[Occurrence of IgE-dependent food allergy in asthmatic disease].

The incidence of food allergy has been evaluated in 67 asthmatic patients (allergic asthma: 52, intrinsic asthma: 15). History was positive for post-meals attacks (7), symptoms involving other target organs (61%). Prick tests were found positive in 17/60; intradermal 77/682. Out of 110 human basophil degranulation tests or RAST, performed in 44 patients, 21/15 patients were found positive. Out of 18 inhalation challenges, 7 were positive. Out of 17 food oral challenges, 6 were positive (8 patients). A definite diagnosis of food allergy was accepted for 9 patients. Elimination diet, with 600 mg/day of cromolyn, induced a major improvement in 7 patients within 3 months, with 2 further failures after 1 year. A successful elimination diet was correlated with a positive food challenge. The relative incidence of food allergy in asthmatic adults is discussed (5.8%), as well as a non-immunologic mediated intolerance to food additives in intrinsic asthmatics, the incidence of vegetable allergies in pollen-sensitive patients, the procedures and meaning of inhalation and oral challenges, the relative importance of trophallergens. Food allergy appears to be a variable co-factor in bronchial hyperreactivity.

[Cancer of the right kidney and tumor of the right liver].

The true benign nature of a right side of liver tumor was revealed only after postoperative histopathology in a patient with cancer of right kidney. This raises the problem of kidney tumors with liver metastases. The preoperative diagnostic value of liver biopsy under ultrasound guidance is discussed, together with reasonable limits of hepatic excision combined with nephrectomy as a function of single or multiple metastases, their location, and their degree of surgical accessibility.

[Is iatrogenic vesico-renal reflux a negligible factor in the evolution of recurrent superficial tumors of the bladder?].

The electrocoagulation and resection of superficial tumors of the bladder leads to iatrogenic refluxes. The frequency of subsequent vesical recurrences may thus favor tumoral grafts in the upper urinary tract. Fives cases illustrate this risk. They were treated by ureteral resections and by one bilateral, and two unilateral nephroureterectomies. Should these refluxes be ignored or treated surgically in the event of repeated papillomatous recurrences?

[Cecocystoplasty. Apropos of 14 cases].

Cystoplasty is the ideal method of bringing back normal urinary flow after cystectomy, and the authors have used this procedure in fourteen cases. The cecum is mobIlized with its vascular pedicle. Bowel continuity is readily and safely restored with the aid of suturing forceps (TA 55, GIA). The cecum is carefully cleaned of all fecal matter. Two UCN join the ureters and the cecum, to which the trigone, the cervix vesicae, or the prostatic or membranous urethra is anastomosed, depending on the case. Three benign cystopathies, four recurrent diffuse papillomatoses and seven vesical carcinomas were treated in this way, with excellent results.

[Right to left (or left to right) cystoprostatectomy].

Surgical techniques using right to left (or left to right) prostatocystectomy are described, although a first-stage posterior approach is used. This procedure, applied for cases treated over the last 2 years, can be completed in approximately one hour and under only slightly hemorrhagic conditions.

Square-wave endurance exercise test (SWEET) for training and assessment in trained and untrained subjects. II. Blood gases and acid-base balance.

In order to obtain information about physiological and homeostasic responses at the maximal Intensity of Endurance of the 45 min "Square-Wave Endurance Exercise Test" (MIE45), three arterial blood samples were taken: (a) at rest; (b) at the 45th min of the SWEET; (c) after 15 min of recovery, to measure paO2, paCO2, [H+], [Hb], and [lactate] in 14 normal male subjects: four trained (T) six well trained (WT) and four others untrained (U). Total mechanical work (TMW) corresponding to MIE45 was significantly higher (mean +/- SEM) respectively in WT (9.22 +/- 0.65 kJ . kg-1, p less than 0.001), than in T (7.17 +/- 0.18 kJ . kg-1, p less than 0.01) and U subjects (4.44 +/- 0.36, p less than 0.001). Because of this the lactate level, which rose significantly during exercise, differed between U and WT subjects (p less than 0.05). In spite of the exhaustive character of the MIE45, [H+] and paO2 remained within the range of normal values. These results suggest that trained and untrained subjects can be trained with the exhausting MIE45 exercise while maintaining a constant [H+] and paO2 at the 45th min of exercise.

Aseptic loosening of cemented total arthroplasties of the hip in relation to positioning of the prosthesis. New utilization of the Tschuprow-Cramer statistical test.

A statistical study of 1200 McKee-Farrar cemented total prostheses of the hip demonstrated the importance of a horizontal position of the cup and a valgus position of the prosthetic stem, in avoiding aseptic loosening of the prosthetic parts. A new test (the Cramer test) showed that a statistical bond exists between the position of the prosthesis and the occurrence of aseptic loosening.

[Results of endoscopic urethrotomy (author's transl)].

The authors report the results of 63 internal urethrotomies for urethral stenosis performed during a four year period. The essential sites were perineal and the most frequent aetiology iatrogenic, particularly after prostatectomy. Overall, the authors obtained 18 satisfactory results, 2 severe complications, 24 failures and 17 patients are too recent to assess or were lost from sight. A second endoscopic urethrotomy gave only very little chance of a definitive recovery. Although the results were not constantly excellent, the series studied by the authors shows that endoscopic internal urethrotomy offers the possibility by a simple technique of obtaining a certain number of durable successes whilst not interfering with the subsequent performance of other types of urethroplasty. Finally, in a certain number of cases endoscopic internal urethrotomy facilitates subsequent dilatations.

A nasal functional test: the opeining of mouth during physical effort.

Apart from rare exceptions (cf Voydeville's Thesis, Nancy 1951), nasal airflow resistance has so far been estimated irrespective of variations in air intake needs. The object of the test presented here is to offer a quick, simple and objective method for determining the level of muscular effort at which a given subject spontaneously switches from nasal to buccal respiration. A thin flexible tube (1,5 mm across) is stuck at one end on to the subject's lower lip and the other into a carbon dioxyde analyser. The subject is then made to pedal at a speed at least 30 rev./min. on an ergometric bicycle, while a braking force increasing by 20 W ever two minutes is applied. The emission of carbon dioxyde starts being detected, and graphically recorded, only when the subject starts breathing through his mouth. Results obtained so far show that this threshold can vary as widely as from 80 to 180 W among healthy individuals. Further aplications of this test to both healthy subjects and patients with impaired nasal function would contribute towards throwing light on the still obsure notion of "nasal comfort".
