Percutaneous transluminal laser angioplasty with contact probes.

Percutaneous transluminal laser angioplasty (PTLA) with a metal contact probe and a sapphire contact probe was performed under experimental conditions and in 88 patients with femoropopliteal artery occlusions. The experiment revealed that more ablation was caused by the sapphire probe than by the metal probe. Because of heat accumulation in subsurface tissue layers the metal probe caused a three to four times larger zone of thermal necrosis surrounding the tissue defect. Clinically both contact probes revealed similar results. The initial rate of recanalization of femoropopliteal artery occlusions with a mean length of 8 cm was 78% (metal probe) to 82% (sapphire probe), and the perforation rate was 14% (metal probe) to 8% (sapphire probe). Doppler ultrasound examinations after 6 months revealed a patency rate of 84%. PTLA has proved to be a successful and safe procedure for recanalization of arterial obstructions that are not appropriate for fibrinolysis.

[Functional diagnosis of biliary endoprostheses using hepatobiliary sequence scintigraphy].

In 11 patients with biliary endoprosthesis, hepatobiliary scintigraphy with dimethyliminodiacetic acid (HIDA) was performed 16 times because of an increase in the serum bilirubin level. For the demonstration of dysfunction of the endoprosthesis, this method proved to have the same sensitivity as transhepatic cholangiography, which was performed as control examination. Therefore, hepatobiliary scintigraphy, a noninvasive and well-tolerated examination, can be recommended as a primary diagnostic tool in addition to ultrasonography for assessment of the drainage function of the biliary endoprosthesis.

[Acroangiodermatitis (pseudo-Kaposi disease)].

We present clinical, angiological, and histopathological findings concerning different kinds of so-called "kaposiform" acroangiodermatitis. In particular, we refer to the etiopathogenesis, prognosis, differential diagnosis, and therapy of each entity. The similarities between the so-called "kaposiform" acroangiodermatitis and Kaposi's sarcoma are primarily derived from clinical criteria. In contrast to Kaposi's sarcoma, the histopathological picture of acroangiodermatitis usually shows stasis dermatitis.

[Percutaneous transluminal balloon valvuloplasty in the treatment of congenital valvular pulmonary stenoses in childhood].

Five children aged 2 to 6 years with isolated pulmonary valve stenosis underwent treatment by percutaneous transluminal balloon valvuloplasty. A peak systolic gradient between right ventricle and pulmonary artery (delta RV/PA) was reduced from 60.4 +/- 18.7 mm Hg to 21.7 +/- 12.6 mm Hg and the right ventricular pressure fell from 76.2 +/- 20.7 mm Hg to 43.0 +/- 16.4 mm Hg. There were no complications. Percutaneous transluminal balloon valvuloplasty is a safe method to treat isolated pulmonary valve stenosis. It benefits many patients over a long term period and may provide definitive treatment in some patients.

Angioplasty, Balloon↗

[Percutaneous balloon dilatation of pulmonary valve stenosis].

Balloon valvuloplasty was carried out on five children with isolated valvular pulmonary stenosis. One child had a residual stenosis following a Brock's transventricular valvotomy. The systolic gradient between the right ventricle and the pulmonary artery fell from 60.4 +/- 18.7 mmHg to 21.8 +/- 12.0 mmHg. Right ventricular pressure fell from 76.2 +/- 20.7 mmHg to 43.0 +/- 16.4 mmHg. Percutaneous balloon valvuloplasty has proved to be an effective method for the treatment of isolated valvular pulmonary stenosis.

[Computerized tomography and myelography findings following lumbar intervertebral disk surgery].

56 patients suspected of having suffered recurrent disc prolapse in the lumbar region were examined by CT and by myelography; the value of these examinations has been analysed. In 32 patients the findings were checked during surgery. There were difficulties in diagnosis, both for CT and myelography, where the findings were complex, i.e. in patients in whom there was postoperative fibrosis as well as disc prolapse and/or osseous spinal canal stenosis. As regards recurrence of prolapse, sensitivity for both methods was 83%. Postoperative scar formation can be clearly demonstrated by CT, but myelography produced a false positive diagnosis of recurrent prolapse in 17%. The changes were, in fact, caused by scar tissue formation.

[Laser angioplasty of peripheral arterial occlusions. Experimental and clinical results].

The first in-vivo trials were carried out on arteries of the extremities which have a straight course and short segmental occlusions. Initial experience indicates that obstructions up to 20 cm in length can be overcome by burning through plaques and thrombotic material by means of a neodymium-YAG-laser and then recanalised. By using a 2.2 mm spherical sapphire, one obtains a channel of 2.5 mm diameter through the obstruction. Subsequently dilatation with a balloon catheter is necessary. Laser angioplasty does not complicate the procedure significantly as far as the patient is concerned, since both the laser catheter and the balloon can be introduced through the same No. 7 F introducer into the femoral artery. Peripheral emboli could not be demonstrated angiographically, nor were they clinically manifest. On the basis of present experience, it seems that recanalisation of occlusions in arteries in the extremities, using a neodymium-YAG-laser focused by a synthetic sapphire, is a feasible alternative to conventional mechanical recanalisation.

[A study of the patency of postoperatively irradiated vascular anastomoses in rats and their histologic results].

The influence of radiation on microvascular anastomoses were investigated on the femoral artery and vein of the rat. In an increasing number of operations free transplantations after tumor resections of the head and neck are performed. In advanced tumors postoperative radiation would be desired as early as possible. The histological changes and the patency of the vessels were investigated. The results indicate that an immediate postoperative radiation is possible even after an free microvascular transplantation.

[Transfemoral catheter-laser thromboendarterectomy of the carotid artery].

Argon laser irradiation via a transfemorally introduced catheter successfully vapourized both fresh thrombi and stenosing plaques in the carotid artery of seven cadavers and two patients. Fresh thrombi were recanalized throughout their length, while the arteriosclerotic plaques were partially reduced. No particles larger than 7 micron were demonstrated in the eluant flushing fluid, indicating that peripheral emboli need not be feared with this technique.

Irradiation of free vascularized tissue transplants in rats.

The aim of this experimental investigation was to show whether immediate irradiation of a free flap or an island flap is dangerous or not. The free epigastric flap of the rat was used as a model for this investigation. The patency of the epigastric vessels of the rat was not influenced by the irradiation and the histological findings showed no early reaction to the irradiation of the vessels of the transplanted tissue. The vessels of this experiment indicate that a free tissue transplantation is possible even if a combined radio-surgical treatment is planned.

In vitro long-term perfusion of different materials for biliary endoprostheses.

An in vitro perfusion of Teflon, polyethylene, polyurethane, and Percuflex TM catheters with human bile was performed to estimate the incrustation rate of the different materials. Microscopic and chemical analyses revealed the highest incrustation rate on Teflon and the lowest on polyurethane (about four times lower than on Teflon). For each material the probable minimum value for the duration of patency of an endoprostheses of a 3-mm inner diameter was calculated.
