Typhoid fever complicated by intestinal perforation: a persisting fatal disease requiring surgical management.

In Bangladesh, clinical records of 323 patients with typhoid fever were reviewed to study the incidence, fatality, and optimal therapy of the complication of intestinal perforation. Fifteen patients (4.6%) developed intestinal perforation. Case-fatality rates were six of nine patients treated medically and one of four patients treated surgically for whom the postoperative courses were known. A literature review of 57,864 cases of typhoid fever in developing countries in the antibiotic era revealed that perforation developed in 2.5% of patients, a percentage that was similar to the incidence of 2.8% reported in the preantibiotic era. The median of case-fatality rates in these reports was 43% and the proportion of all reported typhoid deaths attributable to perforation was 25%. The case-fatality rates for patients with perforation were 70% for 410 patients managed medically and 26% for 1,835 patients managed surgically. Although some reports were biased toward placing patients at lower risk into surgical treatment, the large number of patients treated successfully by surgery suggests real improvement in surgical techniques in countries with endemic typhoid fever. These results indicate that intestinal perforation persists as a major cause of death in cases of typhoid fever in developing countries in the antibiotic era and that surgical treatment with use of antibiotic therapy is optimal for this complication.

Using primary-school children to improve child-rearing practices in rural Jamaica.

Many children in Jamaica suffer from poor development, inadequate dental care and low immunization status. There is a need for low-cost innovative programmes to reach these children. We piloted an approach to teaching primary-school children basic child health and development concepts with the aim of improving their knowledge and practices and those of their guardians. The programme was conducted with 89 children from three grade levels in a remote rural school. The topic was taught throughout the school year with an action-oriented curriculum. Evaluation showed improvement in school children's knowledge and practices and guardians' knowledge. This approach is seen to be a feasible, low cost way of improving the quality of future child care.

A child-to-child programme in rural Jamaica.

A child-to-child programme was carried out in six rural schools in Jamaica. School children in grades 4 and 5 were taught concepts of nutrition, environmental and personal hygiene, and child development. The children were encouraged to take messages home to their parents and improve the care of their younger siblings. Four hundred and twenty-three children and 90 mothers and guardians from four of the project schools were evaluated and compared with 199 children and 47 parents from two control schools. The project children showed higher scores on a test of knowledge in the topics at the end of the year. They also improved in some practices at home. The mothers improved in knowledge in some areas but not in their practices. We consider the results sufficiently encouraging to justify incorporating the programme into the country's education curriculum for primary schools.

Lipoperoxides in sebum of substance users and controls.

Sebum was collected from forehead skin in five compulsive heroin and/or cocaine (substance) users and in five controls over three consecutive periods, each lasting three hours. The participants were adult black and white men similar in age and smoking habits, who did not consume alcohol. Lipoperoxides were determined in sebum as malondialdehyde by high performance liquid chromatography. Two participants were excluded in the control group: in both, urinary lipoperoxides were elevated; in one, urine tested positive for delta-9 tetrahydrocannabinol (THC). All other participants had negative urine drug screening tests. Relative to the controls, all substance users had elevated concentrations of lipoperoxides in urine. Compared to the controls, the rate of sebum excretion in the last collecting period was higher in substance users, but sebum had significantly lower lipoperoxide concentration. It is assumed that compulsive drug use may influence lipoperoxidation of incipient sebum, possibly by altered tissue perfusion.

Diagnosis and treatment of sialolithiasis.

Sialolithiasis is the most common disorder of the salivary glands, and may range from tiny particles to several centimeters in length. While the majority of salivary stones are asymptomatic or cause minimal discomfort, larger stones may interfere with the flow of saliva and cause pain and swelling ... if left untreated, they can result in chronic sialadenitis and glandular atrophy. Conservative treatment may consist of oral analgesics and antibiotics. Surgical management may include salivary lithotripsy, basket retrieval and sialendoscopy.

A radioimmunoassay for tallysomycin.

Antibodies were raised against a new antineoplastic agent, tallysomycin, for use in a radioimmunoassay. The assay was sensitive to 2.5 ng of tallysomycin, had a coefficient of variation of 10.5% at 50 ng/ml, and was highly specific for tallysomycin A. The antibody exhibited reduced cross-reactivity with closely related tallysomycin analogs and little or no immunoreactivity with other antitumor antibiotics including bleomycin and phleomycin. Increased immunoreactivity occurred following copper chelation of tallysomycin B as compared with the non-chelated forms, thus indicating possible conformational alterations upon metal binding.

Diagnosis and treatment of renal and ureteral calculi.

Nephrolithiasis affects about 0.2% of the US population annually with about 5% to 15% of the population suffering at some point in their lives. While 75% of all calculi are calcium based, the remainder are composed of uric acid, cystine, struvite, or are composed of more than one substance. A 1997 policy update by the American Urological Association recommends that stones lodged in the ureter that are not causing excruciating pain for the patient are best initially managed by observation.
