Uterine and cardiovascular effects of aminophylline.

Aminophylline (250 mg., over five minutes) was infused intravenously to 10 patients with oxytocin-induced contractions. The uterine activity decreased to 87 per cent because of the effect on contraction intensity; contraction frequency was unaffected. The maximum maternal pulse rate increased to 33 beats per minute. Blood pressure was unaffected. The fetal heart rate and beat-to-beat variability increased. An increased dosage of aminophylline produced unacceptably high maternal tachycardia. Comparing the results to those produced by the beta-sympathomimetic drugs in the same experimental model, it is concluded that aminophylline exhibits poor uterine selectivity with an unfavorable cardiovascular/tocolytic ratio.

The effects of hexoprenaline, a beta 2-sympathomimetic drug, on maternal glucose, insulin, glucagon, and free fatty acid levels.

Hexoprenaline, an adrenergic beta 2-receptor agonist, was administered as a 10 microgram intravenous bolus to 9 women in the third trimester of pregnancy. The maternal plasma glucose, serum immunoreactive insulin, plasma immunoreactive glucagon (IRG 30 K), and free fatty acid (FFA) concentrations rose significantly at different times following the bolus injection. Thc serum insulin level increased first and reached a peak at 10 minutes, before the rise in plasma glucose, which reached a maximum at 30 minutes, suggesting that beta 2-receptor stimulation affects insulin secretion directly and not via a rise in the glucose level. Plasma glucagon and FFA levels also rose despite the rise in glucose levels. We therefore conclude that beta 2-receptor stimulation has direct actions on insulin and glucagon release and on glucose and FFA metabolism. The possible fetal sequelae due to these changes in the maternal metabolic milieu are discussed in relation to the use of a 10 microgram intravenous bolus of hexoprenaline as a measure in the treatment of acute fetal distress.

Use of a beta 2-sympathomimetic drug as a temporizing measure in the treatment of acute fetal distress.

Severe fetal distress in labor is an obstetric emergency which may result in the delivery of a severely asphyxiated baby or even death of the fetus. This problem may be compounded by a hurried mechanical vaginal delivery or by delay before a cesarean section can be performed. The rationale for using a beta sympathomimetric drug as a temporizing measure in the treatment of fetal distress to prevent deterioration in and even to improve the condition of the fetus before delivery is discussed. Six cases are described in which hexoprenaline (Ipradol), a beta2-selective sympathomimetic drug, has been administered as a 10 microgram intravenous bolus in the treatment of acute fetal distress in labor.

The uterine and cardiovascular effects of oral fenoterol hydrobromide.

Fenoterol hydrobromide was administered as a 5 mg tablet to 10 patients in oxytocin-induced labour and the effects on uterine activity and on the cardiovascular system of mother and baby were evaluated. The drug took effect within 30 minutes in 8 of the 10 patients and it reduced uterine activity to less than 30 per cent of the original. Fenoterol caused a moderate maternal tachycardia, raised systolic and decreased the diastolic blood pressure.

A comparison of the uterine beta2-adrenoreceptor selectivity of fenoterol, hexoprenaline, ritodrine and salbutamol.

A study was undertaken to compare the uterine beta2-adrenoreceptor selectivity of fenoterol, hexoprenaline, ritodrine and salbutamol. The drugs in doses having an equivalent effect on uterine activity, were randomly administered as an intravenous bolus to 10 patients who had been induced at term. The cardiovascular and uterine effects of the drugs were recorded. The rise in maternal pulse rate was significantly less (P less than 0,001) after the administration of hexoprenaline when it was compared with fenoterol, ritodrine, and salbutamol. Hexoprenaline proved to be the most chronotropically beta2-selective drug, having the least effect on the beta1-adrenoreceptors of the heart, for an equivalent effect on the uterus. Blood pressure changes were less significantly different between the drugs. The clinical use in obstetrics of a highly beta2-selective sympathomimetic drug is discussed.

Uterine and cardiovascular effects of beta2-selective sympathomimetic drugs administered as an intravenous infusion.

The uterine and cardiovascular effects of fenoterol, hexoprenaline, ritodrine and salbutamol, given as intravenous infusions in equivalent and therapeutic dosages in oxytocin-induced labour, were studied and compared. All the drugs were effective in reducing the uterine activity to less than 30% of the original. The effect on the cardiovascular system was to produce a moderate tachycardia and a widening of the pulse pressure by raising the systolic and decreasing the diastolic blood pressures.

The effects of the fenoterol hydrobromide (Partusisten) aerosol on uterine activity and the cardiovascular system.

Fenoterol hydrobromide (Partusisten) aerosol was used in a placebo-controlled, double-blind, double crossover trial to study its effects on oxytocin-induced uterine contractions and the maternal cardiovascular system. The active and placebo aerosols were each administered a total of ten times or twice each in five patients who were in established oxytocin-induced labour with intact membranes. Five puffs, of 200 mug each, of the active aerosol reduced uterine activity by more than half; the effect on the cardiovascular system was to cause a mild tachycardia and to widen the pulse pressure.

Cerclage in the treatment of incompetent cervix.

A retrospective study was undertaken of 71 patients who underwent a postconceptional cervical cerclage operation. The previous pregnancy loss in these patients was 71,4% compared with an 84,5% success rate after the operation had been performed. The possible reasons for the failures are discussed. The incidence of immediate postoperative complications was 14%. Infection, probably as a result of insertion of the suture, occurred in 30% of the patients.
