Natural course of unoperated intracranial arteriovenous malformations: study of 50 cases.

The clinical course of 50 patients with conservatively treated intracranial arteriovenous malformations (AVM's) was followed, most of them for more than 5 years. The average follow-up period was 13.4 years. The initial symptom was intracranial bleeding in 29 patients (58%) and seizure in 15 patients (30%). Small and deep-seated AVM's were associated with a high incidence of bleeding; however, repeated hemorrhages were not necessarily indicative of a poor prognosis. Children younger than 15 years had a better prognosis than adults. There was no correlation between pregnancy and bleeding. In the hemorrhage group, the incidence of rebleeding was 6.9% in the 1st year after initial rupture, 1.91% per year after 5 years, and 0.92% per year after 15 years. The overall incidence of rebleeding was 34.5% in the hemorrhage group. Of the 50 patients, 37 (74%) had a good clinical outcome, four (8%) had a fair outcome, and four (8%) had a poor outcome; five patients died.

[Intrathecal perfusion of ACNU neurotoxicity and intrathecal pharmacokinetics in dogs].

We studied the feasibility of intrathecal ACNU perfusion therapy against subarachnoid dissemination of malignant glioma. Intrathecal perfusion was performed in adult dogs by constant drip administration of 1 to 2 mg ACNU dissolved in 10 to 20 ml of lactate Ringer solution into the lateral ventricle and cerebrospinal fluid drainage through the lumbar puncture. The perfusion time was changed from 15 to 71 min. A bolus injection of 2 mg ACNU was also tested in one dog. No neurological symptom was noted during and after perfusion, and histological examination reveal only a minimum denudation of ependyma in a small area. Concentration of ACNU in CSF and serum were measured by HPLC (high-performance liquid chromatography). ACNU was detected in lumbar CSF only by perfusion, not by bolus injection, and the maximum concentrations were 6.26 to 25.76 micrograms/ml. The elimination phase of ACNU in lumbar CSF followed linear kinetics and the half-time was 18 min on average. AUCs (area under the drug concentration-time curve) were 346 to 896 micrograms.min/ml and they were the equivalent of in vitro cell kills in excess of 3 logs for rat 9L gliosarcoma and human glioma 126 cells. Serum concentration was 0.10 micrograms/ml in maximum. These findings suggest the feasibility of intrathecal ACNU perfusion therapy against subarachnoid dissemination of malignant glioma and warrant further studies.

[Regrowth patterns of glioma--cases of glioma regrew away from the original tumor].

There have been conflicting opinions regarding the correct volume to be used in radiotherapy fields for malignant glioma. Whereas many clinicians recommend limited fields designed to cover the tumor volume with a margin according to the facts about 90% of recurrent malignant glioma regrew within 2 cm of the original tumor and regrowth away from the original tumor was seen in about 5%. Two cases of malignant glioma regrew away for the original tumor and one case of malignant glioma regrew into the resected site were presented. And the biological characteristics of glioma cells concerning with the invasiveness were discussed.

[Clinical significance of sialic acid in the CSF; in relation to the malignancy and leptomeningeal dissemination].

We examined the level of sialic acid in cerebrospinal fluid (CSF) in 20 patients with brain tumor, and 5 patients with non-neoplastic disease of central nervous system. CSF concentration of sialic acid in the patients with malignant and semi-malignant or benign brain tumor was 5.26 +/- 2.39 mg/dl, and 1.82 +/- 1.61 mg/dl, respectively. In patients with non-neoplastic disease, it was 1.64 +/- 1.53 mg/dl. The difference between groups was statistically significant (p less than 0.005). In two patients with malignant brain tumor, the level of sialic acid was decreased by radiation therapy or intrathecal chemotherapy. In conclusion, CSF concentration of sialic acid was significant of diagnosis of brain tumor character of malignancy, and effect of therapy for CSF dissemination.

[Antitumor effect of a new anthracycline derivative, MX2, against human glioma cells].

A new morpholino anthracycline derivative, MX2, has a potent antitumor activity similar to adriamycin. Because of high lipid solubility of MX2 (log P = 2.84, pH 7.4), the compound can cross the blood-brain barrier easier than adriamycin. In this study we tested the anti-tumor activity of MX2 against human glioma in vitro and in vivo. MX2 showed a marked cytocidal effect against U105MG, U251MG, U373MG and KMG4 cells when more than 30 ng/ml was added in the medium. Although the sensitivity against MX2 differed depending on the cell lines, the cytocidal effect of MX2 was similar to adriamycin. The colony formation of KMG4 cells was markedly inhibited by MX2 dose-dependently. The growth of KMG4 cells inoculated subcutaneously into nude mice was inhibited by the intraperitoneal administration of MX2 4 or 1.3 mg/kg. These results suggest that MX2 seemed to be useful in the treatment of malignant glioma.

[Angiosarcoma of the liver and pineal region].

Angiosarcoma is a rare malignant tumor originating from vascular endothelial cells. We have experienced a case of 17-year-old man, who had angiosarcoma in the pineal region and the liver. Patient's initial symptom was headache and CT scan revealed a high density mass in the pineal region with obstructive hydrocephalus. After the radiation therapy, the tumor was disappeared completely on CT scan. One year later, he developed abdominal pain, and CT scan and angiogram revealed multiple angioma like lesions in the liver. The mass in the pineal region showed concomitant regrowth, and finally, the patient was died of abdominal hemorrhage. At autopsy, pineal tumor and hepatic tumor were both angiosarcomas, although it was uncertain which was the original tumor.

Intracerebral venous angioma with arterial blood supply: a mixed angioma.

An 18-year-old man was operated upon with a diagnosis of intracerebral hematoma. Although the angiogram of the right carotid artery demonstrated the characteristic umbrella appearance of the venous angioma in the parietal lobe, fine arterial branches supplying the vascular malformation were noted in the preoperative angiogram. At the operation, no nidus was found, and delicate angiomatous networks with red veins were observed around the hematoma cavity. Histologic examination of the angiomatous network proved it to be an arteriovenous malformation. It was a mixed angioma, which combined venous angioma and arteriovenous malformations. Clinical, angiographic, and pathological features of intracerebral venous angiomas with arterial components were reviewed from the literature.

Syringomyelia and syringobulbia associated with an ependymoma of the cauda equina involving the conus medullaris: case report.

A patient with syringomyelia and syringobulbia secondary to an asymptomatic ependymoma of the cauda equina involving the conus medullaris is described. Delayed metrizamide computed tomography myelography was decisive for making the diagnosis of a highly extended syrinx associated with the tumor. Syringomyelic symptoms improved remarkably following the removal of the tumor, and the syrinx was not visualized in a postoperative study. The pathogenesis of syringomyelia associated with a caudally located spinal cord tumor is discussed.

Immunopathological significance of IgG in meningioma in relation to the pseudopsammoma bodies.

We described two histochemical patterns of IgG deposits in meningioma and discussed the pathogenesis of IgG deposits. In meningioma with pseudopsammoma bodies, IgG was secreted from tumour cells which differentiated to secretory epithelial cells, and was found to deposit in the pseudopsammoma bodies. However, in meningioma without pseudopsammoma bodies, IgG was derived from plasma and became deposit around the blood vessels in the meningioma tissue.

[Treatment of meningeal carcinomatosis--neocarzinostatin perfusion therapy in the CSF pathway].

The authors presented the results of perfusion chemotherapy in the CSF pathway with neocarzinostatin (NCS) for the treatment of meningeal carcinomatosis. In three cases of stomach cancer and one case of malignant lymphoma, cranial polyneuropathy, progressive hydrocephalus and positive cytology of the CSF were found during treatment of malignant tumors. NCS therapy was performed by means of perfusion from the spinal subarachnoid space to the ventricle utilizing Ommaya's reservoir, or simply by repeated lumbar punctures. In all of four cases the number of malignant cells in the CSF was significantly diminished, and temporary neurological improvement was observed until the patient's death due to advanced primary lesions. These results indicated that in cases of meningeal carcinomatosis, NCS perfusion therapy using the CSF pathway was a much more favorable method than treatment with other chemotherapeutic agents with less noticeable side effects.
