[Acute haemorrhagic pancreatitis. Criteria of letality (author's transl)].

A retrospective study of severity symptoms in a series of 102 patients operated upon for acute haemorrhagic pancreatitis showed that the risk of death was much significantly higher when shock (p less than 0,00001) and renal failure (p less than 0,0001) were present. The association, during the post-operative period, of shock and renal failure with one of the following symptoms: digestive haemorrhage, psychic disorders, pulmonary oedema, post-operative peritonitis and evisceration invariably proved fatal. It is suggested that controlled therapeutic trials should be carried out in patients presenting with these complications.

[Extracranial carotid stenosis and its sonographic diagnosis].

The direct Doppler examination of the carotid arteries as a method to detect and distinguish stenoses and occlusions of the common, internal, and external carotid arteries allows to decide on operability and indication for surgical treatment. The exact observation of the acoustical flow signals permits to a high degree an identification of even small stenoses and arteriosclerotic plaques. In 125 angiographical controls 95% of the sonographic diagnoses, in stenoses larger than 50% correct diagnosis could be established even up to about 99%. There were no false positive sonographic findings. Because of our good results we did not consider it necessary in recent time to perform an angiography before surgery in 25 patients. These patients had a high angiographic risk and there was a clear cut Doppler finding of an extreme stenosis in the internal carotid artery.

[Clinical experience obtained from attention to and delivery of pregnant women with diabetes mellitus with insulin treatment required (author's transl)].

Perinatal mortality recorded from 87 patients with diabetes mellitus requiring administration of insulin was 6.66 per cent between 1975 and 1977. The authors' results are compared with those obtained at different obstetric centres and discussed. Optimum regulation of metabolism and full utilisation of all diagnostic facilities available during pregnancy were found to be essential to individual delivery in the closest possible vicinity of the calculated term.

[Experiences with replantation of large limbs].

From November 1975 to August 1978 26 limb replantations were performed. All 5 lower extremities had to be reamputated. Of the 21 replanted arms 15 healed without major complications, 9 have regained sufficient function up to now; 3 others show signs of reinnervation.

[Experience with transvenous xeroarteriography in vascular surgery (author's transl)].

The edge contrast pattern in Xeroradiography allows blood vessels to be delineated with highly reduced concentration of radiopaque fluid. The contrast material is injected rapidly into a cubital vein. The brachial arteries are opacified 3--8 sec. and the femoral arteries about 7--15 sec. after the injection is finished. The arteriograms are of good quality which equals direct conventional angiography, if the vessels are not overlayed by bone structures. Thus the popliteal artery can be judged best in lateral view. The indications for transvenous Xeroarteriography are preoperative diagnosis especially in high risk patients, angiographic diagnosis of soft tissue tumors and postoperative control and documentation of vascular reconstruction in the extremities. Transvenous Xeroarteriography is a really noninvasive, painless and easy to handle method whiich allows visualization of peripheral vascular lesions and can be reproduced at short intervals without any risk.

[Can ectopic pregnancy be detected by ultra-sound tomography (author's transl)].

During the past eight years ultra-sound scans were carried out on 281 patients with the suspicion of ectopic pregnancy. Diagnostic criteria were established which were designed to minimize false negative results. In 52 cases the clinical suspicion was confirmed by operation. In 46 of these patients ultra-sound signs of ectopic pregnancy were present. Six ectopic pregnancies were not recognized by ultra-sonographies. 73 patients were suspicious of ectopic pregnancy but did not show ectopic pregnancy. The high rate of ultra-sound misinterpretation in ectopic pregnancy is due to the variability of ectopic pregnancy, abortions of intra-uterine pregnancies, abnormalities of the uterus, and adnexal tumours of other etiology. Negative ultra-sound findings do not rule out an ectopic pregnancy since a number of cases with the clinical suspicion of ectopic pregnancy show intact intra-uterine pregnancies. Ultrasonography is a valuable additional diagnostic method in suspected cases of ectopic pregnancy.

[Interpretation of the trace element content of hairs in criminology, toxicology and environmental protection. Experimental migration of metal ions in keratine (author's transl)].

Metal ions of exogenous origin can penetrate into the hair and can be accumulated. Penetration and distribution of metal ions could be examined by means of electron microprobe-analysis. The dependence of the accumulation of different kations on the concentration of these in the surrounding solutions have been determined by atomic absorption spectroscopy. The dimension of accumulation depends on various factors: the chemical nature of the ion involved, its concentration in the solution and the solutions pH-value, the nature of the anions present as well as the nature and concentration of other kations. The distribution of the kations in the hair can be changed by washing with distilled water. The significance of the present physical-chemical experiments with hairs for criminology, toxicology and to environmental protection were discussed in detail. It was shown that the content of zinc in the hair is of predominantly endogenous origin; moreover, a strong accumulation in the hair root took place. The study of the accumulation mechanism should contribute to clarifying other possible accumulation mechanisms. Further information should be expected from extention of these experiments.

[Surgical treatment of cerebral ischemia. Indications for surgery in carotid artery stenosis (author's transl)].

Reconstruction of carotid arteries should preferably be a prophylactic measure and only in exceptional cases a therapeutic procedure. The best results are seen in stage I (asymptomatic) or in stage II (transient ischemic attacks). Surgical intervention in acute cerebral vascular insufficiency is indicated only with a conscious patient with a negative brain scan and without progressing symptoms (stage III a). In stage IIIb (progressive stroke) and stage IVa (acute completed stroke) operative treatment is contraindicated because of a high mortality rate. In accidents with persisting neurologic symptoms (completed stroke more than 2 weeks old) necessary reconstruction of the contralateral vessel is indicated.
