Incest and child sexual abuse.

Child sexual abuse was examined nationally and in the Washington, DC and Howard University Hospital area. In an attempt to describe this widespread problem, two case histories are presented which reflect some of the typical characteristics of child sexual abuse cases seen at Howard University Hospital. Pertinent literature is reviewed citing the prevalence rates and the personality and environmental factors which may contribute to the sexual abuse of children in this country. Finally, the role of the physician in identifying and treating the physical and emotional effects of child abuse are discussed.

Hope Index Scale: an instrument for the objective assessment of hope.

Although many clinical observations suggest that Hope influences the onset, duration, prognosis, and recovery from mental and physical illnesses, a lack of direct scientific proof persists because no method exists for the objective assessment of hope. We have now constructed the Hope Index Scale, a testing instrument for the measurement of this rather elusive human attribute. Upon testing control and experimental subjects, it was found that score distribution on the Hope Index Scale correlates negatively with Beck's Hopelessness Scale (Pearson r = -.88, P <.001) and is independent of age, race, or sex. It is concluded that this tool can help identify individuals with varying degrees of psychosocial problems and that scores of 150 or below are indicative of pathologic hope deficit often associated with suicide.

Hairy cell leukemia in father and son.

A father and son hairy cell leukemia are identified on the basis of tartrate-resistant acid-phosphatase-positive hairy cells and diagnostic bone core biopsies. Personal interviews of the two patients revealed no evidence of consanguinity, primary immune deficiency, or common medication use. HLA typing revealed both patients to be A1,3 and B8,14.

Tumorigenicity and lysis by natural killers.

Detailed analysis of the natural killer (NK) activity directed at nontumorigenic cell lines and their transformed tumorigenic derivatives has revealed a paradox. On the one hand, a correlation has been found between the tumorigenic potential of chemically transformed fibroblast cell lines and their sensitivity to NK cells in vitro. Nontransformed cells (N-type cell lines) and cells tumorigenic in normal mice (C-type cell lines) are resistant to NK-mediated lysis. In contrast, cell lines that are tumorigenic in ATxFL mice (these mice are very low in NK activity), but not in normal mice (I-type cell lines) are sensitive to NK-mediated lysis. These findings support the concept that NK activity is involved in host surveillance against tumors. On the other hand, NK-resistant fibroblasts, whether taken directly form animals or derived as tumorigenic or nontumorigenic cell lines, compete with NK-sensitive target cells to inhibit their lysis by NK effectors. Not only are both NK-sensitive and -resistant cells recognized by NK effectors but both receive lytic signals from NK effector cells. Target cell resistance is a result of a protein synthesis-dependent mechanism that prevents lysis such that in the presence of inhibitors of protein synthesis all fibroblasts tested are NK sensitive. Those fibroblasts that are normally sensitive to NK-mediated lysis must be deficient in their ability to produce or respond to this counterlytic mechanism. These findings are in contrast with the general findings when lymphoid cells are studied as NK targets where sensitivity appears to be a result of recognition by NK effectors. Because our findings show that transformed and normal cells express the same recognition determinants, in order for NK activity to play an important in vivo role in tumor surveillance, a mechanism must operate to permit NK effectors to find their targets in vivo. In the absence of a special discrimination mechanism, the killing of NK-sensitive transformants that arise autochronously would be less than optimal as a consequence of competition by the normal, NK-resistant, cells.

Frequency of schizophrenia and depression in a black inpatient population.

A retrospective study covering the period 1974-1978 was conducted on the inpatient population at the Department of Psychiatry, Howard University Hospital, for the two major diagnostic categories, major affective disorders (MAD) and the schizophrenias. Among the schizophrenias, the diagnoses of schizophrenia, paranoid type, and schizophrenia, not otherwise specified, account for approximately 73 percent of all schizophrenic diagnoses. The distribution of diagnoses among the MAD category demonstrated that approximately 74 percent of all diagnoses were accounted for by the subcategory depressive neurosis. Further analyses according to age at admission and annual trends were also conducted. The implications of these analyses, conducted on all black patient populations who were diagnosed by nonwhite diagnosticians, are discussed.

Anti-tumor potential of retinoic acid: stimulation of immune mediated effectors.

Data are presented on the effects of retinoic acid (RA) treatment on the in vivo growth of tumors in two mouse strains. Inhibition of tumor growth as a result of systemic RA injection was observed in three out of seven tumor models. Using adult thymectomized, lethally irradiated and fetal liver (A x TFL) reconstituted mice, we found that inhibition of tumor growth is not due to direct toxic effects of RA but rather appears to be the result of stimulation of thymus-dependent immune-mediated effectors to suppress tumor growth. Results suggest that only strongly immunogenic tumors are sensitive to in vivo retinoid inhibition.

Characterization of an antigen from the myelogenous leukemia cell line K-562.

A protein was solubilized from the myelogenous leukemia cell line K-562 WITh 3 M KCl that specifically inhibited the antibody-dependent, complement-mediated cytolysis of 51Cr-labeled K-562 cells by a monkey antiserum to K-562. When the crude 3 M KCl extract uas fractionated with ammonium sulfate, an eightfold increase in specific activity (U inhibition/mg protein) resulted. This purified fraction migrated as a single protein band after polyacrylamide gel electrophoresis (PGE) with no detectable carbohydrate or lipid. The molecular weight of the denatured protein determined by sodium dodecyl sulfate-PGE was 77,000, similar to that of the native protein (80,000) determined by Sephadex exclusion chromatography. The protein was stable at pH 6-8, with an apparent isoelectric point between pH 5 and 6. In addition to being irreversibly denatured at pH 5 or less, it was unstable at osmolarities below 0.25 M (NaCl). It was denatured at temperatures of 56 degrees C or above. Normal human peripheral blood leukocytes were extracted similarly with 3 M KCl and fractionated with ammonium sulfate. Neither the crude preparation nor any fraction purified as described for the specific antigen inhibited the cytolytic assay, which indicated at least a quantitative lack of the protein on the surfaces of normal leukocytes.

A description of Walter Reed Army Medical Center's in-patient psychiatric service population 1973 to 1975.

A three-year evaluation of demographic and diagnostic patterns in a large Army psychiatric in-patient facility is described. Active duty personnel accounted for 83.6 percent of patient episodes. No simple catchment area could be defined for this facility. Active duty patient episodes tended to be with younger, junior enlisted men whose sicknesses were most frequently diagnosed as schizophrenia. The diseases of blacks were diagnosed as paranoid schizophrenia more frequently than in whites. Latent schizophrenia or undifferentiated schizophrenia were diagnosed more frequently in whites than in blacks. The illnesses of active duty female military personnel were more frequently diagnosed as neurotic than as schizophrenic. Of the patient episodes during the three-year period, 12.1 percent were about dependents. They were usually the wives of older, senior enlisted men or senior officers and they stayed an average of 12 days in the hospital. On the other hand, 4.3 percent of the patient episodes were about retired personnel. They came from Walter Reed Army Medical Center (WRAMC) as did their dependents, and the most frequent diagnosis was alcoholism. Their median stay was 15 days. Subsequent studies will attempt to further clarify these initial findings.

Crohn's disease of the appendix presenting as acute appendicitis.

A patient is reported who initially presented with findings simulating acute appendicitis and who was subsequently found to have Crohn's disease isolated to the appendix. Although the hazard of appendectomy in Crohn's disease is well known, it is interesting that none of the known patients with isolated appendiceal Crohn's disease has developed a fistula. Three of the 15 reported cases of Crohn's disease solely involving the appendix developed granulomatous changes involving other regions of the bowel as long as four years following the initial diagnosis. Because of the rarity of this condition, however, specific conclusions regarding the likehood of future recurrence cannot be drawn. We stress increased physician awareness of this entity in order to emphasize long-term follow-up for such patients.
