Relationships between nitric oxide-mediated endothelial function, eNOS coupling and blood pressure revealed by eNOS-GTP cyclohydrolase 1 double transgenic mice.

Endothelium-dependent relaxation in conduit vessels is mediated largely by nitric oxide (NO), produced by the enzyme endothelial nitric oxide synthase (eNOS) in the presence of the cofactor tetrahydrobiopterin (BH4) and mediated through a cGMP-dependent downstream signalling cascade. Endothelial NOS regulates blood pressure in vivo, and impaired endothelial NO bioactivity in vascular disease states may contribute to systemic hypertension. In the absence of sufficient levels of the cofactor BH4, NO becomes uncoupled from arginine oxidation and eNOS produces superoxide rather than NO. The enzymatic uncoupling of eNOS is an important feature of vascular disease states associated with increased oxidative stress. However, whether eNOS coupling, rather than overall eNOS activity, has specific effects on endothelium-dependent vasorelaxation in vitro, or on blood pressure regulation in vivo, remains unclear. In this study, we evaluate the relationships between blood pressure and endothelial function in models of eNOS uncoupling, using mice with endothelium-targeted transgenic eNOS overexpression (eNOS-Tg), in comparison with littermates in which eNOS coupling was rescued by additional endothelium-targeted overexpression of GTP cyclohydrolase 1 (eNOS/GCH-Tg) to increase endothelial BH4 levels. Despite the previously characterized differences in eNOS-dependent superoxide production between these animals, we find that blood pressure is equally reduced in both genotypes, compared with wild-type animals. Furthermore, both eNOS-Tg and eNOS/GCH-Tg mice exhibit similarly impaired endothelium-dependent vasorelaxation. We show that reduced vasorelaxation responses result from desensitization of cGMP-mediated signalling and are associated with increased NO production rather than changes in superoxide production.

Endothelial nitric oxide synthase dysfunction in diabetic mice: importance of tetrahydrobiopterin in eNOS dimerisation.

AIMS/HYPOTHESIS: Impaired nitric oxide (NO) bioactivity and increased superoxide (SO) production are characteristics of vascular endothelial dysfunction in diabetes. The underlying mechanisms remain unknown. In this regard, we investigated the role of tetrahydrobiopterin (BH4) bioavailability in regulating endothelial nitric oxide synthase (eNOS) activity, dimerisation and SO production in streptozotocin-induced diabetic mice. METHODS: Mouse aortas were used for assays of the following: (1) aortic function by isometric tension; (2) NO by electronic paramagnetic resonance; (3) SO by lucigenin-enhanced chemiluminescence and dihydroethidine fluorescence; (4) total biopterin and BH4 by high-performance liquid chromatography; and (5) eNOS protein expression and dimerisation by immunoblotting. RESULTS: In diabetic mouse aortas, relaxations to acetylcholine and NO levels were significantly decreased, but SO production was increased, in association with reductions in total biopterins and BH4. Although total eNOS levels were increased in diabetes, the protein mainly existed in monomeric form. Conversely, specifically augmented BH4 in diabetic endothelium preserved eNOS dimerisation, but the expression remained unchanged. CONCLUSIONS/INTERPRETATION: Our results demonstrate that BH4 plays an important role in regulating eNOS activity and its functional protein structure, suggesting that increasing endothelial BH4 and/or protecting it from oxidation may be a rational therapeutic strategy to restore eNOS function in diabetes.

Deep venous thrombosis after total hip or knee arthroplasty in a "low-risk" Chinese population.

Scarcely any information has been published on deep vein thrombosis (DVT) in Chinese patients after total hip arthroplasty (THA) or total knee arthroplasty (TKA). However, generally, no prophylaxis is given to patients who do not have conventional high-risk factors because they are believed to be at "low risk." We performed a prospective study on 80 such "low risk" patients undergoing THA or TKA (58 TKA and 22 THA) without prophylaxis and performed duplex ultrasonography on both lower limbs 6 to 8 days after surgery. A total of 22 patients (27.5%) showed ultrasonographic evidence of DVT. Eighteen (31%) TKAs and 4 (18.1%) THAs were complicated by DVT. Three patients showed bilateral involvement, all of whom underwent TKA. Two patients had symptomatic pulmonary embolism. The sensitivity and positive predictive value of the clinical examination was 27.2% and 31.6%, respectively. This study showed that patients who are labeled "low risk" for DVT actually had a significant risk and suggests that the current practice of providing prophylaxis to only patients deemed at "high risk" should be revised.

Pancreatitis leading to retroperitoneal fibrosis and ureteric obstruction.

Two patients who had acute pancreatitis subsequently developed characteristic appearance on urography of smooth extrinsic narrowing and medial deviation of the right ureter suggestive of retroperitoneal fibrosis (RPF) resulting in ureteric obstruction. Both these patients had clinical, biochemical and sonographic evidence of acute pancreatitis. CT scan of the abdomen performed on the second patient also documented acute pancreatitis. Intravenous urograms were consistent with ureteric obstruction due to retroperitoneal fibrosis. Both cases were treated conservatively. They were well after an average of 20 months. These 2 cases illustrate the uncommon association between pancreatitis and RPF.

Pattern of sensitization to common environmental allergens amongst atopic Singapore children in the first 3 years of life.

This study was aimed to investigate the sensitization pattern to a range of common allergens in young Singaporean children. A cross-sectional study involving 75 children aged below 3 years was carried out. They presented between December 1995 and April 2000 with symptoms of asthma, rhinitis, eczema, or food allergy. Their levels of allergen-specific serum IgE to a panel of foods (egg white, milk, soy protein, shrimp, wheat and peanut), pet dander, dust mites and cockroaches were measured with Pharmacia CAP System radioallergosorbent test kits. Serum IgE levels greater than 0.35 kU/l represented a positive result. Four children could not be tested with the complete panel because of insufficient serum. The prevalence of sensitization was highest for cow's milk (45.9%) followed by egg white (38.7%), dust mites Dermatophagoides pteronyssinus (31.4%) and Blomia tropicalis (25.5%). Sensitization to ingested allergens was significantly more prevalent in children aged 1 year or younger than in the older children (70.4% of those below 1 year, and 50% of those aged 1-3 years; p < 0.02). Sensitization to inhaled allergens, such as dust mites, was more likely to manifest as respiratory symptoms (allergic rhinitis and asthma), while ingested allergens were associated with gastrointestinal symptoms and eczema (p < 0.001). It was concluded that infants and young children are at high risk of sensitization to common environmental substances. Allergen avoidance is therefore important even in the very young. The prevalence of sensitization to food allergens is higher compared to inhalant allergens in young children.

Direct amplification of Entamoeba histolytica DNA from amoebic liver abscess pus using polymerase chain reaction.

An important and serious complication of intestinal infection with Entamoeba histolytica is the involvement of the liver (hepatic amoebiasis). Hepatic amoebiasis is usually diagnosed by the clinical picture (pain in the right upper quadrant and fever), ultrasound examination and positive serology. However, none of these tests are definitive and the picture overlaps with pyogenic liver abscess caused by bacteria. It is for this reason that the feasibility of using polymerase chain reaction (PCR) for the detection of E. histolytica DNA in liver abscess pus was investigated. A comparative study was done to verify the sensitivity of ten pairs of primers specific for detecting E. histolytica in stools. Samples of liver abscess pus from 22 serology-positive patients were collected under ultrasound guidance; and these were used directly in PCR assays without any prior pre-treatment of the samples. Of the ten pairs of previously published primers tested, two pairs of primers (PI + P2 and P11 + P12) were found to give 100% sensitivity. Based on these results, we recommend that PCR assay can be successfully used to confirm the diagnosis of amoebic liver abscess with the primers identified.

Low level expression of hormone-sensitive lipase in arterial macrophage-derived foam cells: potential explanation for low rates of cholesteryl ester hydrolysis.

Conversion of arterial macrophages into foam cells is a key process involved in both the initiation and progression of atherosclerotic lesions. Foam cell formation involves the progressive accumulation and storage of lipoprotein-derived cholesteryl esters. The resulting imbalance in cholesterol metabolism in arterial foam cells may be due in part to an inadequately low level of cytoplasmic neutral cholesteryl ester hydrolase (NCEH) activity. In this study, we have demonstrated that hormone-sensitive lipase (HSL) mRNA is expressed at very low levels in macrophage-derived foam cells, using the unique approach of extracting mRNA from macrophage-derived foam cells purified from human and rabbit atherosclerotic plaques coupled with reverse transcriptase polymerase chain reaction (RT-PCR). We also demonstrate that macrophage-derived foam cells isolated from rabbit atherosclerotic lesions exhibit a resistance to high density lipoprotein (HDL)-mediated cholesterol efflux along with reduced levels of NCEH activity compared to lipid-loaded mouse peritoneal macrophages. Thus, low level expression of HSL may partially account for the reduced NCEH activity observed in arterial foam cells isolated from atherosclerosis-susceptible species.

Outdoor work and the risk of pterygia: a case-control study.

AIM: To investigate the relationship between outdoor work and pterygium formation in a hospital-based case-control study in Singapore. METHOD: Sixty-one patients with pterygium (49 males, mean age 54.2 years) and 125 controls (41 males, mean age 50.2 years), excluding patients with cataract and age-related macular degeneration presenting at outpatient clinics of the Singapore National Eye Centre were interviewed face-to-face with a standard questionnaire. Demographic data and information on outdoor work and eye protection, were collected. RESULTS: Thirty-eight subjects (20.4%) were outdoor workers and 148 (79.6%) were indoor workers. Outdoor workers were more likely to be male smokers and alcohol drinkers with lower family income and higher sunlight exposure than indoor workers. The crude odds ratio for pterygium in outdoor workers was 7.0 (95% confidence interval 3.2, 15.3). The adjusted odds ratio was 4.2 (95% confidence interval 1.7, 10.1). CONCLUSION: There is a statistically significant association between outdoor work and pterygium formation, which may be related to the higher exposure to sunlight and dust in outdoor workers. Efforts may be made to educate outdoor workers to wear proper equipment to protect their eyes against excessive sunlight.

Perceived masticatory function among elderly people.

This study investigated the status and associations of perceived chewing ability of a group of 891 elderly people, aged 55-91 years (average age 66 years). The majority of subjects (69%) considered that they could chew well. Only 9% reported poor chewing ability. This judgement was not associated with age. More men (11%) than women (6%) had reported poor chewing ability. Subjects with better perceived general and craniomandibular health reported better chewing ability. However, although related to permanent tooth loss, the perception of chewing ability was not seemingly associated with prosthetic rehabilitation. All elderly subjects with complete dental arches had no complaints of perceived chewing difficulty. Freedom from symptoms of craniomandibular pain when chewing, bilateral chewing activity and a wide choice of food range or textures were associated with favourably perceived chewing ability. This study identifies several suitable criteria for judging masticatory function and suggests that these may be used to complement chewing ability as an alternative measure of masticatory performance. These criteria are applicable to the elderly person, in particular, and warrant adequate clinical application.

Pregnancy screening by uterine artery Doppler velocimetry--which criterion performs best?

OBJECTIVE: To test whether repeating Doppler studies of the uteroplacental circulation late in gestation will improve the test's power for predicting pregnancy-induced hypertension and fetal growth restriction (FGR), and whether analysis based on a combination of quantitative and qualitative assessments of the uterine arterial waveforms will yield better results than analysis based on either alone. METHODS: A total of 358 patients considered to be at medium risk for the development of pregnancy-induced hypertension and FGR were recruited. Continuous-wave Doppler studies of the uterine arteries were performed serially at 20, 28, and 36 weeks' gestation. The values of various Doppler indices in the prediction of subsequent pregnancy complications were tested at different gestations and different cutoff levels. The overall significance of performance of the Doppler indices was assessed by the Cohen kappa index, which tests the extent of agreement between the test and the "truth" over that by random chance agreement. RESULTS: A total of 974 examination results on 334 patients were available for analysis. We found that Doppler studies of the uterine arteries at 28 and 36 weeks were less useful than studies performed at 20 weeks. Serial studies at 20 and 28 weeks showed only marginal improvement when compared with a single study at 20 weeks. The best criteria were a mean resistance index (RI) of uterine arteries above the 90th percentile and the presence of diastolic notches in both uterine arteries at 20 weeks. Although the overall kappa index only suggested fair to good agreement beyond chance, the positive predictive value for subsequent complications was good: 57% for severe complications and 93% for any complications. CONCLUSION: Doppler studies of the uterine artery as a test for the subsequent development of pregnancy complications are best performed at 20 weeks with a combination of RI measurements and the assessment of the presence of diastolic notches.

Symptoms of craniomandibular disorder among elderly people.

This study investigated the subjective symptoms of craniomandibular disorder and the restriction in maximal mandibular opening of 891 elderly Singaporean citizens. A distribution of 22% of subjects (20% of the women and 26% of the men) reported subjective symptoms of craniomandibular disorders. The most common single symptom reported was occasional to frequent clicking sounds from the temporomandibular joints (in 15% of subjects). The other single symptoms were reported only occasionally in 2-6% of subjects. Nineteen per cent of the subjects had some restriction in maximal interincisal opening (30-39 mm) and about 1% of subjects were severely restricted in maximal opening (less than 30 mm). Subjectively evaluated limitation of wide mandibular opening was recorded for only 2% of the subjects. Background factors included general musculo-skeletal problems (54%), perceived fair to poor general health (29%), headaches (33%), work-related stress (24%), oral parafunctional clenching (4%) and grinding activity (4%). This paper confirms earlier reports that the subjective signs and symptoms of craniomandibular disorder tend to diminish in elderly people.

Ossification of posterior longitudinal ligament of the cervical spine in non-Japanese Asians.

"The Japanese disease," ossification of the posterior longitudinal ligament, is not confined to the Japanese only. A similar incidence of 0.8% was found in this study among non-Japanese Asians. Of 5167 patients who attended the Mount Elizabeth Hospital in Singapore for cervical spine complaints, 43 patients were found to have ossification of the posterior longitudinal ligament, forming the largest non-Japanese series. All but one patient were of Mongolian origin, and males were affected four times more commonly than females. Diabetes mellitus was present in 16%. There was a significant association between ossification of the posterior longitudinal ligament and calcification of other cervical paraspinal ligaments. It is suggested that a generalized tendency to calcification may be an important etiological factor in ossification of the posterior longitudinal ligament. Four of the patients required surgery, and in our experience, anterior spinal fusion with removal of the ossified ligament or multilevel laminoplasty gives satisfactory results.

A comparative study of continuous- and pulsed-wave Doppler velocimetry in fetal and uteroplacental circulation.

Continuous-wave and pulsed-wave Doppler ultrasonography are the two common methods used to study the fetal and uteroplacental circulation. We used both methods in the same patient population in three different gestations and in various fetal and uteroplacental vessels to compare the results from both machines. Thirty-one normal patients were studied at 20, 28, and 36 weeks gestation. The results showed significant correlation with no statistically significant differences between the two types of machines for the peak systolic/end-diastolic ratios and resistance index for the umbilical artery and uterine arteries. Significant differences, however, were demonstrated when the arcuate artery was studied, or when the pulsatility index was used for comparison. We therefore conclude that when different machines are used for study, comparison of the results is only valid for selected vessels and indices. Results for arcuate arteries and pulsatility index should be compared with caution.

An unusual case of sacrococcygeal teratoma and a short review.

While sacrococcygeal teratoma presents typically at birth as a mass lesion, we had a case of late presentation with the classical triad of urinary retention, constipation and weakness of legs. Interesting enough, a tuft of hair together with a small lump was noted at birth and an associated small meningocoele was found at operation. We review the literature with regard to pathogenesis, clinical features, differential diagnosis, malignant change and treatment of the tumour.

Hormonal requirements for growth and differentiation of ob17 preadipocyte cells in vitro.

The ob17 cell line is a clonal line established from epididymal fat pads of C57 BL/6J ob/ob mice. After conversion into adipose-like cells, ob17 presents both the morphological and biochemical properties of mature rodent fat cells. The adipose conversion process is best represented by a stochastic model in which a pool of stem cells (adipoblasts) gives rise to clusters of adipose cells and to additional stem cells that remain in the population. The role of the different factors involved in the adipose conversion process of ob17 cells is discussed, i.e. 1) mitogenic factors, that enhance the number of committed cells (ACF or adipose conversion factor(s)) 2) lipogenic factors, that enhance the expression of adipocyte enzyme markers (insulin) and 3) adipogenic factors, that are obligatory requirements for adipose conversion (triiodothyronine, growth hormone and other pituitary factors).
