Delayed nerve regeneration in streptozotocin diabetic rats.

Nerve regeneration across a 10-mm gap was delayed in streptozotocin diabetic rats 3 and 4 weeks after transecting the sciatic nerve. Opposite ends of each cut nerve were introduced into a silicone tube, leaving a 10-mm gap. Electron microscopy was used to evaluate the progress of regeneration in sections at 2-mm intervals across the 10-mm gap. After 3 weeks, control axons had bridged the 10-mm gap, and myelin sheaths extended for 6-8 mm. By contrast, axons and their myelin sheaths were seen no further than 2 mm from the proximal stump in diabetic animals. By 4 weeks, axons had bridged the gap in diabetics; however, they appeared immature and showed dystrophic changes. The findings suggest that although regeneration does occur in diabetic nerves, it is significantly delayed and qualitatively impaired.

Anatomical basis of rectus abdominis myo-cutaneous flaps.

The recent development of myo-cutaneous flaps and their use in plastic and reconstructive surgery prompted the authors to have a particular interest in musculo-cutaneous flaps of the rectus abdominis. From their personal studies and from a review of the literature, they consider the anatomical bases for these flaps and the different methods proposed for their construction. Finally they indicate their vast area of application which is essentially related to the wide range over which they can act.

[Primary neuroblastoma of the mandible. Apropos of a case].

Concerning an observation of mandibular neuroblastoma, the problem of a probable primary localization of this neurocristopathy is raised on the face level. Actually, classically speaking, it can only be a question of metastasis revealing a tumor initially located in the abdomen, the thorax, or more rarely in the pelvis or the neck. However, the evolution of the lesion under treatment leading to complete cure within three years denies the secondary nature of the process. Related to the three other cases in the world that have so far been published, our observation enables to assert the reality of the primary development of a neuroblastoma on the mandibular level. The coherent reasoning is founded on the role of neural crests in facial embryogenesis whose vitiation offers a logical explanation to this exceptional phenomenon.

[Malignant transformation of fibrous dysplasia of the orbit].

After a review of the literature, the authors report a case of sarcomatous transformation of orbital fibrous dysplasia and they underline the exceptional unfrequency of it. They discuss above all the diagnostic difficulties and the characteristics of the malignant transformation.

[Necrotizing sialometaplasia. A rare diagnostic pitfall].

Concerning an observation of necrotizing sialometaplasia the authors make a literary survey, paying particular attention to the clinical and anatomopathological descriptions of the lesion. They insist on its deceptive aspect which led in some cases to important quite useless exeresis, since the lesion heals spontaneously. At last the different etiopathogenical hypothesis are reported.

[Distinctive nature of mountain sport maxillo-facial injuries (author's transl)].

Mountain sports were the cause of 400 facial injuries treated in he Maxillo-facial Surgical Unit in Grenoble over a period of 5 years. Deltaplane injuries are becoming more frequent. Isolated facial injuries represent 57 p. cent of the cases treated. The various types of lesions encountered are described, and the distinction drawn between those treated as emergencies, and those for which urgent treatment could be delayed.

[Lateroterminal arteriovenous fistula with "safety valve" for haemodialysis (author's transl)].

We describe a new type of arteriovenous fistula for haemodialysis, aimed at controlling arterial spasms which contribute to thrombosis. A laterolateral fistula is created, and the artery is sectioned below the anastomosis. A Fogarty catheter inserted through this "safety valve" relieves the spasm without interfering with the arteriovenous shunt. Mere ligature of the distal segment of the artery transforms the fistula from terminolateral into terminoterminal.
