Effects of exercise on the coronary and collateral vasculature of beagles with and without coronary occlusion.

The purpose of this study was to determine if exercise stimulates coronary collateral vessel growth and if exercise after coronary occlusion can promote additional collateral growth. One group of 10 beagles served as control. The second group considered of 10 normal beagles exercised at 50% of maximum O2 consumption 7 days/week for 6 weeks. In the third group, the circumflex was occluded (Ameroid method) in eight beagles for 5 months. In the fourth group, seven beagles were exercised for 8 weeks following 3 months of circumflex occlusion. The coronary and collateral circulation was assessed, using an isolated heart preparation. It was found that exercise did not stimulate collateral growth in normal beagles. However, exercise after coronary occlusion doubled collateral conductance to be impaired vascular bed compared to animals with only coronary occlusions. Our study shows that, in beagles with coronary occlusion, exercise after a 3-month convalescent period promotes epicardial and intramyocardial (septa) collateral growth. Coronary reserve in the trained animals returned to 52% of initial capacity vs. 34% for the untrained animals.

Comparative trial of two sulfisoxazole regimens in acute urinary tract infection.

Many clinicians are utilizing a 2-g loading dose of sulfisoxazole in the treatment of uncomplicated urinary tract infection. Although some of these clinicians understand the theoretical reasons for not utilizing such a treatment plan, they may be reluctant to depart from the official recommendations for sulfisoxazole because of the lack of supporting clinical data. The findings of this study provide support for the theoretical considerations outlined previously. Also, considering the potential disadvantages of the loading dose employment, for example, source of patient misunderstanding and complicated patient instructions data supporting the omission of a sulfisoxazole loading dose should be most welcome. In conclusion, the study results suggest that the inclusion of a 2-g loading dose of sulfisoxazole in the treatment of this sample of acute, uncomplicated urinary tract infections did not offer any therapeutic benefit.

The septal artery and its collaterals in dogs with and without circumflex occlusion.

The purpose of this investigation was to study myocardial perfusion and the collateral circulation of the septal artery in dogs without occlusion and to determine the magnitude and time course of septal collateral development with gradual circumflex occlusion. After the dogs with circumflex occlusion for 1 and 3 mo were allowed to develop a prolific collateral circulation under normal physiological conditions, coronary and collateral resistances were determined on an isolated heart preparation with maximal vasodilation. It was found that the septal artery contributes about 25% to the collateral circulation of the major coronary arteries (right, circumflex, and anterior descending). Septal collateral growth increased up to 15 times with circumflex occlusion. Due to the anatomic location of the septal artery, this collateral growth was intramyocardial in contrast to epicardial collaterals. Although there was collateral proliferation to all major coronary vessels, the most significant growth was toward the circumflex, the ischemic vascular bed. Although primary collateralization in the dog is via epicardial collaterals, the septal artery participates in collateral development.

Research policies and practices in family practice residencies.

A questionnaire survey of all US family practice residency programs was undertaken to gather information pertaining to the policies and practices of resident research projects. Eight-one percent of the programs responded. The majority of the programs which responded require or encourage research by their residents. Cross-tabulations of the data relate several characteristics of residency programs to their positions on resident research. University based or affiliated/administered programs, programs with fewer residents and larger faculties, and programs in the faculty are engaged in research tend to encourage or require research by residents although these findings are not consistent. Information is also presented pertaining to financing resident research, preparing residents for conducting research, and disseminating the results of residents' projects.

Vertebral artery occlusion following manipulation of the neck.

A 23-year-old woman developed brainstem infarction following cervical manipulation. Vertebral angiography showed total occlusion of the left vertebral artery with a thrombus extending into the basilar artery. The literature dealing with this rare but serious complication of cervical manipulation is reviewed.

Family utilization of a medical center.

This study shows that "high utilization families" do exist and demonstrates a method, based on the calculation of expected contact rates for individuals and families, for identifying them. The high utilization families are not composed of chance combinations of high utilization individuals. This trend becomes apparent in two member families and is statistically significant for families of three or more. High utilization families were shown to have greater numbers of social problems, economic problems, and health maintenance problems when compared to a control group.
