[The lower-limb amputation in geriatrics suffering from occlusive artery disease (author's transl)].

Owing to the biology of arteriosclerosis elderly previously affected patients have, as a rule, to undergo lower limb amputation for occlusive artery disease. This creates particular problems which must be taken into account when choosing the surgical technique and providing the prosthesis. The article surveys the common methods and describes, in particular, the knee disarticulation including the early prosthetic fitting carried out at the author's hospital, which in their experience can decisively improve the patient's rehabilitation.

[Swine liver ultrastructure following hypothermia and hyperbaric oxygenation preservation].

The livers of 20 pigs were preserved for up to 16 hours, applying hypothermia (4 degrees C) and hyperbaric oxygenation (3 ata). As a preparation solvent we used in one group the solution by Lie (17) which is rich in potassium and glucose, and in the other group the cryoprecipitated serum by Belzer (5). After 8 and 16 hours of preservation, the fine structure was examined light- and electronmicroscopically. After 8 hours of preservation, there were only little reversible changes in both groups. After 16 hours, both groups showed distinct changes of the endoplasmatic reticulum e. g. desorganisation, vesicular dilatation, and dissociation of the mural ribosomes. In addition, there were swellings of mitochondriae and Kupffer's cells. These changes were particularly distinct after preservation in cryoprecipitated serum. In this group a cellular depletion of glucogen was noticeable.

[Pathogenesis, clinical application and therapy of chylothorax (author's transl)].

With the growth of traffic density and the spread of thoracic surgery, the clinical picture of the chylothorax can be observed with increasing frequency. Above all, the spontaneous or symptomatic chylothorax presents diagnostic problems, since, in about 70% of the cases, it only represents the concomitant symptom of a malignant tumor. Only after exact diagnostics and the exclusion of a malignant primary disease can one be content with the cursory diagnosis of an idiopathic chylothorax. However, this requires strict postoperative control and observation of the patients. Two patients are presented in whom the genesis of the chylothorax still remained unclear 3 1/2 years after the successful ligature of the thoracic duct.

[History and technical development of hip joint replacement operations (author's transl].

The development of hip replacement operations is closely connected with the failures in operations to save the head of the femur. After many materially determined set-backs and errors due to false biomechanical ideas, there was renewed progress in allo-arthroplasty of the hip in the, 50's. But even newer models--like the Weber-Huggler-prosthesis-show that the technical development does not yet fully take into account the actual biological demands.
