The embryonal carcinoma of the parotid gland. A rare example of an embryonal tumor.

Embryonal tumors are a neoplastic proliferation of cells of organ rudiments. Morphologically, these tumors are similar to the developmental stages of these organ rudiments. Embryonal tumors of the salivary glands have not been previously described. In the salivary gland register, reviewing the years 1965-1982 (n = 8043), we diagnosed 2,878 tumors of the salivary glands, of these 73 were tumors in children. One case was a malignant epithelial tumor in a 12-year-old boy, which showed the criteria of an embryonal carcinoma in light- and electron microscopy. The tumor revealed solid undifferentiated areas, epidermoid structures with keratinization and acinic structures. Immunhistochemically, the better differentiated epidermoid cells reacted positively with anti-CEA and anti-keratin, the acinic cells were positive with anti-amylase. The ultrastructure was characterized by primitive ductular epithelial cells and acinic cells with their typical morphological features. The embryonal carcinoma has to be distinguished from undifferentiated carcinomas of the salivary glands, which consist of primitive ductular structures only. The failure to detect other tumor markers (lactoferrin, tissue polypeptid antigen) indicates that poorly and well differentiated areas can exist simultaneously in embryonal carcinomas.

[The forensic importance of surgical lesions of sensory nerves in the maxillofacial region].

Operative lesions of sensory nerves involve direct or also indirect effects on nerve trunks, peripheral fibers, or terminal plexuses in the mucoperiosteum. Iatrogenic trauma produced by infiltration anesthesia, tooth extraction, or intraoral operative procedures may lead to various complaints accompanied by simultaneous hyperesthesia, normal esthesia, hypoesthesia, or also anesthesia. Transitory and permanent lesions should be differentiated forensically, prognostically, and thereapeutically. If the nerve conduction structure is intact, re-sensitization may require up to one year. Operative measures are available which help alleviate persisting scar complaints. Since constant pain can usually be traced back to scars, the therapeutic concept consists of using release procedures to create more favorable scar conditions (e.g., neurolysis and underpadding with soft andoor hard tissue). The prognosis of microsurgical procedures for nerve defects is all the more favorable if as little time as possible elapses between injury and reconstruction.

[Follow-up study on coagulation stabilizing filling of bone defects using the new Gelastypt M after cystectomy of large mandibular cysts and after extraction of ectopic teeth].

The good results were confirmed in a follow-up examination of 23 patients two to six months after surgery. No signs of disturbed wound healing and/or inadequate organization of the clot could be observed catamnestically, clinically, and roentgenologically. The late infections often observed after implantation of Gelastypt S (with Surfen) did not develop after use of Gelastypt M 1/2. This preparation therefore may be recommended without restriction.

[Burning tongue (author's transl)].

Burning tongue mostly does not occur as a disease per se, but as a symptom primarily of internal but also of neurological, dermatological, psychiatric and dental diseases. To facilitate differential diagnostic considerations, a short description is given of the individual clinical pictures as well as of the pertinent findings. The knowledge of these diseases is a prerequisite to the causal and lasting treatment of patients affected by the obstinate and occasionally even painful symptom of the burning tongue.

[Odontogenic soft tissue abscesses in the maxillofacial region. An analysis of 1386 cases from 1957--1976 at the Northwest-German Dental Clinic in Hamburg].

Between 1957 and 1976, 1386 patients with odontogenous abscesses in the area of the mouth, jaw, and face were treated on an in-patient basis at the Hospital for Jaw Surgery (Nordwestdeutschen) in Hamburg. The catamneses of these patients were evaluated noting the localization and frequency of each abscess, the age and sex distribution of the patients, and the possible teeth responsible for the abscess. Any pathogens demonstrated were carefully recorded. The degree of sensitivity and/or resistance of the various groups of pathogens were also included in the study, particularly streptococcus and staphylococcus. Uncritical and undifferentiated administration of antibiotic therapy for pyogenic inflammations in the area of the mouth, jaw, and face is not to be recommended.

Principles of the surgical therapy of eyelid tumours.

The case histories of 290 eyelid tumours at the North-West Germany Clinic for Maxillo-Facial Surgery, Hamburg, are evaluated and the experiences gained in the surgical therapy of this type of tumour reported. It is emphasized how necessary it is that the radicality of tumour resection by controlled by frozen section microscopy during the operation. This ensures a low recurrence rate. The results which were evaluated at the earliest six months after eyelid reconstruction, show that, despite different initial situations, a high percentage of both good functional and aesthetic results may be obtained after extensive tumour resection. This requires that the reconstructive method most suitable for the individual case be chosen out of the wealth of techniques. The most important ones used at our hospital are discussed on the basis of various examples.

[Indications and technic of transdental fixation].

Transdental fixation, as closed implant, offers the most favourable preconditions of all dental procedures of implantation. Teeth where the root-crown relation is altered to the detriment of root length, which is held firm in bone, can with its help be made functional. Loosening and premature loss of teeth can be prevented. Further discussed is the indication for this procedure, especially in tooth fractures and luxations as well as the operative steps. Up to now transdental fixation has proved itself although the number of cases and the follow-up time are insufficient to allow final judgment.

[Animal experimental studies on bone regeneration following drilling of the bone].

Experiments in dogs showed that drilling at the surface of bone provokes damage which increases with the number of revolutions per minute. Polychrome sequence marking and histological tests demonstrated that filling up of bony defects started later with turbine-driven drills than with drills of 2,000 r.p.m. and has not even been completed after eight weeks. It is therefore recommended to abstain from using turbines in drilling at the surface of bone.
