Labelling patterns of carcinomas of the cervix and their precancerous stages after 3H-thymidine incorporation.

1. In-vitro labellings of non-keratinizing squamous cell carcinomas of the cervix uteri yielded greatly differing labelling indices (2.1% to 20.2%) and less greatly varying mitotic indices 0.2% to 1.0%). When the existing limits are taken into account, these values may be indicative of the growth intensity of tumours. 2. The labelling indices obtained for the individual layers of both the normal and atypical squamous epithelium of the portio uteri show that there takes place with the growing degree of dysplasia via the carcinoma in situ to the microinvasive carcinoma a progressive broadening of the zone of growth. Parallel to this, there is an increase in labelling intensity, although it is not possible to derive there from a criterion of malignancy in a given case.

The proliferative activity of carcinomas of the cervix after radiation therapy.

The findings obtained so far from 15 patients with advanced non-keratinizing squamous cell carcinoma of the cervix uteri show a growing fall in 3H-thymidine incorporation at different intervals during radiation therapy under the application of 2,000 to 5,000 rad. The radiogenic reduction of S-phase nuclei occurs at lower radiation doses than do changes in the histopathological structure; this reduction is however ambiguous, since it is no more than just a rough indication of the decreasing proliferative intensity.

Proliferation, migration and cell renewal within the arterial wall.

1. Arterial endothelial cells and adventitial fibroblasts have a higher rate of renewal than medial myocytes. As has been shown for rabbit aortic endothelial cells, the decrease with age in the cell turnover os caused by both a lengthening of the cell cycle of the proliferating cells and a reduction of the growth fraction. The arterial wall cells exhibit a heterogeneous division potential which is dependent on their position within the vessel. 2. A dilation trauma caused by a balloon catheter as well as numerous exogenous (freezing, ionizing radiation, noradrenaline application) and endogenous (hypertension, hypoxia, endotoxin shock) influences bring about more or less pronounced endothelial alterations. Probably, the secondary development of fibrocellular intimal plaques is dependent on the extent of endothelial regeneration. There is a possibility that complete re-endothelialization provides some protection against the development of plaques. 3. Proliferation kinetic characteristics of intimal cells from atherosclerotic plaques tell against the development of such plaques from one cell clone. The question whether or not proliferating intimal cells are of a monoclonal or a polyclonal origin cells for further investigations. Monocytogenic and myogenic foam cells have a relatively high proliferative activity and the ability to emigrate from atherosclerotic plaques at certain stages of their development. The "response-to-injury" hypothesis of atherosclerosis put forward by Ross (1977), which corresponds to the view held by European authors that atherosclerosis is a modified form of wound healing, is supported by the proliferation kinetic behaviour of vascular wall cells.

[Interruption of pregnancy by vacuum aspiration or uterotomy. Intra- and postoperative complications].

From 1971-1977, 1097 pregnancy-interruptions were performed by vacuum-aspiration and 152 by uterotomy. In 93,3% of the patients a psychiatric indication was registered and in 6,7% the interruption was medicosomatically indicated. Surgery was done under stationary conditions in anaesthesia with halothane/N2O. 39,5% of the women operated were unmarried, 34,1% primigravides and 39,7% without children. Before the undesired pregnancy approximately half of the patients did not practice contraception. The average interval between first visit to the doctor and interruption was 18,9 days. 77% of the vacuum-aspirations were performed in pregnancies of 10 to 12 weeks, 42,7% of the uterotomies after the 12th week. Simultaneous sterilization was undertaken in 35,9% of the cases after vacuum-aspiration. The rate of complications of vacuum-aspiration was 25,8%, after uterotomy 16,5%.

[Therapy of dysmenorrhea with prostaglandin synthetase inhibitors: experiences with fluproquazone].

The prostaglandin-synthetase inhibitors fluproquazone and mefenamic acid were evaluated for relief of severe primary and secondary IUD-induced dysmenorrhea in a controlled double-blind cross-over study in 48 patients. Relief of dysmenorrhea was good to excellent in 48.7% of the treated patients and in 22.5% of the controls. This difference is statistically significant. It is concluded that fluproquazone and mefenamic acid are safe and effective in most patients for the relief of primary dysmenorrhea, and that prostaglandin-synthetase inhibitors are a suitable short-term therapy for this syndrome.

[Current diagnosis and therapy of gestational trophoblastic tumors: experience with 24 patients].

Observation of 24 gestational trophoblastic tumors, i.e. 20 hydatidiform moles, 1 invasive hydatidiform mole, 1 non-metastatic and 2 metastatic choriocarcinomas, is reported. The diagnoses were based on case record data, clinical findings, echography and radioimmunoassay of human gonadotropin (HCG). The therapy was oriented towards maintaining fertility in reproductive women. For treatment with cytostatic drugs the patients were allocated to a low risk and a high risk group according to history, diagnosis and former therapy, and an appropriate mono- or combined therapy was started. 17 of 24 patients were cured by surgery in an initial therapy phase. 7 tumors were progressive, 6 of which responded to secondary surgical or cytostatic treatment. One patient died from metastases of choriocarcinoma. The total cure rate of 96% was rendered possible by regular follow-up including bimanual palpation, echography, HCG-assay and chest X-rays.

[Urodynamic findings in anamnestic urge incontinence (author's transl)].

Cystometric finding of 93 urge incontinent and 157 not urge incontinent women were compared by evaluating the following 4 parameters: uninhibited detrusor contractions, first desire to void, maximum cystometric capacity and bladder compliance. Of these 4 parameters only uninhibited detrusor contractions were significant for the urodynamic diagnostic evaluation of urge incontinence. Bladder compliance especially showed no correlation to anamnestic urge incontinence.

[The clinical significance of the unstable bladder (author's transl)].

The anamnestic, clinical and urodynamic findings of 377 consecutively investigated women were evaluated. 86 (23%) patients showed bladder instability. The intensity of involuntary bladder contractions and the occurrence of incontinence in association with involuntary detrusor contractions influenced the clinical significance of unstable bladders. The maximum urethral closure pressure however had no influence on it. Urge incontinence was the most reliable symptom distinguishing those patients with bladder instability. Frequency of micturition, nocturia and nocturnal enuresis were less reliable in this respect. Urinary tract infections showed no correlation to bladder instability.

[Epizootiology of bovine cysticercosis in the Jindrichuv Hradec District].

The results of the study of the epidemiology and epizootology of Taenia saginata in the Jindrichův Hradec district indicate that this tapeworm species has adapted successfully to anthropogenic conditions. Mass occurrence of bovine cysticercosis, coming from a limited source, was observed in the Jindrichův Hradec district. Our of the 907 employees of one farm, T. saginata was found in two workers engaged in animal production and in one person working in crop production. Their recovery resulted in the eradication of cysticercosis in the cattle kept on that farm; before eradication the disease was responsible for losses of several hundreds of thousands of crowns.
