[Scintigraphy in cardiomyopathy].

The function of the right ventricle (RV) and left ventricle (LV) was evaluated in 32 patients with congestive cardiomyopathies (COCM), 10 patients with hypertrophic cardiomyopathy (HOCM) and 21 normal subjects (N). In all these patients myocardial perfusion was analysed using quantitative 201 Tl myocardial perfusion imaging. In COCM parameters of RV and LV performance were significantly reduced (p less than 0.01); regional wall motion analysis revealed a reduced radial shortening ability. Regional Tl-distribution was not different in COCM and N; however, relative Tl-uptake was significantly reduced (p less than 0.05). 80% of all patients with COCM showed the RV free wall on Tl-perfusion scintigrams. These scintigraphic aspects allow a noninvasive differentiation of a disturbed LV function and the classification of coronary and primary myocardial etiology. In patients with COCM the scintigraphically determined thickness of septum and free wall of LV was increased as compared to N (p less than 0.05). The ratio septum/lateral wall of LV averaged 1.3 +/- 0.20. Quantitative analysis of regional Tl-uptake revealed increased segmental relative Tl-uptake. In the majority of patients there was a disproportionate septal hypertrophy on the gated blood pool scan in LAO. These radionuclide techniques allow a dynamic two-dimensional evaluation of the interventricular septum and the LV.

[Occlusion of both femoral arteries in one case of ergotism (author's transl)].

In a 29 year-old female occlusions of both femoral arteries were caused by the administration of ergotamine in the form of Cafergot-suppositories. Angiographically long smooth-walled stenoses and occlusions over a segment of approximately 5 cm length in each superficial femoral artery as well as occlusions of both tibial arteries were demonstrated. Follow-up was carried out with oscillography, rheography, plethysmography and Doppler ultra-sound technique. A repeat angiogram 1 week after discontinuation of the drug and medical management with intravenous infusions of Ronicol and Valium revealed patent vessels of the lower extremities.

[Doppler ultrasound diagnosis of extracrânial carotid stenoses (author's transl)].

Percutaneous measurements by Doppler ultrasound techniques over the supratrochlear, supraorbital and common carotid arteries may detect the presence of a stenosis in the internal carotid artery between the bifurction and the branching of ophthalmic artery. Out of 200 patients examined by Doppler ultrasound 60 patients underwent carotid angiography and the angiographic data were compared with the noninvasive results. The diagnostic accuracy was 85% in cases with stenoses greater than 50% and occlusions; in cases with stenoses of greater than 50% it was 78%. Occlusions could always be verified. Doppler ultrasound is very valuable for follow-up examinations after reconstructive procedures of the internal carotid artery. In 30 patients we were able to objectively document the normalisation of circulation in successfully operated cases by pre- and post-operative examinations.

[Diagnosis of extracranial carotid stenoses using the Doppler sonography].

Percutaneous measurements by Doppler ultrasound techniques over the supratrochlear, supraorbital, common carotid and internal carotid arteries may detect the presence of a stenosis in the internal carotid artery between the bifurcation and the branching of the ophthalmic artery. Out of 550 patients examined by Doppler ultrasound 111 patients underwent carotid angiography and the angiographic data were compared with the noninvasive results. The diagnostic accuracy was 91% in cases with stenoses greater than 50% and occlusion; in cases with stenoses of greater than 50% it was 86%. Occlusions could always be verified. Doppler ultrasound is very valuable for follow-up examinations after reconstructive procedures of the internal carotid artery. In 70 patients we were able to document objectively cases by pre- and postoperative examinations.

Beneficial effect of PGI2 on circulating endothelial cells.

Endothelial damage is an early event in atherogenesis. Recent data present evidence that the counting of circulating endothelial cells in human peripheral blood may provide reliable information about endothelial integrity. We therefore assessed the influence of PGI2 on the number of circulating endothelial cells in patients suffering from peripheral vascular disease. As cigarette smokers and patients with hyperlipoproteinemia show an increased number of circulating endothelial cells, they were excluded from this investigation. PGI2 causes a certain drop in circulating endothelial cells. In parallel, a prolongation in platelet half-life can be noted. This is most pronounced at a daily duration of infusion between 4 an 12 h. A shorter duration of PGI2 therapy is less effective in reducing circulating endothelial cells. All of the three different doses used, however, were equally effective. Placebo controls were ineffective. These findings indicate that PGI2 might exert a rather long-lasting beneficial action on endothelial integrity and the hemostatic balance.
