Influence of standing on vestibular neuronal activity in awake cats.

Single-unit activity of vestibular nuclear neurons was recorded in chronically prepared, awake cats. To examine the influence of tonic activation of limb proprioception on vestibular function, the vestibular modulation by horizontal rotation (0.2 Hz, 17 deg) and head-tilt (0.1 Hz, 7 deg) was recorded in animals with freely hanging limbs and was compared with the modulation in standing cats. Of 29 examined cells responding to horizontal rotation, only about 30% were affected during standing, with most exhibiting a decrease of the mean discharge rate and gain. In contrast, about 70% of the 28 tilt-modulated cells showed pronounced effects during standing with a decrease of the gain and an increase of the mean discharge rate. The increase of the mean discharge rate in tilt cells may be caused by the excitatory spinovestibular afferent fibers or by the efferent vestibular system. For the observed inhibitory effects on the gain different mechanisms may be responsible: cerebellar inhibition and/or efferent vestibular receptor control. This control of labyrinthine information by somatosensory afferent fibers may serve for the stability of equilibrium in the moving animal.

Responses to corneal stimulation in vestibulospinal units of nucleus Deiters.

In precollicular decerebrated cats, the response patterns of antidromically identified vestibulospinal Deiters's neurons to stimulation of corneal receptors were investigated. These patterns were compared with the responses of the somatosensory receptors of the neck and limbs as well as with the vestibular input of the horizontal semicircular canals. Of the 162 antidromically driven Deiters's units, 23 were influenced, mainly bilaterally, from corneal receptors. Response latencies evoked by electrical stimulation of the cornea ranged from 6 to 16 ms (mean 12 ms). In 99 Deiters's cells, the influence from the limbs was examined: 51 revealed primarily an ipsilateral modulation from proximal joint receptors. Convergence with joint receptor input was found in 16 of the 18 corneally driven cells tested. Of the 115 cells tested, 15 neurons responded to neck rotation in the horizontal plane. A contribution from the lateral semicircular canals was found in 7 of the 101 examined Deiters's neurons. The connection of corneal receptors with the spinal motor system, via a vestibulospinal pathway, may mediate a nociceptive reflex protecting the eyes and the face.

The effect of measurement error on differences in hospital expenditures.

Differences in expenditures for medical care among various subgroups in the population according to age, income, residence, and ethnicity have important public policy implications. However, few efforts have been made to assess the extent to which these apparent differences among subgroups observed in social surveys may be influenced by measurement error. This paper provides a framework for examining the impact of various kinds of error including random error and biases. The framework is applied to data on hospital expenditures from a national survey of health services expenditure which includes in addition to consumer reports, verifying information from hospitals, physicians, and third-party payers who provided services to persons in the sample. Differential error according to demographic and social characteristics is estimated. The impact of such differential error on conclusions about the distribution of health resources is discussed.

[Atrioventricular block in gynecologic-obstetric surgery].

Delivery ending with cesarotomy in women with A.-V.-block is thought to be the least hazardous. In cases where cardiac syncope occured before delivery, temporary cardiac pacemaker is applied. A complicated course of gynaecologic operation in a patient with permanent pacemaker on demand is described.

[Did the introduction of rehabilitation improve the course and prognosis of myocardial infarct?].

Experience with early mobilization and rehabilitation of patients after myocardial infarction in a coronary care unit is presented. There was not visible unfavorable influence of the early rehabilitation in patients after overcoming the acute phase of myocardial infarction. Early rehabilitation had a positive influence on physical as well as on mental condition of patients and resulted in a shortened stay in hospital and duration of impaired working capacity.

[Echocardiographic evaluation of asynergy of the left ventricle in the early stages of acute myocardial infarct and its value in predicting infarct complications].

The authors report on their experience with the semiquantitative echocardiographic evaluation of left ventricular asynergy (group of 36 patients examined by two-dimensional echocardiography, dividing the left ventricle into 20 segments) in the early stage of myocardial infarction and its application in the prediction of complications of infarction. The authors evaluated asynegry using two parameters: RA (range of asynergy and IA (asynergy index) which characterizes the grade of ventricular affection. They describe the calculation of these parameters. The authors revealed a significant correlation (p less than 0.005) between RA or IA and the incidence of decompensation or cardiogenic shock. RA and IA values (median values and scatter) in three groups of patients (non-complicated infarction, complicated infarction and infarction with cardiogenic shock and death) give the opportunity of early stratification of patients with acute infarction. RA above 50% and IA above 20 indicate that the patients is at high risk. On the other hand, RA lower than 25% and IA lower than 5 suggest a minor infarction and probably an uncomplicated course of the disease. In the investigated group anterior infarctions affected a greater mass of the left ventricle than diaphragmatic ones and the higher death rate in this group (20% vs. 14%) was consistent with this.
