[Eosinophilic colitis caused by allergy to cow's milk protein].

Eosinophilic colitis is one of the clinical manifestations of allergy to cow's milk during the first year of life. We report a series of 9 infants who, under 9 months of age and while clinically well, presented rectal bleeding of variable magnitude, with or without diarrhea, shortly after a cow's milk-based formula was initiated (n = 6); yet, 3 cases received only breast feeding. Bleeding disappeared in all patients after milk withdrawal from the diet. Challenges were planned after 12 months of treatment; three patients have not yet reached this moment, 3 had a negative challenge at 12, 18 and 28 months of age and are on a complete diet, and 3 are still on cow's milk free diet because ingestion of milk at 12, 18 and 25 months still induced rectal bleeding. This series of patients gathered in 3 years, follows the trend reported in many countries that there is a relative increase of patients diagnosed with allergy conditions early in life.

Steroidogenic capacity and oxidative stress-related parameters in human luteal cell regression.

The steroidogenic capacity and oxidative stress-related parameters of the human corpus luteum (CL) at different stages of the luteal phase were studied under basal and human chorionic gonadotropin (hCG)-stimulated conditions. Mid CL exhibited the maximal steroidogenic capacity, together with lower levels of glutathione and higher thiobarbituric acid reactants content, macrophage count, and superoxide dismutase (SOD) activity, compared to the late CL. Addition of hCG to luteal cell cultures led to a preferential increase in progesterone synthesis in the late CL compared to the mid CL, without changes in the oxidative stress-related parameters, except for the increased SOD activity found in the late CL. It is concluded that an oxidative stress condition is established in the mid CL, coinciding with the maximal steroidogenic capacity and macrophage infiltration of the organ, which may be of relevance as one of the major mechanisms initiating CL involution in the human.

Morpho-functional study of human luteal cell subpopulations.

It has been reported that the mammalian corpus luteum is composed mainly of two subpopulations of luteal cells (large and small) of different morphology and function. The aims of this study were first to characterize cytologically the human corpus luteum throughout the luteal phase, and second to establish the in-vitro steroidogenic capacity of a well-defined human mid-luteal cell system. The results show that the most predominant (> 70%) cell shape, is polyhedric, and the number of cells per unit area is significantly different in the early, mid- and late corpus luteum (P < 0.005). Moreover, small cells (< 22 microns) were most common (56.8%) in all tissues analysed. On the other hand, both subpopulations synthesized progesterone, oestradiol and testosterone, although a significantly greater production of basal steroids was observed in large luteal cells (P < 0.05). Nevertheless, the response of small cells to human chorionic gonadotrophin (HCG) was significantly greater (P < 0.05) than that of large cells, in agreement with the preferential specific binding obtained for [125I]HCG to the small cell subpopulation. In summary, these results indicate that the human corpus luteum possesses distinct cell types, which may be related to endocrine function and its control.

[Nonimmunological hydrops fetalis. Experience with 33 cases].

A retrospective study of all perinatal autopsies done during 11 years at hospital SBA showed 33 cases of nonimmunologic fetal hydrops (1.83% of all perinatal autopsies). The incidence was 1/3624 liveborn babies. Intrauterine fetal infections and chromosome genetic abnormalities accounted for 27.3% each; and in 15.2% of the cases some pathology of pregnancy was identified (including twining). All the cases showed severe anemia and in 58% of them pulmonary hypoplasia was identified. Moreover 31% of the cases showed renal hypoplasia. A placenta megaly was identified in 100% of the cases studied. The three most important mechanisms associate with the genesis of nonimmune fetal hydrops are: chronic intrauterine anemia, hypoproteinemia and intrauterine fetal heart failure.

[A molar pregnancy coexistent with a viable fetus in a twin pregnancy].

A case of molar gestation associated with an only fetus, both product of an twin gestation is presenting. We analyzed the clinical picture, the diagnostic method, the after birth resolution and evolution of this gestation. The result of a live newborn of 1,300 g and verify coexisting of complete hydatidiform mole.

[Anomalies of the umbilical cord and its association with fetal malformations].

An evaluation of 1500 perinatal autopsies is presented and the findings of umbilical cord anomalies are related to fetal malformations. The most frequent pathologies of the umbilical cord were one umbilical artery and stenosis of the cord. The author highlights the importance of a thorough examination of the umbilical cord and of the placenta.

Rhabdomyomatous dysplasia of the lung.

This article deals with the presence of nontumoral striated muscle fibers in the lungs of 3 neonates. These cells were diffusely distributed in one lung (case 1) or in both (case 2), or focally localized to the lung parenchyma adjacent to the liver in a case with a large right diaphragmatic hernia (case 3). The striated muscle fibers were located in the walls of small bronchi and bronchioli or in the alveolar interstitium. Other major lung malformations found simultaneously were absence of lobation, hypoplastic lungs, and hypoplastic pulmonary vessels. The origin of striated muscle fibers in the neonatal lung has been attributed to anomalous differentiation of mesoblastic cells (as in cases 1 and 2). The presence of striated muscle cells in the lower margin of a hypoplastic lung associated with a right diaphragmatic hernia (case 3) suggests that intrapulmonary inclusion of diaphragmatic muscle fibers might be a source as well. Striated muscle fibers in the lung are commonly associated with major malformations involving heart and lungs, suggesting a much wider morphogenetic error.

Rhabdomyosarcoma arising in a congenital giant nevus associated with neurocutaneous melanosis in a neonate.

The case of a newborn with a pedunculated embryonal rhabdomyosarcoma arising from a pigmented giant congenital nevus located on the posterior part of the neck and upper trunk is presented. The nevus was associated with melanosis of the leptomeninges and central nervous system. The pedunculated tumor was removed and the infant was given subsequent chemotherapy. In spite of well-tolerated chemotherapy, he died on the 30th day of life due to a staphylococcal pneumonia.

Congenital leukemia with placental involvement. Report of a case with ultrastructural study.

The study of a stillborn infant and the placenta, showing extensive infiltration of fetal organs and chorionic villi by leukemic cells, is reported. The nature of the malignant cells was not apparent by light microscopic examination. Ultrastructural examination showed that these were immature myeloblasts, stressing the usefulness of electron microscopic study in establishing the diagnosis. The current case study seems to be the first in the literature in which both placenta and fetus were studied in extenso.

Estradiol and progesterone nuclear and cytosol receptors of hydrosalpinx.

Hydrosalpinx is usually associated with a low intrauterine pregnancy rate following restoration of tubal patency by microsurgery. The biochemical evaluation of pathologic tubes from 11 infertile patients showed the presence of estradiol (E2) and progesterone (P) receptors at the cytosol and nuclear levels. The binding constants (Kd) for these specific proteins for E2 and Promegestone (R5020) were of the same magnitude as observed in normal tissue. Mean levels of E2 cytosolic and nuclear receptors and cytosolic P receptors of hydrosalpinx were significantly lower than those of a normal fallopian tube (P less than 0.05). No correlation between the severity of the histologic appearance of the tissue and the subcellular distribution of receptors was observed. We conclude that the decrease in the steroid receptor population of these damaged tubes could be counted as another factor to be considered in the poor intrauterine rate of the salpingoneostomy.

Fat-containing uterine smooth muscle cells in "toxemia": possible relevance to atherosclerosis?

Uterine smooth muscle cells in "toxemia of pregnancy" contain varying amounts of fat--a feature to date believed to characterize only the arterial smooth muscle cells in atherosclerotic lesions. Thus, the smooth muscle cells at these two sites do not differ essentially in their reactivity to certain forms of injury: hypoxia may represent an injurious factor common to both "toxemia" and atherosclerosis. These observations imply that the view that the arterial smooth muscle cells are biologically different than are those elsewhere may no longer be tenable.
