Hostility and myocardial ischemia in coronary artery disease patients: evaluation by gender and ischemic index.

Two studies assess relationships of hostility to extent and severity of exercise-induced cardiac ischemia and daily life ischemia in patients with coronary artery disease (CAD). CAD patients (N = 80) who underwent exercise tomographic thallium testing to assess myocardial perfusion completed the Cook-Medley Hostility Inventory (Ho). A Composite Hostility (Chost) score obtained from the Ho scale was also used. Statistically controlling for the effects of gender in the full sample (N = 80), Chost was positively correlated with severity of perfusion defects (r = 0.22, p < 0.05). This relationship was nonsignificant in the 63 male patients, but was significant among women (N = 17, r = 0.42, p < 0.05) and among middle-aged men (less than 60 years, N = 17, r = 0.55, p < 0.05). In an overlapping sample of 42 CAD patients with ischemia (ST-segment depression > or = 1 mm) during 24 to 48 hours of ambulatory electrocardiographic monitoring, after controlling for gender, both Ho and Chost were positively correlated with total minutes of ischemia (r = 0.28 and r = 0.29, respectively, p < 0.05). In addition, there was a significant gender x hostility interaction, with stronger relationships with hostility evident for women. Results from these two studies indicate that in patients with coronary artery disease, hostility traits are significantly correlated with extent of daily life ischemia and with severity of exercise-induced myocardial ischemia. These relationships appear to be significant among female and middle aged male patients, but may be less evident among older male patients.

Cholesterol awareness and treatment in patients with coronary artery disease participating in cardiac rehabilitation.

OBJECTIVES: To survey cholesterol management practices among patients with coronary artery disease enrolled and not enrolled in cardiac rehabilitation. BACKGROUND: The National Cholesterol Education Program (NCEP) initially established guidelines regarding cholesterol awareness and treatment in 1987. Serum cholesterol reduction is most effective for reducing cardiac events in patients with established coronary artery disease, yet surveys of cholesterol awareness and treatment have not included these patients. METHODS: Three hundred seventy-nine men and women with coronary artery disease were surveyed according to cholesterol awareness, serum cholesterol level, frequency of lipid-lowering medication use, and frequency of achievement of a serum total cholesterol < 5.2 mmol/L (200 mg/dL) corresponding the 1987 NCEP guidelines for coronary artery disease patients, which were in place at the time of the study survey. RESULTS: Overall, 72% of the patients were aware of their cholesterol level, with an average serum total cholesterol of 5 5 +/- 1.0 mmol/L (213 +/- 39 mg/dL). Use of lipid-lowering medication was 26%. Forty-three percent had a total cholesterol < 5.2 mmol/L (200 mg/dL). Patients enrolled in a long-term cardiac rehabilitation program demonstrated enhanced cholesterol awareness (78%), lower total cholesterol values (5.2 +/- 0.9 mmol/L [203 +/- 36 mg/dL]), higher use of lipid-lowering the therapy (34%), and more frequent achievement of total serum cholesterol of < 5.2 mmol/L (200 mg/dL) (48%) compared to the other patient groups (all P < .05). CONCLUSIONS: Patients with coronary artery disease demonstrate relatively low rates of cholesterol awareness, lipid-lowering medication use, and achievement of total serum cholesterol < 5.2 mmol/L (200 mg/dL) corresponding to the 1987 NCEP guidelines. Participation in long-term cardiac rehabilitation is associated with enhancement of these rates. Further efforts to educate physicians and develop programs to optimize cholesterol management in patients with coronary artery disease are needed.

Fish trypanosomes: their position in kinetoplastid phylogeny and variability as determined from 12S rRNA kinetoplast sequences.

Fish trypanosomes have traditionally been classified according to the host species from which they were isolated, each isolate being regarded as a distinct species. To test the soundness of this practice, the genetic variabilities of the kinetoplast 12S rRNA-encoding genes of different fish trypanosomes isolates were compared. The DNAs were extracted from trypanosomes cloned from blood samples of 15 donors representing ten different fish species in four orders from waters of three major river systems of Central and Northern Europe. Comparison with other trypanosomatid sequences revealed that the fish trypanosomes form a monophyletic group with Trypanosoma brucei as a sister group. Pairwise comparisons of genetic distances yielded a wide range of continuous variation with no indication of any discontinuities attributable to barriers to gene flow. The genetic distances did not correlate with either the identity of the host species or geography. The host specificity of fish trypanosomes appears to be limited.

High frequency of Mlsa-reactivity among Ab-restricted, H-Y-specific T cell clones.

A high frequency (80%) of Ab-restricted, H-Y-specific T cell clones, but not of Ab-restricted LDH-B-specific T cell clones, was found to be Mlsa-reactive. This reactivity could be inhibited by class II (A and E)-specific antibodies, and required an E-positive stimulator cell. The proliferation was also blocked by T cell receptor-specific and by CD4-specific antibodies. Possible interpretations of these findings are discussed.

Comparative study of therapies for fibular ligament rupture of the lateral ankle joint in competitive basketball players.

This retrospective study compares the results of different therapies for fibular ligament rupture in a homogenous group of professional athletes. The endpoint "competitive sports" was an outcome consideration. Subjects were examined by means of a standardized questionnaire and a structured interview. One hundred and seventy-nine of the questionnaires were completed and returned for evaluation. All of the basketball players with severe ankle sprain (supination trauma with swelling, pain, and inability to bear stress) were included. Those players with fractures of the foot, pronation trauma, or additional distal fibula or tibia fractures were excluded from this study. Of the 179 basketball players 160 (89%) had suffered severe ankle sprain. The treatment was divided into three groups: primary surgery (N = 35), plaster cast (N = 39), and functional treatment (N = 89). While simple ligament injuries (Grade I and II) were mostly treated functionally, complex ligament injuries (Grade III) were usually operated on. A total of 119 (74%) of the players reported no further pain. For pain reduction surgical and functional treatments showed advantages over plaster treatment. In the surgical group 63% of the players judged their regained stability to be equivalent to that of their healthy leg. Only 50% of the players in the plaster and functional groups believed their ankle joints to have regained the same stability as before their injuries. Despite the achievement of good results through surgery, there were clear differences in the players' assessments of their performance in competitive sports. Most subjects (92%) did not have any problems in everyday life regardless of which kind of therapy had been chosen.(ABSTRACT TRUNCATED AT 250 WORDS)

Immobilization of human red blood cells.

Human red blood cells were immobilized in an alginate network which was cross-linked with Ca2+ ions. The immobilized cells were stored for longer than 5 weeks at 4 degrees C in an isotonic buffered NaCl solution containing glucose, inosine, adenine, and guanosine for energy supply. The immobilized cells were released from the alginate network by dissolving the matrix with citrate. Both the immobilized and released cells retain their biconcave shape over the storage period. Measurements of the released cell population in a hydrodynamically focussing Coulter Counter demonstrated that the mean size of the size distribution, the breakdown voltage, and the internal conductivity have not changed in contrast to control measurements on red blood cells stored conventionally in suspension indicating that immobilization preserves cellular functions.

Fetal distribution of cocaine: case analysis.

The implications of gestational cocaine use are widely documented. There is evidence that cocaine crosses the placenta in animals and humans; however, the fetal distribution of the drug and its metabolites has not been documented. We describe the distribution of cocaine and benzoylecgonine (BE) in the fetus of a cocaine user mother. Cocaine and BE were detected in brain, scalp, liver, kidney, heart, placenta, blood, and hair. The liver contained substantially more cocaine than other tissues, whereas the brain had more BE than any other sample.

Lung resection in a neoadjuvant protocol.

Neoadjuvant therapy was defined as a cytoreductive therapy administered prior to a definitive locoregional treatment--surgical resection. While surgical resection can be curative in patients diagnosed with localized early-stage non-small cell lung cancer, patients with nodal (N1, N2) disease are generally not candidates for exclusive surgery. Inductive therapy can improve the microscopic metastatic disease, eradicate the primary tumor and thereby improve survival.

Inversion lateral ankle trauma: differential diagnosis, review of the literature, and prospective study.

This study is comprised of 71 patients with inversion injuries. The purpose of this article is to present a logical and orderly approach in evaluating these injuries. A comprehensive review of the literature concerning diagnostic techniques and mechanisms of injury related to inversion ankle trauma is presented. Inversion ankle injuries are frequently misdiagnosed as simple sprains when frequently there are other pathologies present. The standard approach of evaluating inversion ankle injuries is often inadequate.
