[Cor pulmonale in patients with pneumoconiosis].

Of 577 patients with pneumoconiosis who died in 1964-1988 cor pulmonale was revealed on necropsy in 334, i.e. in 57.8%. In 120 it was the cause of death, i.e. in 20.7% (and it remains the most frequent cause of death). The mean age of the patients who died from cor pulmonale does not differ significantly from the rest of the group. The prevalence of cor pulmonale in different forms of pneumoconiosis did not differ significantly. Emphysema of the lungs was revealed in 71.2% of the patients who died from cor pulmonale. Embolization of the lungs was not more frequent in those who died with cor pulmonale; ischaemic heart disease was significantly less frequent in patients with cor pulmonale than in the rest of the group. Death from cor pulmonale is still the most frequent cause of death among occupational diseases.

[Immunohistologic detection of pepsin and gastricsin in carcinoma of the stomach].

Immunohistological tests for pepsin and gastricsin were carried out in bioptic samples of 51 patients with carcinoma of the stomach. Pepsin was detected in only 2 cases (4%), gastricsin in 26 patients (55%). Compared to the histological picture, the rate of incidence of gastricsin showed no difference between the intestinal and diffuse types of this tumour. In all examined cases, only gelatinous carcinomas were negative. Nor were any differences found in the cardia, the body or antrum. Surprisingly, these enzymes were found in the cytoplasma of neutrophylic granulocytes in the inflammatory infiltrate of the mucosal and tumorous stroma. Intestinal metaplasia of the epithelium was always negative even in the neighbourhood of positive tumours. The detected changes are evidence against the possibility of different histogenesis of the diffuse and intestinal forms of carcinoma of the stomach as well as against the possibility that intestinal metaplasia in a chronic inlammation of the stomach could be regarded as a direct first stage of the intestinal form of carcinoma of the stomach. In cases of metastases of unknown origin, pepsin and gastricsin cannot serve as markers due to their insufficient organ and cell specificities.

[Pepsinogen C in the human prostate].

The authors investigated the proteolytic activity of the normal prostate, the prostate with benign hypertrophy and with cancer. Pepsinogen obtained from the normal prostate was isolated on DEAHP pearl cellulose and purified by means of affinite chromatography. Separation on affinite chromatography and by electrophoresis on agar confirmed that the pepsinogen is identical with gastric progastricsin. The proteolytic activity was examined by a modified method of Anson and Mirsky, and with the aid of synthetic substrates it was assessed quantitatively in all three examined groups. The enzyme activity of the carcinomatous tissue practically did not differ from the activity of the normal prostate. In beniming hypertrophy of the prostate the proteolytic activity assessed by the breakdown of haemoglobin is markedly reduced and when tested on the breakdown of synthetic substrates [For-Tyr-Tyr- and Ac-Phe- Tyr (I2)] it is practically zero.

Lung cancer growing around a silicotic nodule.

The authors report on a patient, working for 17 years in ceramic industry, in whom simple silicosis was revealed in 1977. In 1985 the disease progressed and a nodule-like opacity in the left upper lung lobe was detected. After one year of further progression cavitation was found. Suspected cancer was confirmed cytologically by discovering epidermoid cancer cells in the lavage from the growing around the silicotic nodule. Lobectomy was performed, the postoperative course being without complications.

[Migration inhibition factor in patients with rare pulmonary diseases (author's transl)].

In 98 cases of pulmonary diseases (active and inactive sarcoidosis, idiopathic interstitial lung fibrosis [Hamman-Rich], collagenous diseases, Goodpasture syndrome, alveolar lipoid proteinosis, malignant lymphogranulomatosis and bronchial carcinoma) and in 18 tuberculin and varidase positive, Kveim negative control cases the migration inhibition factor (MIF) was investigated. Migration inhibition of macrophages was induced by the antigens tuberculin, varidase and Kveim's antigen and measured in Mackaness-chambers 24 hours later. Significant migration inhibition by the antigens tuberculin and varidase were seen in control cases, bronchial carcinomas and alveolar lipoid proteinoses. Increased migration indices frequently were found in interstitial lung fibrosis, Goodpasture syndrom and collagenous diseases. Kveim's antigen caused a significant migration inhibition in most cases of active sarcoidosis and in each one single case of collagenose and Hodgkin disease. The use of Kveim's antigen seem to have practical differential diagnostic value in this way.

The relationship between agoraphobia and primary affective disorders.

Detailed clinical records were kept on a series of 55 agoraphobic patients who presented to a general hospital psychiatric practice over a 3 year period. A review of the records revealed that 91% of these patients were diagnosed as suffering from a unipolar or bipolar primary affective disorder. The anxiety and phobic symptoms tended to mask the presence of the affective disorder. This observation is consistent with most of the published data on the agoraphobic syndrome. It could also explain the inconsistent effects of treatment of agoraphobia compared with simple phobias. The possible biological and psychological connections between primary affective disorders and the agoraphobic syndrome are discussed.

Effects of cycloheximide and streptovitacin A on protein synthesis and gastric secretion in rats.

Cycloheximide and streptovitacin A administered in vivo to rats display a similar dual effect on the labelling of soluble liver proteins by valine-14C, and result in a similar enhancement of liver uridine kinase activity. On the other hand, in pylorus-ligated rats, both antibiotics markedly depress gastric secretion, acid output, and the level of mucoproteins and proteolytic activity in secreted juice. Streptovitacin A on a molar basis was in all cases 5 approximately 8 times more effective than cycloheximide.

[Angiotensin converting enzyem (ACE) - a blood chemistry parameter in the diagnosis of sarcoidosis].

Serum angiotensin converting enzyme (ACE) activity was studied in 50 patients with sarcoidosis (39 active, 11 inactive sarcoidosis), as well as in 50 control patients (34 chronic lung diseases, 9 Hodgkin-disease, 7 rheumatoid arthritis). There is a significant difference (p less than 0.001) of ACE-activity between sarcoidosis patients and controls, and between active (without steroid treatment) and inactive sarcoidosis. Steroid treatment apparently lowers ACE-activity in sarcoidosis, however, without evidence for clinical improvement. Increased ACE-activity was also found in a patient with primary biliary cirrhosis.
