On the premiss that an important aim of the initial phase of supervision of the neophyte group psychotherapist is to convert a new and threatening situation into an old and familiar one, the function of a preparatory interview--whose focus was the therapist's set of expectations about his impending experience of leading a group--was examined. The findings of a series of 26 interviews point to the need for a systematic controlled study to test the effects of a preparatory interview of the trainee therapist on group process and leadership behaviour.

Chlorpromazine (CPZ) 50 mg IM was administered to 21 schizophrenics receiving neuroleptic treatment. A further increase in plasma prolactin (PRL) was provoked in 8 subjects who had lower daily doses of neuroleptics than the remainder. The difference in the baseline PRL concentrations between the subgroups was inconsistent. In some patients, the test dose produced no change in plasma PRL but a further PRL rise was induced by raising the challenge dose or by an increase in the daily dose of neuroleptics. It is concluded that the PRL rise following CPZ 50 mg IM identifies some patients whose PRL response to the current treatment was only submaximal but the absence of PRL increment after this test dose is not sufficient evidence that the baseline level was already maximal.

A partially age-related admission policy coupled with a "single-ward" scheme for treatment and rehabilitation was introduced by the Hull geriatric department in 1970. With rare exceptions, elderly patients needing hospital care have been admitted directly to the geriatric unit, and the proportion of the retired population admitted by the general physicians has been greatly reduced. The proportion of inpatients needing continuing care has been reduced to less than 20%, the mean length of inpatient stay has fallen to under 30 days, and separate long-stay wards are no longer needed. More than 91% of patients are admitted without preceding domiciliary assessment, and only 5-6% of admissions are transferred from other units within the area.
Pachyonychia congenita is a genetic syndrome of epithelial dysplasia that is inherited as an autosomal dominant trait. Its unique involvement within the larynx of a 3-year-old boy prompted this brief report of its clinical behavior and management.
In a prospective controlled study of 290 men and women with uncomplicated gonorrhea, carbenicillin was shown to be less than 90% effective. Positive cultures were the basis for initial diagnosis and therapeutic failure in both sexes. Carbenicillin was evaluated in three different schedules: 5gm p.o., 10 gm p.o, and 2gm IM Plus 5GM p.o. The three schedules were equally effective. Eleven of 20 failures in women occurred in infection of the rectum. MICs of carbenicillin ranged from 0.025% to 1.0 mug/ml with a mean of 0.29 mug/ml.
Photodynamic action is an extraordinarily potent physicochemical process which is dramatically lethal for herpesvirus and other organisms in vitro. Recent studies in animal models and man indicate that the process is effective against in vivo skin infections as well. Although a great deal is yet to be learned about the application of photodynamic action to clinical disease, current experience indicates that photodynamic action will find a permanent place in the physician's therapeutic armamentarium.
