Possible prognostic significance of the assessment of dipeptidylpeptidase II in peripheral blood lymphocytes of patients with chronic lymphocytic leukemia.

In 115 patients with chronic lymphocytic leukemia (CLL), grouped according to the staging system as proposed by Rai et al., dipeptidylpeptidase II (DPP II) reaction was carried out in smears of peripheral blood using Lys-Pro-MNA and Lys-Ala-MNA as substrates and Fast Blue B (FBB) as the coupling agent in cacodylate buffer, pH 5.5. In patients with stable disease a significantly lower number of DPP II positive lymphocytes was found. On the other hand, a high number of DPP II positive lymphocytes indicated a progressive disease. Lys-Ala-MNA proved to be a better substrate for these purposes. When comparing these results with the stage of the disease proposed by Rai et al., a good correlation was found for Stages 0, I and II. However, in the Stages III, and particularly in the Stage IV, there was a wide range of DPP II values, although the average value calculated from the data of all patients of this group was high. Patients with low DPP II values had usually a stable disease in spite of thrombocytopenia which precluded them from allocation to Stage IV. It is concluded that the assessment of the number of DPP II positive lymphocytes represents a simple and reliable prognostic criteria in patients with B type CLL.

Radiation data input for the design of dry or semi-dry U tailings disposal.

Before discussion of design criteria for the handling of dry or semi-dry tailings, it is necessary to obtain an insight into the radiation levels associated with the tailings particles and to study the basic physical properties of dry tailings. This article presents the experimental results of assessing Ra and specific alpha-activity distribution with respect to particle size of the Ranger (RUM) and Nabarlek (QML) uranium mines dry tailings samples. The variation of Rn emanation coefficient versus particle size of dry tailings has also been measured. The nuclear-track detection technique, gamma spectrometry and alpha counting were used for the above measurements. Surface Rn flux from the hypothetical Nabarlek semi-infinite dry tailings pile is 32 Bq m-2 s-1 and the Rn flux for Ranger is 10 Bq m-2 s-1. The theoretical exposure rates for 1 m above these hypothetical tailings piles are 0.95 microC kg-1 h-1 and 0.28 microC kg-1 h-1, respectively. The derived air alpha-contamination limits (DAAC) for the tailings dust were calculated to be 1.2 Bq m-3 for workers and 0.034 Bq m-3 for a member of the public. The limit for workers corresponds to the air tailings dust concentration of 0.79 mg m-3 for QML tailings and 2.2 mg m-3 for RUM tailings. The DAAC limit for the public corresponds to the air tailings dust concentration of 0.022 mg m-3 for QML tailings and 0.064 mg m-3 for RUM tailings.

Distribution of T-lymphocytic subpopulations detected by monoclonal antibodies in peripheral blood of patients with chronic lymphatic leukaemia.

This paper reports on 93 patients with chronic lymphatic leukaemia (CLL) where CLL of B-type (E-, T-, B+, Ig+/-) was defined in 84 cases (90%), CLL of T-type (E+, T+, B-, Ig-) in 3 cases (3.2%) and in the remaining 6 cases (6.4%) neither T nor B were clearly detected. In 18 patients exhibiting lower total leukocyte count in peripheral blood T-lymphocytic population (OKT-3+), helper/inducer (OKT-4+) and suppressor/cytotoxic subpopulations (OKT-8+) were analysed using monoclonal antibodies in an indirect immunofluorescence test. The results show that the mean T-lymphocytic population counts in patients with CLL of B-type amounted to 1.7 X 10(9)/1 which is the value somewhat increased compared with the mean values observed in normal donors (1.5 X 10(9)/1). The counts of helper/inducer T-lymphocytic subpopulations (OKT-4+) were 1.7 X 10(9)/1 corresponding to those of healthy donors. In contrast, the counts of suppressor/cytotoxic T-lymphocytic subpopulations (OKT-8+) in patients with B-CLL were increased (0.7 X 10(9)/1). The immunoregulation index in patients with CLL decreased to 1.4 compared with that of a control 1.96).

Problems with prognostic criteria in patients with chronic lymphocytic leukemia with special reference to DPP II assessment.

106 consecutive patients with chronic lymphocytic leukemia (CLL) were entered into prospective study with the aims of finding out whether a detailed clinical, biochemical, immunological and cytochemical examination might help to subclassify CLL with respect to prognosis. None of the biochemical and immunological parameters studied correlated either with the clinical stage of the disease according to Rai, or with further evolution of the disease. On the other hand, the results of cytochemical tests indicated that the reaction for DPP II (dipeptidylaminopeptidase) might be of prognostic value, because in patients with stable disease, significantly lower number of DPP II positive lymphocytes were found when compared with patients suffering from progressive disease. The authors also point out some short-comings of Rai's staging system in respect to its prognostic significance.

Atrial flutter treatment by rapid atrial pacing.

49 patients with 53 episodes of atrial flutter were treated by rapid atrial pacing. Sinus rhythm was attained in 55%, atrial fibrillation in 26%, type II flutter in 9% of patients. In the other cases, the effect on atrial action was only slight or short-lasting. Among the causes of the unsuccessful outcome of treatment, the authors include--besides insufficient rate and duration of pacing, bad electrode contact in the atrium and insufficient outlet current--specific properties of the type of flutter (impure flutter, type II flutter, type I atrial fibrillation, uncommon flutter) as well as unsuitable indication (mitral defect, thyrotoxicosis).
