Eighty-seven fine needle aspiration cytological samples from 29 patients suffering from irregular perimenopausal uterine bleeding were evaluated. Aspiration cytology was performed prior to hysteroscopy, after distension of the uterine cavity and finally after uterine curettage. In this paper, cytological examination of fluid from the pelvic content in women with benign endometrial findings is compared with that of patients with adenocarcinoma. Endometrial curettage influenced the cell content of the cytologica specimens.
The incidence of invasive carcinoma of the cervix in the Czech Republic is higher than in Western Europe. The only effective test for early discovery of the precancerous lesions is cytodiagnostics--"PAP" smears. The study points to preventing mistakes in the cytodiagnostic process and stresses quality control according to the International Academy of Cytology (I.A.C.) and the European Federation of Cytological Societies (E.F.C.S.).

In 1995 75 years had elapsed since the time when the decision to found the Second Department of Obstetrics and Gynaecology was made by the Medical Faculty. An Institution arose which trained a great many qualified medical workers, midwives, students, and specialist obstetricians-gynaecologists. In the present article we should like to remember the teachers who have taken over the style of work from their predecessors and educated their pupils in the love and devotion to their discipline. In the last 50 years five Heads of the Department worked in the Second Department of Gynaecology and Obstetrics. Each of them made part of the history of the Department and imprinted there his or her characteristic image. Our wish is to acquaint you with the message of the predecessors as well as with the contemporary history and development of the Department in the field of education, research, and therapeutics.
The quality of cytodiagnosis depends on a number of operations. It is associated with collection of cellular material, it depends on its processing in the laboratory and on the evaluation of the processed preparation by the cytologist who establishes the diagnosis. In collaboration with three laboratories we tried to evaluate the role of the used nomenclature in the evaluation system. According to the abbreviated classification of Papanicolau 8185 cytological preparations were evaluated which comprised 206 suspect smears, i.e. 2.5%. According to the Munich nomenclature II 22 212 cytologies were evaluated. They comprised 155 suspect ones, i.e. 0.69%. Of 50 540 screening smears evaluated by the Bethesda system, 0.4% were suspect in the category HGSIL. The best results (i.e. agreement with the histological picture, the lowest false positivity and false negativity) was found in the evaluation of cytological smears according to the Bethesda nomenclature (TBS). It will be therefore most probably the best basis for so-called "Quality Assurance".
1. Cytological diagnosis of changes of the cylindrical epithelium in the endocervix has not been elaborated in such detail as the cytodiagnosis of squamous epithelium of the ectocervix. 2. The original proposal and experience of Vooijs of 1995 for the classification of incipient endocervical lesions corresponds best to histological practice. 3. Cytodiagnosis of endocervical changes will be more accurate if we use for evaluation the cellular pattern from the endocervix, demonstrated on the plates. 4. If we find any cellular abnormalities of the cylindrical epithelium, we recommend a procedure as used in lesions of squamous epithelium a) Low-grade abnormalities should be followed up primarily by cytological methods. If the lesion persists or progresses, histological examination is indicated. Examination of an endocervical lesion is usually not accessible to colposcopy. b) High-grade abnormalities and in particular those with the above described architecture, should be subjected to histological examination. 5. In mixed cylindrical and squamous lesions the procedure depends on the more serious lesion. In mixed lesions or suspected mixed lesions it is an advantage to use histochemical examination for betagalactosidase, the method described by Lojda, which differentiates safely the squamous and cylindrical component and in particular its tendency of malignant reversal.

1. Evaluation of cytological cervico-vaginal smears by the Bethesda system enlisted cytodiagnostics among important laboratory methods which can be used also in risk pregnancies. 2. Vaginal cytology makes it possible to test at the same time the hormonal situation during pregnancy, which reflects the placental function, and to evaluate also the vaginal biocenosis. 3. The authors provided evidence that the large number of superficial cells on the cytological smear (more than 10%) is associated with low oestriol and pregnandiol levels which are warning signs of the approaching termination of pregnancy. 4. By the action of microorganisms on the vaginal epithelium typical morphological changes develop in the cell nucleus and in the cytoplasm. By polychromatic staining also the causal agents of inflammations and infections threatening the mother and foetus are apparent. 5. The authors assume that cytological examination and evaluation according to the Bethesda system should be included in the complex of antenatal examinations also in women without clinical symptoms of premature delivery or without signs of vaginal infection.

Dynamic follow-up of cell-mediated immunity in women with malignant neoplasms of the ovary provides more valuable information on the course of the disease than a single value. The properties of leukocytes in patients with malignant ovarian tumours must be followed up on a longitudinal basis, as a signal of an approaching relapse of the disease is apparent sooner than its clinical manifestations. It appears that changes of biophysical mechanisms participate not only in the course of malignant ovarian disease but perhaps also in its genesis.

The authors operated 155 women where ovarian dysfunction was the cause of infertility. The first group was formed by 125 women with the Stein-Leventhal syndrome, the second group was formed by 30 women with dysgenesis of the gonads, karyotype, 46,XX. In the first group 97 of the operated women (77.6%) became pregnant. In the latter group after mere resection of the gonads 45 women (36%) became pregnant. After combined therapy (surgery and hormonal therapy) 52 infertile women (41.6%) became pregnant. Twenty-eight patients (22.4%) did not become pregnant. In the second group, i.e. in the group of gonadal dysgenesis 7 of 16 women (44%) became pregnant but only in the group of sclerocystic dysgenetic gonads. None of the women with streak or hypoplastic gonads became pregnant. In gonadal dysgenesis it is important to assess the quality of the follicular apparatus. For successful surgery of the ovary it is necessary to preserve a maximum of functional tissue and to use a careful surgical technique.
