Tropical splenomegaly syndrome in a Rwandan kindred in Uganda.

Twenty members of an immigrant Rwandan kindred were all found to have tropical splenomegaly syndrome compared with 29 out of 50 matched neighbour controls. Splenomegaly among the controls tended to cluster within families. IgM levels were significantly raised in the kindred compared with the control subjects without splenomegaly but were similar to those of neighbours with splenic enlargement. We advance a hypothesis that tropical splenomegaly syndrome results from the production of circulating immune complexes in individuals genetically predisposed to produce an IgM antibody response.

Familial cases of endomyocardial fibrosis in Uganda.

This report describes nine cases of endomyocardial fibrosis occurring in four families. All patients came from Rwanda or South-western Uganda, and five had tropical splenomegaly syndrome as well. It seems likely that genetic as well as environmental factors are operative in the aetiology of endomyocardial fibrosis.

Intrathecal chemotherapy in Burkitt's lymphoma.

Meningeal involvement with Burkitt lymphoma cells constitutes the most challenging therapeutic problem in the management of Burkitt's tumour. The results of intrathecal chemotherapy with methotrexate or cytosine arabinoside in 55 episodes of malignant pleocytosis in 38 patients with Burkitt's tumour are described. The response was complete in nearly all patients after the administration of either agent. Cerebrospinal fluid (C.S.F.) remissions were more prolonged in patients receiving intrathecal methotrexate or cytosine arabinoside daily for four days as opposed to a 10-day schedule. A controlled randomized trial of "prophylactic" intrathecal chemotherapy in patients without malignant cells in the C.S.F. on admission showed no protective effect against the subsequent development of malignant pleocytosis. Future therapeutic approaches are considered in the light of these results.

Burkitt's lymphoma: a time-space cluster of cases in Bwamba County of Uganda.

Seven cases of Burkitt's lymphoma have to date (March 1971) been diagnosed in the county of Bwamba in Uganda. All these patients had the clinical onset of their tumour in the 27 months from October 1966 to December 1968; five of them in the last six months of this period. Two of the patients were full sibs living in the same house. This is the most discrete time-space cluster of the disease yet recorded and provides further evidence for a role in the aetiology of Burkitt's lymphoma of an infective agent with a relatively short latent period.

Clinical trial of malaria prophylaxis in tropical splenomegaly syndrome.

A controlled, randomized, double-blind trial of malaria prophylaxis in tropical splenomegaly syndrome showed a significant reduction in spleen size and an improvement of anaemia and symptoms in patients treated with antimalarials compared with control subjects receiving placebo. This study confirms the observations from West Africa and provides further indirect evidence for a malarial aetiology of tropical splenomegaly syndrome.
