Bone banking for transplantation in orthopaedic reconstructions.

Results of orthopaedic reconstructions in 1014 patients with the use of preserved allogenic bone grafts are presented. The method of grafts preservation seems to be of significance in surgical applications; deep-freezing is more favorable in more comparison with lyophilization. The result of treatment after bone transplantation is dependent on numerous biological and clinical variables.

Therapeutic effects of transplantation of lyophilized and radiation-sterilized, allogeneic bone.

Lyophilized radiation-sterilized bone graft operations were analyzed according to 27 variables in 1014 patients with two-year follow-up results. In 91.3% of patients, the result of treatment was "very good" (full-health restoration) or "satisfactory" (restoration with some small disorders). In 5.1%, the results were unsatisfactory. There was no clear association between the results of treatment and the patient's age. A definite correlation was found in preoperative diagnosis and end results. Better results were achieved in benign tumors and congenital malformations than in post-traumatic and degenerative conditions. Preferred anatomic host beds for transplantation were the coxa, arm, and vertebral column. Less satisfactory results were found in the leg and forearm. The number of good results of treatment declined with increasing duration of preoperative or chronic illness and with increasing postoperation time. Supplementary autogeneic bone did not seem to improve the end result. Concomitant disease was not always a contraindication for bone graft surgery. The significance of various traits in analysis of the final "result of treatment," "fitness for work," and "role of the graft" was evaluated with the aid of Pearson's indices of coincidence.

Aluminium release as a new factor in the estimation of alumina bioceramic implants.

Aluminium release from an alumina bioceramic was investigated parallel to standardized biocompatibility testing in an animal experiment. Alumina implants were introduced into both femurs of male rats and into the mandible of male guinea-pigs. After a period of 6 or 8 months animals were sacrificed. The bone surrounding the implant was examined by utilizing standard histological procedure. The material was well tolerated both in the rat and in the guinea-pig. The surface of the removed implants was examined in the SEM. No changes were observed. In both femurs of the 10 experimental and six control rats the aluminium content was determined by utilizing the atomic absorption spectroscopy method (AAS). It was found that the level of aluminium was essentially higher in the bones of the experimental animals. Therefore the safety of alumina introduction into human organisms for a long period becomes open to doubt, in spite of the favourable results of the short-term biocompatibility tests.

[Psychosocial conditions of attempted suicide among children and adolescents in the light of our studies].

The study comprised 30 children aged 12 to 17 years, including 22 females and 8 males, treated or consulted in the Psychiatry Department of the Medical Academy Institute of Pediatrics in Warsaw due to the realized trial of suicide. The psycho-demographical and the biological factors as well as direct causes preceding the suicidal trial have been analysed. It has been stated that the suicidal trials are dominated by the family conflicts and the bad emotional contacts among both the parents and children which in a majority of cases are associated by problems of school education. The cognitive functions of subjects studied were on the whole within the normal range.

Fitness for work after preserved bone transplantation.

The fitness for work in patients after preserved bone transplantation was analyzed. The analysis was based on clinical control of 1014 patients two years or more post surgery. It was found that 73% of all patients recovered fully their working ability. Important factors for working ability in such treatment are: age, diagnosis and localization of process, presumed role of transplant and duration of illness before operation.

Analysis of hyaluronidase diffusion for histochemical purposes.

Observations on hyaluronidase difussion from natural and artificial objects to precipitated medium are described. The conclusions from this investigation are: (1) hyaluronidase diffusing from biological objects in a precipitated medium is an active enzyme. (2) hyaluronidase diffusion may be utilized for localization of the enzyme. (3) certain parameters of the reaction hyaluronidase--hyaluronic acid may be measured with the use of the notion "diffusion radius".

Preserved bone in clinical transplantation.

Results of treatment by preserved bone transplantation in 552 patients are presented. Analysis is based on the activity of the Central Tissue Bank in Warsaw. Radiation-sterilization of lyophilized bone allografts is a routine method used in this bank. After long term observation of patients, the importance of age and diagnosis for clinical results of bone transplantation are discussed.
