Comparison of standard and large forceps for transbronchial lung biopsy in the diagnosis of lung infiltrates.

In order to determine whether larger biopsy forceps improve the diagnostic yield over smaller standard forceps, we performed 30 transbronchial biopsy procedures in 28 patients with lung infiltrates using three types of instruments. We found that compared to the standard forceps specimens the large forceps acquired larger specimens in 22 of 29 procedures and that the alligator forceps acquired larger specimens in 7 of 10 procedures. However, in only 2 of the 29 procedures did the larger biopsy specimen alter the pathologic diagnosis. Four of the patients subsequently underwent open lung biopsy which did not alter the pathologic diagnosis rom the transbronchial biopsy procedure. We conclude that the large forceps do not significantly improve the diagnostic yield of transbronchial biopsy in our patients.

Effect of lung volume on collateral ventilation in the dog.

We studied the effect of lung volume on resistance through collateral pathways (Rcoll) and small airways (Rsaw) before and after the injection of methacholine into obstructed segments of intact dogs. Before methacholine, Rcoll decreased 15.0 +/- 4.9 (SE)% per cmH2O increase in transpulmonary pressure (Ptp) and Rsaw decreased 5.1 +/- 7.0 (SE)% per cmH2O increase in Ptp. Following methacholine, lung inflation resulted in similar decreases in Rcoll and Rsaw. The fall in Rcoll was significantly greater than the fall in Rsaw. When pressure in an obstructed segment (Ps) was increased with constant Ptp (nonhomogeneous inflation), Rcoll fell approximately half as much for each cmH2O increase in pressure compared to when Ptp was increased (homogeneous inflation). We conclude 1) that increases in lung volume have small effects on Rsaw so that there is a relative increase in flow through collateral channels serving obstructed poritons of lung and 2) that Rcoll is a function of the size of the obstructed segment that increases more under homogeneous than nonhomogeneous conditions.

Pathologic fracture of the clavicle caused by arteriovenous malformation.

An anusual pathologic fracture of clavicle was caused in a 48-year-old woman by an arteriovenous malformation. The lesion radiologically mimicked a neoplastic process. Arteriography disclosed the vascular cause. Treatment was achieved by interrupting the feeding vessels and resecting the involved bony segment.

Prevalence of antibody to the Norwalk virus in various countries.

Serum samples from children and adults from several countries were tested by radioimmunoassay for antibody to the Norwalk virus. Antibody was commonly found in adults from all the countries tested. Antibody appears to be acquired more rapidly in children from underdeveloped countries than in children from the United States.

Inhibition of the in vitro outgrowth of Epstein-Barr virus-infected lymphocytes by TG lymphocytes.

Human T lymphocytes subpopulations from subjects not acutely infected with EBV have been selected and examined for ability to inhibit the outgrowth of autologous B lymphocytes that have undergone in vitro infection with EB virus. The results show that only TG lymphocytes are inhibitory. TG lymphocytes from subjects who are EBV antibody-negative inhibit as well as those from subjects who have EBV antibody. TG lymphocyte populations, as well as other T cell fractions obtained from neonatal subjects, fail to inhibit the outgrowth of infected, autologous lymphocytes under the conditions tested. We propose that NK cells are responsible for the inhibitory effects described in this report.

Mycetomas in pulmonary sarcoidosis: non-surgical management.

The course and prognosis of all patients with mycetomas in the setting of pulmonary sarcoidosis from 1960 to 1978 were reviewed. Twelve patients were identified. All patients were managed conservatively without surgery or antifungal agents. Three patients died; however, only one death could be attributed to the presence of a mycetoma and all deaths were associated with severe pulmonary insufficiency. Episodes of both major and minor hemoptysis were managed successfully with supportive therapy alone in the remaining nine patients. Survival after the diagnosis of a mycetoma was 10-14 years in three patients, 5-7 years in three patients and 3-4 years in three patients. All nine surviving patients have required corticosteroids for control of their underlying sarcoidosis. Extracavitary invasive disease due to Aspergillus organisms was not seen in this group. In the setting of chronic pulmonary sarcoidosis with mycetoma formation, fatal hemoptysis is infrequent and episodes of hemorrhage may be safely managed with supportive medical therapy alone.

Pituitary-adrenal response to separation in mother and infant squirrel monkeys.

Pituitary-adrenal response in mother and infant squirrel monkeys following brief separation was assessed. Each mother and infant pair was tested under each of 3 conditions: (1) Basal; (2) Separation-Reunion; and (3) Separation. Samples were obtained from mothers and infants in the Separation and Separation-Reunion conditions 30 min following the initial disturbance. A similar paradigm was used to assess the effects of separation in surrogate-reared animals. The results indicate that mothers, infants, and surrogate-reared infants respond to 30-min separation with a substantial increase in plasma cortisol. The values obtained in the Separation-Reunion condition did not differ significantly from Basal values in any of the 3 groups, suggesting that the effect of separation on the pituitary-adrenal system is not due to the disturbance involved in the separation procedure. The response of surrogate-reared infants suggests that these infants develop similar attachments to the surrogate as normal infants to their own mothers.
