Handling properties and thrombogenicity of microsutures.

The thrombogenicity and handling properties of 1 polypropylene and 2 polyamide 10-0 microsuture materials were evaluated in a paired comparative study in rats. Using a sequential analysis method with significance limits corresponding to 2 alpha = 0.05 and beta = 0.05, it was found that the handling properties of Supramide (polyamide) sutures were better than those of Prolene (polypropylene), and that the handling properties of Prolene did not differ from those of Nylon (polyamide). Also using a sequential analysis method with significant limits corresponding to 2 alpha = 0.05 and beta = 0.02, it was found that thrombogenicity of Prolene was significantly less than the thrombogenicity of Supramide and Nylon. It is concluded that Prolene is the microsuture of choice among the three materials tested.

The blood-retinal barrier permeability in diabetic patients.

By the of aid an extended corpus vitreum fluorophotometric technique, the blood-retinal barrier permeability for fluorescein was studied in diabetologically well characterized patients with insulin dependent diabetes mellitus. The method, which involves simultaneous determination of the fluorescein concentration in corpus vitreum and plasma, is described and discussed. A clear correlation was found between the degree of retinopathy and permeability (P). Patients with normal visus, ophthalmoscophy, fundus photo and fluorescence angiography exhibited P-values of 1.10(-7) cm . sec-1. This was similar to P-values found in normal volunteers. Simplex retinopathy without macular oedema showed values of 2.5 . 10(-7) cm . sec-1 while simplex retinopathy with macular oedema showed P-values of 10 . 10(-7) cm . sec-1.

Uterine cellular stromal infiltration. A diagnostic problem.

Extreme pleomorphic stromal cellular infiltration of the cervix and the corpus uteri was demonstrated by curettage in a 29-year-old woman with menorrhagia. The histological classification was not decisive and a hysterectomy was performed. The histological picture may be compatible with a malignant disease such as a stromal sarcoma or a lymphoma of Hodgkin or non-Hodgkin type, or with an inflammatory or immunologic disease. The clinical course, however, indicates a benign disease.

Inactivation of Epodyl in aluminium foil cups as a cause of ineffective treatment of non-invasive bladder tumours.

Over a 5-year period 38 patients were treated at Rigshospitalet with intravesical instillation of Epodyl for multiple, non-invasive bladder tumours. The results were significantly inferior (p less than 0.00025) to those reported from other hospitals, though the composition of the case series was comparable. Analysis of procedure at Rigshospitalet revealed that when Epodyl was mixed with water in aluminium foil cups, which was standard practice at the outpatient department, the concentration was halved in 5 minutes. Ineffective treatment thus had been unwittingly given. The results, however, do not warrant rejection of Epodyl treatment; they may instead constitute direct evidence in it favour.

The incidence and characteristics of acute myeloid leukaemia arising in Hodgkin's disease.

In 201 consecutive patients with Hodgkin's disease treated from 1964 through 1975 with intensive irradiation and/or combination chemotherapy 3 cases of acute myeloid leukaemia were observed. The observed number of cases was 75 times over the expected (P less than 0.01). An analysis of 47 reported cases of acute myeloid leukaemia arising in Hodgkin's disease shows that these cases differ considerably from 'spontaneous' cases of acute myeloid leukaemia by appearing in a much younger age group, by a very poor response to anti-leukaemic chemotherapy, and by a relatively low male/female ratio (0.84). The intensification of radiotherapy and cytostatic therapy of Hodgkin's disease during the last decade is considered the explanation of the increased incidence of acute myeloid leukaemia in these patients.
