Early studies of Burkitt's tumor in Africa.

While still a trainee in oncology, John Ziegler had the unexpected opportunity to direct a Burkitt's tumor research project in Africa. He was given a leading role in conducting early studies on the treatment of this tumor. Dr. Ziegler's youthful zeal, administrative talent, and scientific ingenuity enabled him, in a relatively short time, to carry out fundamental studies on treatment of Burkitt's tumor as well to establish an outstanding center for cancer research in tropical Africa. The lessons learned from these studies have become principles for treatment of many other human cancers.

Acquired Immunodeficiency Syndrome↗

Non-Hodgkin's lymphoma in 90 homosexual men. Relation to generalized lymphadenopathy and the acquired immunodeficiency syndrome.

We describe the histologic and clinical features of non-Hodgkin's lymphoma diagnosed between January 1980 and December 1983 in 90 homosexual men from San Francisco, Los Angeles, Houston, and New York. The median age was 37 years, with an age distribution identical to that for cases of AIDS reported to the Centers for Disease Control. Sixty-two per cent of the patients had high-grade (aggressive) subtypes of lymphoma, 29 per cent had subtypes of intermediate grade, and 7 per cent had low-grade subtypes. Histologic subtypes and malignant cell phenotypes were consistent with a B-cell origin. All but two men had extranodal lymphoma: central-nervous-system, bone-marrow, bowel, and mucocutaneous sites were most commonly involved. Thirty-five of 66 evaluable men (53 per cent) had complete responses to combination chemotherapy or radiotherapy or both, and thus far, 19 (54 per cent) of them have had a relapse. Mortality and morbidity were closely related to prodromal manifestations; death or illness have occurred in 19 (91 per cent) of the 21 men who presented with AIDS, in 26 (79 per cent) of the 33 who presented with generalized lymphadenopathy, and in 5 (42 per cent) of the 12 who had no prodromal manifestations. Mortality rates analyzed according to histologic grade were higher than currently reported rates in other patient populations. Kaposi's sarcoma or severe opportunistic infections characteristic of AIDS developed in 14 of 33 men (42 per cent) who presented with generalized lymphadenopathy and in 3 of 12 (33 per cent) without prodromal manifestations. We conclude that non-Hodgkin's lymphoma in members of an AIDS risk group is a serious manifestation of AIDS and the AIDS-related complex.

Acquired Immunodeficiency Syndrome↗

Acquired immunodeficiency syndrome is an opportunistic infection and Kaposi's sarcoma results from secondary immune stimulation.

Two hypotheses are presented to explain the recent outbreak of acquired immunodeficiency syndrome (AIDS) and Kaposi's sarcoma in certain risk groups, particularly homosexuals, and the absence of these diseases from other segments of the population. According to the first, AIDS is itself an opportunistic infection. It causes disease only in individuals who are already immunocompromised by hepatitis B, cytomegalovirus, parasites, or other immunosuppressive factors. This hypothesis predicts that evidence of previous contact with the AIDS agent would be found in healthy individuals who have had the immune capability to suppress its pathogenicity. It also suggests that individuals with evidence of immunodeficiency are at risk of this syndrome and should adopt a lifestyle favouring recovery of their immune system. According to the second hypothesis, the high incidence of Kaposi's sarcoma observed in immunocompromised AIDS patients is not due to decreased immunosurveillance but to a reactive secretion of immunomodulating factors with angiogenesis-promoting activity. They are released in an attempt to bring the immune system back into balance. These factors would enhance capillary proliferation and subsequent transformation of endothelial cells, perhaps via cytomegalovirus.

Spirochetes isolated from the blood of two patients with Lyme disease.

We isolated spirochetes from the blood of 2 of 36 patients in Long Island and Westchester County, New York, who had signs and symptoms suggestive of Lyme disease. The spirochetes were morphologically similar and serologically identical to organisms recently found to infect lxodes dammini ticks, which are endemic to the area and have been epidemiologically implicated as vectors of Lyme disease. In both patients, there was a rise in specific antispirochetal antibodies in paired specimens of serum. We conclude that the l. dammini spirochete has an etiologic role in Lyme disease.

Analysis of human urine for mutagens associated with carcinoma of the bilharzial bladder by the Ames Salmonella plate assay.

The Ames Salmonella plate assay was employed to test urine samples from bladder cancer patients and controls living in Egypt for the presence of chemical mutagens. Urine from five groups of Egyptian adults were tested, including individuals with (1) neither bilharziasis nor bladder cancer, (2) urinary bilharziasis and normal urinary cytology, (3) urinary bilharziasis and atypical urinary cytology, (4) carcinoma of the bilharzial bladder, and (5) bladder cancer without bilharziasis. Plates treated with histidine dependent bacteria, S-9 mix, beta-glucuronidases and 0.3 ml sterile urine from all five groups yielded 50 to 150 percent more colonies than plates treated with saline instead of urine. These differences were highly statistically significant for all groups except subjects with bladder cancer without bilharziasis. Gross and microscopic inspection suggested, however, that plates containing urine had heavier bacterial lawns than plates treated with saline. A procedure for quantitating viable bacteria in the lawn was devised which demonstrated that the increase in colonies on urine treated plates could be attributed to increased numbers of viable bacteria in the bacterial lawn on those plates. There was, therefore, no convincing evidence for the presence of mutagenic substances in these urine samples.

Outbreak of Burkitt's-like lymphoma in homosexual men.

Four cases of Burkitt's-like lymphoma (undifferentiated, monoclonal, B-cell tumours) in homosexual men were seen in a 9-month period in San Francisco. One tumour contained both Epstein-Barr-virus nuclear antigen (EBNA) and cytomegalovirus (CMV) antigen. Another tumour contained EBNA, and a third contained no viral antigen, but EBNA and CMV antigens were detected in the overlying epithelium. This outbreak widens the array of neoplasms affecting immunosuppressed homosexual men and provides further evidence of an oncogenic role for EBV and CMV.

Cytomegalovirus and Kaposi's sarcoma in young homosexual men.

10 homosexual men with Kaposi's sarcoma (KS) were studied for evidence of cytomegalovirus (CMV) infection. IgG and IgM antibodies to CMV were detected in 9 out of 9 and in 7 out of 9 of these patients, respectively. CMV was recovered from body secretions or peripheral blood of 7 patients. Viral cultures of KS tumour biopsy specimens were negative in 8 out of 8 patients, but CMV RNA was detected by in-situ hybridisation in 2 out of 3 and CMV antigen(s) by immunofluorescence in 6 out of 9. Normal tissue specimens from 3 KS patients were negative for CMV antigen. These observations suggest an association of CMV with KS.

Effects of postoperative adjuvant chemotherapy and radiotherapy on ovarian function in women undergoing treatment for soft tissue sarcoma.

Ovarian function was evaluated in 11 women 16 to 43 years of age at treatment who received doxorubicin, cyclophosphamide, and high doses of methotrexate with or without radiotherapy in adjuvant therapy of soft tissue sarcoma. Five women (16-33 yr old) who received chemotherapy alone or combined with radiotherapy only at sites distant from the ovaries (chest wall, thigh, and leg) had minimal menstrual irregularities or temporary cessation of menses during therapy; cyclic menses returned promptly after therapy. Gonadotropin levels (expressed as means +/- SD [follicle-stimulating hormone (FSH), 10 +/- 5 mlU/ml; luteinizing hormone (LH), 10 +/- 4 mlU/ml] and 17 beta-estradiol (E2) levels (means +/- SD, 208 +/- 147 pg/ml) were normal. By contrast, 4 older women (ages 36-43 yr) who received similar treatment developed persistent amenorrhea with postmenopausal levels of gonadotropin (FSH, 108 +/- 29 mlU/ml; LH, 72 +/- 19 mlU/ml) and E2 (19 +/- 8 pg/ml). Two additional women (ages 21 and 39 yr) who received radiation (7,000 rad) to the pelvis plus chemotherapy developed prompt cessation of menses and became functional castrates (FSH, 77 and 80 mlU/ml; LH, 40 and 58 mlU/ml; E2, 10 and 19 pg/ml). However, this result would be expected from the radiation dose alone. The data demonstrated that ovarian dysfunction may follow the use of doxorubicin, cyclophosphamide, and high doses of methotrexate and that the injury is age related.

Long-term experience with Burkitt's lymphoma in Uganda.

The cumulative results and long-term follow-up of all patients with Burkitt's lymphoma treated at the Uganda cancer Institute Kampala are reported. The annual admission rate is 29. The tumor patients commonly present with jaw swelling (72%), abdominal swelling (56%) and central nervous system involvement (30%). Complete response rate is achieved in a high proportion of patients (81%). About 50% of these relapse, equal numbers relapsing before and after 3 months. The most important factor influencing remission duration and survival is disease stage. Other important factors are treatment protocols and, to a lesser extent, the type of relapse. Central nervous system relapse does not necessarily augur poor prognosis as second remissions and long-term survival can be achieved with appropriate therapy. Presently 25% of all treated patients have survived free of disease well beyond 5 years.
