed 77% demonstrated an acute grief reaction, 45% remained symptomatic 6 months after the pregnancy ended compared with no such reaction after spontaneous abortion or therapeutic abortion earlier in pregnancy. The response of termination of a wanted pregnancy was similar to stillbirth reaction, but follow up visits by midwife or general practitioners were limited to only 16% of the post termination group. It is concluded that such women should receive support immediately after the termination and later receive genetic counselling.

Successive factor VIII doses for a haemophiliac undergoing major surgery calculated on a micro-computer.

Doses of factor VIII to be infused into a haemophiliac undergoing major surgery were calculated on a micro-computer. The degree to which the particular patient responded to infused factor VIII and the rate at which it decayed were estimated by two pre-operative experiments. Plasma levels of factor VIII were predicted over fourteen days during which time the patient was infused with 53,000 iu. A continuous plot of predicted values with time was displayed on a video monitor. The predicted values showed good agreement with assay results.

Cholest-5-ene-3 beta, 26-diol: synthesis and biomedical use of a deuterated compound.

To further studies of the metabolism of 26-hydroxycholesterol in fetal and neonatal life, a deuterated compound was prepared from kryptogenin by Clemmensen reduction. The spectra of the deuterated 26-hydroxycholesterol showed that five to nine deuterium atoms were incorporated per 26-hydroxycholesterol molecule, with the maximum incorporation of eight deuterium atoms. The deuterated compound was recovered unchanged from the feces of a child following oral administration. Comparison of the ratio of deuterated to protium compound indicated the presence of an endogenous pool of 26-hydroxycholesterol. Parenteral administration of the compound to a hamster indicated metabolism to deuterated chenodeoxycholic acid. The compound is useful as an isotope tracer for studies of the endogenous metabolism of 26-hydroxycholesterol.

Cryoanalgesia in the management of chronic facial pain.

The results of 85 cryogenic nerve blocks in 43 patients with chronic facial pain over a period of 4 years are reviewed. In 67% of patients with non-herpetic neuralgia the duration of pain relief (median 93 days) exceeded the duration of sensory loss (median 60 days).

Canine pneumocystis pneumonia.

This is a case report of pneumocytosis of an eight month old Dachschund from the Cape Province. Clinically it was an afebrile disease with signs limited primarily to the lower respiratory tract. The report consists of a short history, the histopathologic findings, evidence of the electron microscopic confirmation of the diagnosis and a brief discussion. It is believed to represent the first case of canine pneumocystosis in the Republic of South Africa.

Cognitive tempo in educable mentally retarded children.

The present study investigated the relationship of time and errors on the Matching Familiar Figures test to the mental age of 40 Caucasian male, educable mentally retarded (EMR) children. The children, who ranged in age from six to 18 years, had intelligence test scores of less than 80, were exhibiting problems in adaptive behavior, and were all receiving special education services. An ANOVA revealed that with increasing mental age, elapsed time to first response increased and errors decreased, thus indicating a mental age developmental decrease in impulsivity. For all subjects, time was inversely related to errors. The data suggest that the development of cognitive tempo in EMR children parallels that of nonhandicapped children.
