Computed tomography in adrenal tumors.

Computed tomography (CT) was used to evaluate 26 patients with a variety of adrenal lesions. Surgical proof was available in 22 patients and clinical confirmation with a variety of other studies in the other four patients. Nine patients ahd aldosterone-producing adrenal adenomas and CT correctly identified seven. Four patients had cortisol-producing adenomas and five patients had cortisol-producing carcinomas; CT identified each of these tumors. Prominent but normal shaped glands were seen in each of the four patients with adrenal hyperplasia. Adrenal metastases from malignant melanoma in two patients were identified. Only one of two pheochromocytomas in two patients could be seen on CT. CT is a noninvasive method of localizing adrenal tumors and may be helpful in distinguishing adenomas from adrenal hyperplasia.

Intraperitoneal contrast infusion for assessment of intraperitoneal fluid dynamics.

For patients undergoing intraperitoneal ("belly bath") chemotherapy, it is useful to verify the distribution of infusion fluid. For this reason, 10 patients were examined by computed tomography (CT) after intraperitoneal instillation of dialysate mixed with water soluble contrast material. The extent of intraperitoneal distribution was assessed, and in two patients filling defects, presumably representing tumor, were identified.

Paraplegia due to posttraumatic pelvic arteriovenous fistula treated by surgery and embolization. Case report.

A case is presented in which a posttraumatic pelvic arteriovenous fistula caused progressive paraplegia because of voluminous shunting into the epidural venous system. Surgical ligation and transcatheter embolization of major and minor arterial feeders decreased shunt flow sufficiently to permit direct embolization of the fistula by an injectable plastic. This combined approach may allow obliteration of unresectable acquired or congenital arteriovenous malformations.

Acute occlusion of the posterior spinal vein. Experimental study in monkeys.

The posterior spinal vein was occluded with silicone in seven rhesus monkeys, and locally resected in one. There were no neurological findings associated with acute venous obstruction of the cord. Follow-up arteriography revealed diversion of venous outflow into the anterior spinal venous system. Histology revealed gliosis associated with demyelinization confined to the posterior columns.

Retrorenal cysts.

Three patients are presented in whom computed tomography incidentally demonstrated low density masses posterior to, but separate from, the right kidney. In two, these were proven to represent unilocular cysts, and the third is presumed cystic. Possible etiologies are discussed.

Noninflammatory gas formation following embolization of adrenal carcinoma.

Gas formation as a result of transcatheter tumor embolization has recently been observed by conventional radiography. We report the computed tomography identification of similar benign gas formation following transcatheter embolization of a large adrenal carcinoma. Computed tomography should be the most sensitive method available for detecting gas formation. It is important to recognize this gas formation as a consequence of embolization and cell necrosis rather than as an indication of an inflammatory complication.

Lung tomography in cancer patients. Full-lung tomograms in screening for pulmonary metastases.

In a retrospective study of the cases of 415 patients with various malignant neoplasms, full-lung tomography and conventional chest roentgenography were compared for accuracy in the detection of pulmonary metastases. Sufficient information was available on 410 patients to permit confirmation as to whether pulmonary metastases were present at the time of screening chest roentgenographic and tomographic examinations. On screening by routine chest roentgenography, metastases were detected in 36 patients, 66.7% of the total with confirmed metastatic disease. Screening by full-lung tomography identified metastases in 51 (94.4%). Fifteen patients with normal chest roentgenograms had metastatic lesions on tomograms. Metastases were detected in 8.8% by chest roentgenograms and in 12.4% by tomograms in a population of cancer patients with a 13.2% incidence of pathologically confirmed metastatic lesions.

A rapidly polymerizing polyurethane for transcatheter embolization.

A polyurethane prepolymer was evaluated as a transcatheter embolizing agent. Low viscosity permits injection through small catheters. Polymerization is initated on contact with blood and completed within 10 seconds. Downstream propagation is better then that of cyanoacrylate and comparable to that of silicone rubber. No systemic toxicity was observed in acute animal experiments. However, dissolution of arterial walls and extravasation out of the vascular bed of chronically embolized organs suggest significant local tissue toxicity and make further evaluation necessary before clinical testing.

Excretory urography in evaluation of the surgical cancer patient.

Excretory urography was performed routinely in 504 patients with sarcoma, melanoma, pelvic, head and neck, or other localized tumors over a 5-year period. There were benign abnormalities in 14.4% of the patients examined, tumor associated abnormalities in 10.7%, and 1.4% false positives. The diagnosis of a benign lesion did not change cancer therapy in any patient. Findings on excretory urography were valuable in the management of patients with pelvic and intra-abdominal tumors. In these patients ureteral obstruction was always caused by tumor. There was a high rate of false positively in non-abdominal tumors. No second primary tumors were found.

Cyclophosphamide-induced remissions in advanced polyarteritis nodosa.

Two patients with far advanced polyarteritis nodosa involving multiple organ systems and with aneurysm formation demonstrable by celiac axis and renal arteriograms were treated with oral cyclophosphamide, 1 to 2 mg/kg/day, and alternate day prednisone therapy. Dramatic remissions of disease activity were achieved within weeks of initiation of therapy. Repeat angiograms obtained after one year revealed complete resolution of all aneurysms. Both patients are now receiving only cyclophosphamide and are maintained in complete remission 27 and 18 months after the start of therapy. Cyclophosphamide therapy can thus be highly effective even in far advanced polyarteritis nodosa.

Adrenal cortical tumors with low attenuation coefficients: a pitfall in computed tomography diagnosis.

Four cases of adrenal tumors with unusually low attenuation coefficients are reported. The tumors presented with values ranging from 8 to 17 EMI units (EU: 500 scale) instead of the expected 30 to 40 EU range of the remainder of our adrenal tumors. Two of these patients had primary aldosteronism and two had Cushing's syndrome. An unusually high lipid content may be responsible for the lucent appearance.

Detection of subcutaneous metastases by computed tomography.

Subcutaneous fat provides excellent contrast for water density tissue. Five patients with subcutaneous metastases visualized on computed tomography (CT) scans are presented. In some cases, CT proved to be the optimal diagnostic modality for visualizing these lesions and documenting their response to therapy.

Transcatheter brush biopsy of intravenous tumor thrombi.

Transcatheter brush biopsy of intraluminal filling defects demonstrated during venography was performed in 6 patients with abdominal neoplasms. Tissue specimens consistent with either known or subsequently proved tumor were obtained from 4 patients. Biopsy was negative in the other 2 patients, who lacked surgical confirmation of the intraluminal mass. This technique is useful in establishing histological proof of neoplasm and in determining the extent of known tumor.
