Pentobarbital anesthesia during the proestrous afternoon blocks lactotroph proliferation occurring on estrus in female rats.

Estradiol stimulates the synthesis and secretion of PRL and lactotroph proliferation, and its long-term administration induces PRL-secreting pituitary tumors. We examined changes in the number of proliferating lactotrophs throughout the estrous cycle in female rats and the involvement of the brain in the regulation of the lactotroph proliferation by anesthetizing with pentobarbital. Female rats revealing regular 4-day estrous cycles were injected ip with the thymidine analog 5-bromo-2'-deoxyuridine (BrdU) 3 h before decapitation to label DNA-replicating cells. Dispersed anterior pituitary cells obtained from these rats were stained for PRL and BrdU with a double labeling immunofluorescence technique. The rate of lactotroph proliferation was represented by the BrdU-labeling index (BrdU-labeled lactotrophs as a percentage of total lactotrophs counted). Lactotrophs from rats injected with BrdU at 1000 h showed a high BrdU-labeling index of 2.6% on estrus, whereas they showed almost undetectable levels of the BrdU-labeling index during the other stages of the estrous cycle. The anterior pituitary cells other than lactotrophs were scarcely labeled during any stages. The BrdU-labeling index began to rise by the midnight between proestrus and estrus, peaked between 0800 and 1200 h, and returned to undetectable levels by the midnight between estrus and diestrus I. Pentobarbital (35 mg/kg, i.p.) injected at 1345 h on proestrus, which was effective in blocking ovulation on estrus, eliminated completely the increase in the BrdU-labeling index as determined by BrdU injection at 1000 h on estrus. In contrast, the high BrdU-labeling index on estrus was partly suppressed to a level of 1.4% by pentobarbital injection at 0900 or 2100 h on proestrus. The rats injected with pentobarbital at 1345 h on proestrus did not show any increases in the BrdU-labeling index even after BrdU injection was delayed from 1000 to 1400 h on estrus. However, a high BrdU-labeling index of 3.7% was obtained in these animals when BrdU was injected at 1000 h on diestrus I. We conclude that 1) lactotrophs of cycling female rats proliferate selectively on the day of estrus; and 2) the proliferation of lactotrophs on estrus is not due to a direct action on the anterior pituitary of estradiol secreted from the ovaries but triggered by neural events occurring during the proestrous afternoon, which are closely related with the regulation of preovulatory surges of gonadotropin and PRL secretion.

Exercise-induced secretion of brain natriuretic peptide in essential hypertension and normal subjects.

To determine the major stimuli for the release of brain natriuretic peptide (BNP) and atrial natriuretic peptide (ANP), we measured their plasma concentrations in 14 normal subjects and 19 patients with essential hypertension during exercise with a bicycle ergometer. The plasma levels of both hormones at baseline were significantly higher in the hypertensive group than in the controls (p < 0.05). The exercise raised both the plasma BNP and ANP, with concomitant increases in systolic blood pressure (SBP), heart rate (HR) and plasma norepinephrine (NE) or epinephrine (Epi) in each group. In the controls the change in ANP correlated with those in SBP, HR and NE (p < 0.05), and similarly the change in BNP with those in SBP, HR, NE and Epi (p < 0.05). In multivariate regression analysis only NE was found to be a significant stimulus for ANP secretion, whereas SBP or Epi was related to BNP release. In the hypertensives the change in ANP correlated with those in HR and NE, but on multivariate regression analysis the change in ANP correlated only with that in HR. The change in BNP in the hypertensives correlated only with that in HR. These findings indicate that in normal subjects the exercise-induced release of BNP and ANP is more sensitive to a similar but slightly different sympathetic stimulus, whereas in hypertensives the major stimulus for the release of both hormones is heart rate, indicating that the mediators for BNP or ANP release are altered by some factors involved in hypertension.

Presence of alternatively spliced-estrogen receptor mRNA variants in normal human uterine endometrium and endometrial cancer.

Presence of alternatively spliced-estrogen receptor (ER) mRNA variants has been revealed in the breast cancer tissues. The ER variants transcribed from these mRNA variants were supposed to cause changes in the estrogen responsiveness of breast cancer. Although uterine endometrial cancer also has an estrogen-dependent profile, these ER mRNA variants have not yet been reported in the tumor. In the present study, we attempted to detect the exon 7 deletion- (del.7-) and exon 5 deletion (del.5) ER mRNA variants in normal human uterine endometrium (hEM) and uterine endometrial cancer tissue (hEC) by the use of reverse transcription-polymerase chain reaction-Southern blotting (RT-PCR-SB) with the PCR primers: hE4 (forward), hE6 (reverse), and hE8 (reverse), which were located in exons 4, 6, and 8, respectively. Two major products were generated from RNAs of both hEM and hEC with primers hE4 and hE8. The nucleotide sequence of the longer product was identical to exon 4-8 of human ER cDNA, whereas that of the shorter one completely deleted exon 7. Moreover, when the RT-PCR was done with the primers hE4 and hE6, the shorter product lacking exon 5 was detected with the longer one having the same sequence as exon 4-6 of human ER cDNA. Since the RT-PCR-SB with primers hE4 and hE8 produced a very low or undetectable level of the signals corresponding to del.5 ER mRNA variant, the level of del.7 ER mRNA variant seemed to be higher than that of del.5 ER mRNA variant.(ABSTRACT TRUNCATED AT 250 WORDS)

Ruptured subcapsular giant hematoma of the spleen as a complication of chronic pancreatitis.

We present a 63-year-old man with chronic pancreatitis and the rare complication of giant subcapsular splenic hematoma. The hematoma showed no size reduction for 6 weeks. Then, the hematoma was infected with pseudomonas aeruginosa after the recurrence of the pancreatitis, and it finally ruptured. This case suggested that in cases of giant subcapsular splenic hematoma with chronic pancreatitis reductive pressure treatment should be administered as early as possible.

Plasma adrenomedullin in various diseases and exercise-induced change in adrenomedullin in healthy subjects.

Adrenomedullin is a novel hypotensive peptide recently discovered in human pheochromocytoma. In the present study, we measured the plasma immunoreactive adrenomedullin of healthy subjects and patients with various diseases. Immunoreactive adrenomedullin was found to circulate in blood of the healthy subjects at a considerable concentration (3.3 +/- 0.3 fmol/ml). Plasma adrenomedullin was significantly increased in the patients with congestive heart failure (5.4 +/- 0.3 fmol/ml), essential hypertension (5.3 +/- 0.4 fmol/ml) and renal disease (4.9 +/- 0.4 fmol/ml). In healthy volunteers physical exercise significantly increased the plasma adrenomedullin concentration. The increase of adrenomedullin was inversely related to systolic blood pressure. These findings indicate that adrenomedullin participates in the circulation control in both physiological and diseased conditions. Although the exact origin of circulating adrenomedullin is still unknown, it is thought to be released rapidly by acute exercise, thereby regulating the cardiovascular system by its vasodilating activity.

[A successful case of ischemic cardiomyopathy associated with left ventricular aneurysm and mitral regurgitation].

A 67-year-old female patient underwent a left ventricular aneurysmectomy and mitral valve replacement for ischemic cardiomyopathy. The replacement was done through left ventricle rather than left atrium. She had heart failure of NYHA class IV despite aggressive medical therapy. Preoperative examination revealed a left ventricular aneurysm and mitral regurgitation of grade II with markedly reduced ventricular function (LVEF 17%). Mitral valve replacement through left ventricle preserving chorda of the mitral valve is a promising method to eliminate regurgitation and shorten the cardiac ischemic time.

Molecular forms of human brain natriuretic peptide (BNP) in plasma of patients on hemodialysis (HD).

Plasma brain natriuretic peptide (BNP) levels have been reported to increase in patients with heart failure and end-stage renal disease (ESRD); however, little is known about molecular forms of plasma BNP that increase in these diseases. In the present study, we analyzed the molecular forms of plasma BNP in ESRD patients of both before (pre-) and after (post-) hemodialysis (HD) state. The plasma extract was analyzed by gel filtration on a TSK-GEL G2000 SW column followed by a RIA for both BNP and atrial natriuretic peptide (ANP). In the pre-HD patients, a 14- to 2300-fold increase in plasma level of immunoreactive (ir-) BNP was observed when compared to normal controls. A ratio of BNP-32 to g-BNP (pro BNP) in plasma from the patients was much larger than that in plasma from normal subjects, indicating that the high plasma level of ir-BNP level in the patients on HD largely results from a marked increase in BNP-32. HD significantly (P < 0.01) lowered the plasma levels of both BNP-32 and g-BNP with a greater reduction in BNP-32 than in g-BNP. Whereas, a-ANP was a main molecular form of plasma ANP in both pre- and post-HD plasma. These results suggest that plasma BNP-32 plays an important role in the sodium-fluid balance and that secretion and metabolism of BNP may differ from those of ANP in the HD patients.

[Dysfunction of E-cadherin due to mutation of beta-catenin in a scirrhous gastric cancer cell line].

E-cadherin is a calcium dependent cell-cell adhesion molecule. In cancer tissue, detachment of the adhesion system is indispensable for invasion and metastasis of cancer cells. We have investigated mechanism of the dysfunction of E-cadherin-dependent cell-cell adhesion system in gastric carcinoma cells. Although the high expression of E-cadherin in a scirrhous gastric cancer cell line HSC-39, the function of E-cadherin was completely abolished. Western blotting of beta-catenin in HSC-39 cells demonstrated that a truncated beta-catenin was detected. The truncation was due to partial deletion of beta-catenin DNA. It was concluded that in HSC-39 loss of E-cadherin dependent cell-cell adhesion was due to mutation of beta-catenin gene.

[MR angiography of the venous system of lower extremities with gadolinium-enhanced fast spoiled GRASS].

MR angiography (MRA) of the venous system of the lower extremities was carried out in 10 patients with varicose veins using the Gd-DTPA-enhanced Fast Spoiled GRASS (FSPGR) method. The quality of imaging of the popliteal vein (n = 20), calf deep vein (n = 20), varicose vein (n = 12), and femoral vein (n = 6) were assessed in terms of four categories: excellent, good, marginally adequate and nondiagnostic. Nineteen popliteal veins, seventeen calf deep veins, and nine varicose veins were judged as excellent or good. All femoral veins were evaluated as excellent. Enhanced FSPGR was useful for the preoperative evaluation of patients with varicose veins.

[Hb M-Iwate [alpha 87 (F8) His-->Tyr]: analysis of the genomic DNA and biosynthesis].

Hb M-Iwate [alpha 87 (F8) His-->Tyr] was identified as the cause of cyanosis in a 21-year-old Japanese female. Amplification and sequencing of the alpha 2- and alpha 1-genes demonstrated the mutation CD87 CAC (His)-->TAC (Tyr) in the alpha 2-gene. Analysis of the in vitro globin biosynthesis in the reticulocytes disclosed a well-balanced beta/alpha synthetic ratio of 1.04 but an unexpectedly low alpha M/total alpha. Although the cause of the lowered alpha M-globin biosynthesis is not yet clear, it might be related to a defect in chain assembly rather than to a modified stability or a reduced amount of the abnormal alpha-globin mRNA.

Detection and quantification of soluble asialoglycoprotein receptor in human serum.

We describe the first evidence that soluble asialoglycoprotein receptors (AGPR) are present in human serum and that they are quantifiable by an enzyme-linked immunosorbent assay (ELISA). An affinity chromatography gel immobilized with monoclonal antibodies (McAbs) against human liver AGPR was mixed with normal sera, and the bound fraction was analyzed both by sodium dodecyl sulfate polyacrylamide gel electrophoresis and by Western blot analysis. Immunoreactive bands corresponding to 35 to 40 kd were obtained, which were lower than those of liver AGPR (41 kd and 46 kd). Soluble AGPR in human serum was able to bind to D-galactose-immobilized beads, indicating that the soluble AGPR remained ligand-binding activity. In order to quantify soluble AGPR, we established an ELISA using a monoclonal antibody (30220 McAb)-immobilized microplate and horseradish peroxidase-labeled F(ab')2 of another monoclonal antibody (30201 McAb). Reproducibility of intra- and interassay of the ELISA were 4% to 14% and 7% to 14%, respectively. Analytical recoveries ranged from 93% to 99%. The detection limit was estimated to be 0.1 micrograms/L. By nonparametolic analysis, a median and a 90% tile of serum AGPR level obtained from 283 normal volunteers were 0.4 micrograms/L and 2.4 micrograms/L, respectively.

[Effects of preserving mitral apparatus on left ventricular systolic function and geometry in mitral valve replacement].

We investigated the left ventricular (LV) systolic function and geometry in mitral valve replacement (MVR) with and without preservation of mitral apparatus. Five patients had conventional MVR without mitral annulus papillary muscle continuity (group C) and five patients had MVR with preservation of the continuity (group P). LV cineangiograms were assessed before and one month after surgery. LV volume was calculated by the method of Kennedy. Long axis dimension is defined as the distance between the apex and the center of mitral orifice in right anterior oblique projection. Short axis dimension is defined as the cross-sectional dimension at the middle point of the long axis. Paired Student's t tests were employed to compare the preoperative and postoperative data. LV ejection fraction decreased significantly from 69.4 +/- 9.5 to 51.8 +/- 10.5% in group C (p < 0.05) and did not change significantly in group P (from 51.6 +/- 19.9 to 56.8 +/- 12.9%). Fractional shortening of the long axis increased significantly from 15.0 +/- 11.8 to 21.2 +/- 14.0% in group P (p < 0.02) and did not change significantly in group C (from 16.5 +/- 8.6 to 13.1 +/- 3.3%). Ratio of LV end-systolic long axis to short axis dimension increased significantly from 1.76 +/- 0.31 to 2.12 +/- 0.21 in group C (p < 0.05) and did not change significantly in group P (from 1.90 +/- 0.22 to 1.95 +/- 0.22). We conclude that the ratio of systolic long to short axis increases, i.e., systolic LV shape changes to become longer and slender, and ejection fraction decreases after surgery in MVR without preservation of mitral apparatus. Conversely, preserving the mitral apparatus in MVR results in no significant change in systolic LV function and geometry, because of preserving fractional shortening of long axis.

[A case of RSD with complete disappearance of symptoms following intravenous ketamine infusion combined with stellate ganglion block and continuous epidural block].

A 74 year-old woman with a 6-month history of RSD following herpes zoster on her right arm was treated with stellate ganglion blocks (SGB), continuous epidural block (CEB) and continuous intravenous infusion of ketamine known as one of the NMDA receptor blockers. Of the symptoms of RSD, burning pain and hyperperspiration but allodynia disappeared after the treatment with SGB 8 times and CEB for 4 days. Allodynia disappeared completely after ketamine treatment, where ketamine was infused once using a subanesthetic dose for 2 hours. It is considered that ketamine is one of the useful drugs for the treatment of neuropathic pain with allodynia.

[What patients expect from home hospice medicine].

Over 60% of men and women with terminal illness wish for providing hospice care at home. The goal of hospice is to enhance the quality of life through relief of physical, emotional, social and spiritual pain. Hospice must be accepted by patients and family when their illness is without cure and hospice medicine is palliative. Providing patients with truthful information about their illness is essential for patients to control in determining how best to spend the remaining lives. Above described conditions allow patients choice, "informed choice". However as for Japanese concern, it is not yet sufficient enough about providing the patients and family what hospice medicine means with truthful information about illness. Therefore, the urgent needs enforce us to ask the questionnaire to let them understanding what is hospice medicine about and providing the patient and family with maximum self-determination. The results indicated that the most of men and women are expecting to exercise self-determination rights (over 80%) with their appropriate information to choice the specialized program of hospice medicine.

Nucleic acid sequence and DNase hypersensitive sites of the 5' region of the mouse progesterone receptor gene.

To allow study of the control of the promoter of the mouse progesterone receptor gene and its specific elements, we have isolated nine individual clones and selectively sequenced the 5' region, which includes 5' promoter sequence and the non-coding region. Sequence comparison was performed versus the progesterone receptor genes of other species. Moreover, potential estrogen response elements, progesterone response elements, and other transcription factor recognition sites were determined by sequence analysis. In addition, since it has been confirmed that chromatin structures are important for regulation of gene expression, we also examined them in different estrogen target organs using the DNase hypersensitivity assay. A different pattern of the DNase hypersensitivity sites was detected between uterus and mediobasal hypothalamus and these sites were close to specific recognition elements.
