Occurrence and activity of myofibroblasts in human capsular tissue surrounding mammary implants.

Capsular tissue obtained from a series of 16 patients with silicone mammary implants was exposed in vitro to smooth-muscle stimulants and relaxants. Capsule relaxation was produced in 75 percent of the patients. The most active antagonists were papaverine and veratrine. Capsule contracture was produced in 69 percent of the patients. The most active agonists were histamine and epinephrine. Myofibroblasts were identified in several capsules. These data support the hypothesis that the myofibroblasts is a probable cause of capsular contracture.

Results of closed compression capsulotomy for treatment of contracted breast implant capsules.

A prospective study of 544 nonsurgical, closed compression capsulotomies performed on 243 patients for rupture of contracted breast capsule, with at least 1 year follow-up after the last closed capsulotomy procedure, is reported. Two-thirds of the 243 patients maintained softness equal to an unoperated breast for a minimum period of 1 year after the last closed compression capsulotomy. The majority of the patients followed in this study developed breast capsular contracture in less than 12 months after augmentation mammaplasty. The time interval for recurrence of capsular contracture after this first closed capsulotomy correlated closely with the length of time for development of capsular contracture after augmentation mammaplasty. Closed compression capsulotomy will relieve most capsule contractures following augmentation mammaplasty; however, multiple closed capsulotomies may be required to maintain soft breasts, and the patient may require surgical capsulotomy. The procedure offers and alternative to additional surgery or accepting firm breasts and is performed with a minimum of morbidity and financial burden.

Detection and follow-up of moderate and severe hypertensive subjects in the Australia community.

A national screening project has detected 989 subjects aged 30 to 69 years with untreated moderate or severe hypertension. The 1-2% incidence of this condition indicates a total of about 100,000 people in the Australian community potentially requiring therapy. The sample showed that half the subjects were previously unaware of a blood pressure problem. Those so detected showed a very high rate (90-7%) of acceptance of advice to see their own doctor. The local doctors confirmed the elevated pressure level in nearly two-thirds of the patients referred, and initiated treatment in a similar proportion.

Treatment of recurrent pregnancy hypertension by prophylactic anticoagulation.

A patient is described who had two pregnancies complicated by severe pre-eclampsia in the second trimester which resulted in intrauterine death. During the third pregnancy anticoagulant therapy with warfarin was instituted at 14 weeks gestation and a healthy female infant was delivered at 38 weeks gestation.

Report on progress in the Australian National Blood Pressure Study (NBPS).

1. The National Blood Pressure Study (NBPS) is a single blind trial designed to test the efficacy of active drug treatment in reducing complications from mild hypertension (mean diastolic pressure = 95-109 mmHg). 2. Between 1973 and 1975, four centres screened about 104 000 subjects aged 30-69 years, yielding an estimated prevalence of hypertension (greater than or equal to 95 mmHg diastolic) of 16% and of moderate-to-severe hypertension (greater than of equal to 110 mmHg diastolic) of 1-3%. 3. Some 4000 subjects selected for untreated uncomplicated mild hypertension were randomized to either active treatment (cholorothiazide +alpha-methyldopa and/or a beta-adrenoreceptor antagonist as required) or to matching placebos. 4. At 1 year mean pressures had fallen significantly below entry pressures in both groups but in the active group the fall was greater by a margin of 14-4+/-1-3 (SEM) mmHg systolic and 7-1+/-0-7 mmHg diastolic. At 1 year 5% of subjects in the placebo group had been placed on active treatment on the ethical grounds that pressure had exceeded the mild hypertension limit. 5. Trial end-points (death, morbidity from stroke, hypertensive heart and renal disease, and ischaemic heart disease) number 106 (nine deaths) thus far, of which ischaemic heart disease accounts for 71% and stroke 19%. 6. The duration of trial may need to be extended beyond the original estimate of 5 years.

Closed compression technique for rupturing a contracted capsule around a breast implant.

When a breast becomes hard after an augmentation mammaplasty, the fibrous capsule around the implant can be ruptured, in most cases, by closed compression of the breast-resulting in immediate softness of the breast. As yet the duration of this improvement is unknown, but follow-up examinations to date indicate that it is persisting in about 3 or 4 patients so treated.
