Epilepsy and agitated delirium caused by an astrocytoma of the amygdala.

The clinical picture of a tumour involving the amygdaloid nucleus was characterized over a period of 6 years by generalised epileptic fits, then a state of agitated delirium with oneirism evolving during 2 months and leading finally to pulmonary embolism. The association of epilepsy and psychiatric disorders is the clinical picture usually observed in tumors of the limbic system but these symptoms are various and unspecific. The conventional neuroradiological procedures often fail to discover the lesion.

Polyneuropathy in Waldenström's macroglobulinemia. Deposition of M component on myelin sheaths.

A chronic sensory neuropathy was the initial symptom in a case of Waldenström's macroglobulinemia in a 53-year-old woman. Ultrastructural analysis of a biopsied peripheral nerve revealed a modification of the lamellar structure of the myelin sheath. Immunofluorescent microscopy showed IgM-positive deposits on the myelin sheath. The monoclonal globulin (M component) present in the myelin appears to be a causative factor in the neuropathy.

Cervical cord compression by solitary osteochondroma of the atlas.

A 28 year old woman presented a syndrome of slow spinal compression evolving over four and a half years at the level C1--C2 vertebrae due to a solitary osteochondroma of the posterior arch of the atlas which was excised by laminectomy. Radiographys of the skeleton did not show the presence of other osteogenic tumours. This is the fifth case of solitary osteochondroma to be published. Elsewhere, in six cases out of 14, the vertebral lesion formed part of generalised exostoses.

[Clinical and ultrastructural study of a case of human rabies].

A patient of Morracan origin, aged 26 years, who died as a result of rabies after a 15 day illness in August 1976 in Bordeaux. No evidence of contamination was found by history from the patient and his family and friends nor by examination. A cerebral biopsy carried out a few hours before death confirmed the diagnosis by immunofluorescence, whilst examination under the optical microscope demonstrated Negri bodies and ultrastructural studies revealed rabies virus. These findings are discussed and compared with a few other cases reported in the literature with electron microscope studies.

Orbital phlebography in the diagnosis of painful ophthalmoplegia.

Three cases of painful ophthalmoplegia have been described in which symptoms suggesting a tumor of the orbit justified neuroradiological assessment. Phlebography in each case revealed stenosis of the superior ophthalmic vein in its third portion, and non-opicification of the cavernous sinus. Hirtz incidences revealed contralateral cavernous sinus opacification and venous drainage through the coronary sinus. These neuradiological findings helped to differentiate this syndrome from other affections which have similar signs and symptoms.

[Familial amyloid neuropathy of Corino Andrade. Ultrastructural study of the peripheral nerve in 2 patients].

The authors describe two typical cases of Portuguese amyloid neuropathy in immigrants. One of the patients had been ill for only a short time while the other's condition had been developing over more than ten years. The first patient's neuropathy was characterized by a perforating ulcer of the foot and loss of sensation. The neurological disorder as well as the effect on general health were much more serious in the second patient. Neuro-muscular biopsy was carried out on each of these patients and revealed excessive endoneural amyloid and very severe lesions of nerve parenchyma mainly secondary to Wallerian degeneration. The results were compared with the few existing ultrastructural studies on the peripheral nerve in amyloid neuropathies.
