Renal oncocytoma: new observations.

Renal oncocytomas are uncommon, benign tumors that can be treated by local excision or heminephrectomy; their preoperative differentiation from renal cell carcinoma, treated by radical nephrectomy, would be invaluable. A particularly important finding, a central scar--not stressed in previous reports, is frequently demonstrated by CT examination. We evaluated radiographic studies of 18 pathologically confirmed cases of oncocytoma and compared findings with results of CT, sonography, and angiography studies of 18 renal cell carcinoma cases. Oncocytomas can be suggested if a stellate scar is identified within an otherwise homogenous tumor on ultrasound (US) and CT; if the mass appears homogeneous but no scar is present, angiography should be performed. An oncocytoma can be suggested in these cases if a spoke-wheel configuration and homogeneous blush are present. These criteria, which are reliable only if the mass is 3 cm or larger, would have resulted in the correct diagnosis of oncocytoma in 16/18 cases.

Angiomyolipoma: ultrasonic-pathologic correlation.

Previous reports of angiomyolipoma have emphasized the ultrasonic finding of a very echogenic intrarenal mass. Ten cases of angiomyolipoma for which ultrasound examinations were available were retrospectively reviewed and correlated with their pathologic findings, and, when available, with computed tomographic findings to determine if other sonographic patterns could be recognized. In addition to the typical echodense pattern (six cases), a mixed pattern (two cases) and a low sonodense pattern (two cases) were discovered. To determine if the typical echodense pattern is specific for angiomyolipoma, 42 cases of renal cell carcinoma for which ultrasound examinations were available were also retrospectively reviewed. In two cases of renal cell carcinoma echodense patterns that were indistinguishable from those of angiomyolipoma were found. Two conclusions are reached: (a) there is a spectrum of ultrasonic findings with angiomyolipoma, and (b) the presence of an echodense intrarenal mass, although very suggestive of angiomyolipoma, is not pathognomonic.

Intravascular bronchioloalveolar tumor.

We report clinical and histologic characteristics of a case of a recently described, rare pulmonary neoplasm. This entity, intravascular bronchioloalveolar tumor, is often misdiagnosed as a result of lack of awareness of both its clinical presentation and unique morphologic appearance. Histologic features in our case lend support to a vascular origin of this tumor.

Angiomyolipoma: computed tomographic-pathologic correlation of 17 cases.

The angiomyolipoma is a benign renal tumor composed of fat, smooth muscle, and abnormal blood vessels. The tumor often extends into the perinephric space and often is complicated by intratumoral or perinephric hemorrhage. The CT findings of 17 cases of angiomyolipoma were correlated with their pathologic findings and are described. CT scans usually demonstrate a large fatty mass intermixed with areas of tissue density which may represent nonfatty parts of the tumor or areas of hemorrhage. CT is more sensitive than plain films at detecting the fat within the angiomyolipoma. CT is also useful in identifying perinephric extension and hemorrhage. In those angiomyolipomas composed primarily of smooth muscle or in which intratumoral hemorrhage has obscured the fatty part of the tumor, the CT appearance is indistinguishable from other solid renal tumors.

Checking the reliability and validity of HSPQ profiles.

Developed a scale designed to detect invalid response patterns on the High School Personality Questionnaire (HSPQ). Using a large sample of protocols drawn from the HSPQ standardization group, ten items were found to satisfy the necessary criterion for inclusion in the resulting "random" or "validity" (RV) scale. When applied to separate cross-validation groups of nearly 2,000 valid and randomly generated response protocols, RV was found to correctly classify 79% of the profiles. As a further test of the scale's utility, 100 profiles classified as valid and 100 classified as invalid were selected for comparison. In the invalid group, KR-20 reliabilities of the 14 trait scales were almost uniformly lowered, Also, for the invalid group the correlations among the trait scales were attenuated and altered to the extent that the underlying factor structure of the test was upset.

Cerebral laterality and verbal processes.

Research suggests that we process information by way of two distinct and functionallly separate coding systems. The localization of these two processing systems appears to be somewhat dependent on cerebral laterality, which has been shown to vary in right-handed and left-handed persons. To test the dual coding model, right-handed and left-handed subjects learned lists of abstract and concrete words under various conditions of visual and tactile interference. Right-handed subjects showed a significant superiority in the remembering of highly concrete items, while total recall did not differ reliably between groups.
